Trigeminal neuralgia is probably the most excruciatingly painful disorder to afflict mankind and is refractory to all conventional analgesic agents. Fortunately, a number of specific medical and surgical therapies are now available. An accurate diagnosis is thus essential for the successful management of patients afflicted with trigeminal neuralgia. This article reviews the signs and symptoms, natural history, and diagnosis of trigeminal neuralgia. A discussion follows on current views on its pathogenesis and on available medical and surgical treatment modalities.

Trigeminal neuralgia in its classic form is usually an isolated disease that occurs in sporadic fashion without epidemiological or genetic significance. This paper discusses a family with three nontwin sisters, all of whom have classic trigeminal neuralgia occurring in early middle age with symptoms severe enough to require medical and surgical treatment. These three sisters also have one aunt with a history of severe facial pain, although this has not been medically documented. Clustering of such cases in a given family has been reported before, but is rare. The pathophysiological implication…

Pimozide was compared with carbamazepine in a double-blind crossover trial in 48 patients with trigeminal neuralgia who were refractory to medical therapy. Pimozide treatment produced greater reduction in trigeminal neuralgia symptoms than carbamazepine treatment. All of the pimozide-treated patients improved, while only 56% of carbamazepine-treated patients were relieved of their pain. Although both drugs provoked some adverse effects, it was not necessary to interrupt the trial in any case. After this 24-week trial, all patients began receiving pimozide and were followed up according to an o…

Magnetic resonance (MR) imaging was used to evaluate the relationship of the cisternal portion of the fifth cranial nerve to surrounding vascular structures in six patients with documented trigeminal neuralgia and in 85 asymptomatic patients. MR imaging clearly demonstrated the course of the fifth nerve from its root entry zone (REZ) to the Meckel cave and its relationship to the surrounding vertebrobasilar system. In the six patients with trigeminal neuralgia, the presence of a vascular structure at the REZ of the fifth nerve was identified. In the 85 asymptomatic patients, examination of 170…

Results of 280 radiofrequency lesions on 229 patients with trigeminal neuralgia are presented with three months to eight years (average 3.8 years) follow up. The patients were aged from 18-91 years. There was a high overall success rate of 94%. The complication rate has been low, with sensory paraesthesiae the commonest (15%) and cranial nerve palsies very rare (2.4%) compared to other reported series.

Trigeminal neuralgia nearly always can be relieved initially with drug therapy. Long-term relief, however, is achieved in only 25 percent of patients. When medical therapy fails, surgery may be necessary. Percutaneous rhizotomy is an effective technique for pain relief, with few medical contraindications. Posterior fossa exploration and neurovascular decompression or sensory root sectioning is reserved for those patients who do not improve after rhizotomy and for selected younger patients.

A case of combined trigeminal and glossopharyngeal neuralgia is described. The superior cerebellar artery and normal choroid plexus compressed and indented the root entry zones of the trigeminal and glossopharyngeal nerves, respectively. Complete relief was obtained after microvascular decompression and resection of the choroid plexus.

A 73-year-old patient with obturator neuropathy presenting as disabling thigh pain is described. The clinical diagnosis was verified by electromyography. No indication for surgery could be found. The patient's discomfort was not influenced by common pain-killers, but was relieved by the antipsychotic drug Melleril.

From the correlation between pain and morphin-like peptides there comes the suggestion of an etiopathogenetic relation between essential trigeminal neuralgia and endorphins. The clinical and pharmacological observation utilizing Naloxone, a morphin-like substances antagonist, and the transcoutaneous electrotherapy there seem to exclude that this etiopathogenetic relation there can be.

Sodium valproate, which has been recently introduced for the treatment of epilepsy, was tried in 20 patients with trigeminal neuralgia. Six out of 20 patients had no attacks for six months to 18 months while, in three patients, the frequency and severity of attacks were reduced by at least 50%. Four patients responded well when sodium valproate was used in combination with other drugs. Six patients showed little or no response while one patient showed poor tolerance to sodium valproate.
