Movement disorders in Alzheimer's disease: more rigidity of definitions is needed.

Rigidity, slowness, gait impairment, and other disorders of movement accompany Alzheimer's disease (AD) at various stages of the illness. The presence of these so-called extrapyramidal features have been reported to predict disease prognosis and pathologic localization. Unfortunately, failure to accurately characterize the movement disorder, particularly to distinguish parkinsonism from cortically based motor disturbances (that is, paratonia, apraxia), makes the results of many published studies difficult to interpret. There is an important need to precisely characterize movement disorders in

When and how to investigate the patient with headache.

The very common symptom of headache most often has a benign cause and can usually be diagnosed following a thorough history and neurologic and focused general physical examinations. Because the potential etiologies can threaten life and neurological function, headaches provoke understandable concern on the part of the patient and health care provider. Thoughtful testing can exclude the worrisome underlying causes. This article reviews the neurological and general symptoms and signs that should prompt diagnostic testing and the specific diagnostic studies that are recommended. The worrisome fac

Nerve injury about the shoulder in athletes, part 2: long thoracic nerve, spinal accessory nerve, burners/stingers, thoracic outlet syndrome.

Nerve injuries about the shoulder in athletes are being recognized with increasing frequency. Prompt and correct diagnosis of these injuries is important to treat the patient and to understand the potential complications and natural history, so as to counsel our athletes appropriately. This 2-part article is a review and an overview of the current state of knowledge regarding some of the more common nerve injuries seen about the shoulder in athletes, including long thoracic nerve, spinal accessory nerve, burners and stingers, and thoracic outlet syndrome. Each of these clinical entities will b

Clinical diagnosis, testing, and electromyographic study in brachial plexus traction injuries.

A system for evaluation of patients with brachial plexus injury is based on careful observation, including the patient's history and a physical examination, a radiographic assessment, and electrodiagnosis. The clinical history and physical examination remain the cornerstone of this evaluation and their documentation is vital. Traction injuries, wherein there is a possibility of root avulsion, were examined myelographically and electromyographically at one month. Although they must be considered, the limitations of these methods of evaluation do not vitiate their usefulness. Modification of the

Secondary syphilis with acute meningitis and quadriparesis.

This report describes a case of secondary syphilis presenting with an atypical cutaneous eruption and acute spastic quadriparesis. The etiology of the cutaneous and neurologic manifestations was obscure until cerebrospinal fluid (CSF) studies revealed pleocytosis, elevated protein, and a positive CSF, VDRL, and fluorescent treponemal antibody (FTA). These findings, correlated with the histologic presence of plasma cells in the dermal infiltrate, indicated secondary syphilis with acute syphilitic meningitis as the cause of the patient's illness. Therapy with intravenous penicillin, 20 million u

Feasibility of using training cases from International Spinal Cord Injury Core Data Set for testing of International Standards for Neurological Classification of Spinal Cord Injury items.

STUDY DESIGN: Descriptive comparison analysis. OBJECTIVE: To evaluate whether five training cases of International Spinal Cord Injury Core Data Set (ISCICDS) are appropriate for testing the facts within the International Standards for Neurological Classification of Spinal Cord Injury (ISNCSCI) and could thus be used for testing its training effectiveness. METHODS: The authors reviewed the five training cases from the ISCICDS and determined the sensory level (SL), motor level (ML) and American Spinal Injury Association Impairment Scale (AIS) for the training cases. The key points from the tra

Clinical assessment of mobility and balance impairments in pre-symptomatic Huntington's disease.

BACKGROUND: Quantitative movement analysis can detect impairments in mobility and balance in persons with pre-symptomatic HD (pre-HD); however, it is not known whether clinical tests can also detect such impairments. Given the ease of administering clinical tests, and the importance of assessing mobility and balance, we examined whether clinical tests can detect motor impairments in individuals with pre-symptomatic HD. METHODS: Two clinical tests (Functional Reach Test [FRT] and Timed Up and Go [TUG]) and the Unified Huntington's Disease Rating Scale (UHDRS) were administered, along with quan

[Gait analysis of idiopathic normal pressure hydrocephalus].

Normal pressure hydrocephalus (NPH) is a clinical syndrome associated with dementia, gait disturbance and urinary incontinence. Gait disturbance is usually the initial sign and most important symptom, but its objective evaluation has not been established. We analyzed the gait of an idiopathic NPH before and after ventricular shunting with the gait analysis system. Before shunting, the stride was short and irregular, and the truncal movement was unsteady. Three-dimensional patterns of angular relationships between 3 joints, namely the ankle, knee and hip were small and irregular. The vector pro

Clinical diagnosis of spinal metastases: a narrative review.

Spinal metastases are a frequent and disabling manifestation of systemic cancer. Pain, neurological compromise, and functional decline dominate the clinical picture, yet diagnostic delays remain common despite widespread MRI availability. To synthesize contemporary evidence on the clinical diagnosis of spinal metastases, emphasizing context‑dependent reasoning, characteristic clinical patterns, key differential diagnosis, and the central role of structured clinical examination. The review integrates recent epidemiological data, highlighting the influence of tumor histology, frailty, and treatm

Model-Based Prediction of Motor Scores From Sensory Scores in the International Standards for Neurological Classification of Spinal Cord Injury (ISNCSCI): Implications on Motor Levels in Segments Without Clinically Testable Key Muscles.

BACKGROUND: In the International Standards for Neurological Classification of Spinal Cord Injury (ISNCSCI), motor levels are inferred from sensory levels for high cervical, thoracic, and low sacral injuries, as key muscles are only assessed in upper and lower extremities. This is known as the "motor follows sensory level" rule. OBJECTIVES: To develop regression models for estimating motor scores from sensory scores in segments without clinically testable key muscles and to validate the consensus-based "motor follows sensory level" approach. METHODS: A total of 6940 ISNCSCI examinations from

Clarifications of the Motor Level Definition in the International Standards for Neurological Classification of Spinal Cord Injury in Not Clinically Testable Myotomes.

BACKGROUND: In the International Standards for Neurological Classification of Spinal Cord Injury (ISNCSCI), two approaches for determining motor levels (MLs) in not clinically testable myotomes (C2-C4, T2-L1, S2-S5) are described: one where the motor level follows the sensory level (MFSL) and another deriving motor function from sensory function (MFSF). Their results differ when (1) all key muscles of an upper (or upper and lower) extremity are scored as intact, (2) sensation is not normal in key muscle segments, and (3) a contiguous region of normal sensation starts at T2 (or S2). OBJECTIVES

Can We Improve Our Routine Urological Assessment to Exclude Neurogenic Causes for Lower Urinary Tract Dysfunction? ICI-RS 2024.

AIMS: After presentation with urinary symptoms, an underlying neurological mechanism sometimes emerges subsequently. Increased awareness may bring earlier diagnosis, improving prognosis and outcomes. METHODS: A 2024 International Consultation on Incontinence Research Society think-tank considered the clinical pathway for identification of an undiagnosed neurological or autonomic contribution precipitating urinary symptoms, and the implications for prognosis. RESULTS: Alongside adult-onset neurogenic conditions, potential for missed diagnosis includes congenital and pediatric-acquired neuroge

Practical approach to the child presenting with acute generalised weakness.

Acute generalised muscle weakness in children is a paediatric emergency with a broad differential diagnosis. A careful history and neurologic examination guides timely investigation and management. We review some of the more common causes of acute generalised muscle weakness in children, highlighting key history and examination findings, along with an approach to lesion localisation to guide differential diagnosis and further investigation.

Association between falls and nonmotor symptoms in patients with Parkinson's disease.

INTRODUCTION: Parkinson's disease (PD) is a chronic neurodegenerative disorder. Falls are common in patients with PD and can lead to disability, bedridden status, and death. The mechanisms of falls induced by symptoms of PD have not been fully clarified. We investigated the association between falls and nonmotor symptoms in PD patients. METHODS: A total of 361 patients with Parkinson's disease were included. Whether the patients had fallen in the past half a year was recorded. Nonmotor symptoms were assessed by 30 items from the nonmotor symptom questionnaire (NMS Quest), Parkinson's Disease
