[Emergency anesthesia in a woman with mitochondrial neurogastrointestinal encephalopathy].

Mitochondrial neurogastrointestinal encephalopathy (MNGIE) is characterized by leukoencephalopathy, peripheral neuropathy, ptosis, ophthalmoplegia, and gastrointestinal dysmotility. Mitochondrial myopathies are rare diseases and little is known of how to manage them when the patient requires anesthesia. We describe the anesthetic procedure used during emergency surgery for megacolon in a 26-year-old woman with MNGIE. Variables monitored were electrocardiogram, invasive arterial pressure, oxygen saturation by pulse oximetry, end-tidal carbon dioxide pressure, neuromuscular block, and depth of a

Infantile postoperative residual curarization (IPORC) - A prospective observational study.

OBJECTIVE: Residual neuromuscular blockade (RNMB), defined as a train-of-four ratio (TOFR) <0.90, is a complication of neuromuscular blocking agents (NMBA). Data about RNMB in children are rare. This single-center observational trial evaluated the rate of neuromuscular monitoring (NMM), the incidence, and consequences of RNMB in pediatrics. METHODS: Children over 1 month undergoing elective and urgent surgery during core work hours receiving NMBA were included in an 84-day observation period. When the anesthesiologist decided to extubate, a blinded investigator measured the TOFR by acceleromy…

Heterotopic ossification in the setting of neuromuscular blockade.

OBJECTIVE: Heterotopic ossification (HO) is a disorder characterized by the formation of new bone in tissue that does not ossify under normal conditions. We report a series of 6 cases in which HO occurred in the setting of adult respiratory distress syndrome (ARDS). We wished to show that HO can occur after neuromuscular blockade and that these cases might provide additional evidence that HO is influenced by neural mechanisms. METHODS: Cases of HO were selected from the consultation services at the Massachusetts General Hospital and the Brigham and Women's Hospital. Affected patients all had

Effect of total intravenous anesthesia versus sevoflurane anesthesia on intraoperative facial nerve monitoring: a randomized controlled trial.

PURPOSE: Intraoperative facial nerve monitoring (IFNM) facilitates effective nerve preservation during ear and head and neck surgeries. Quantitative differences in the timely feasibility of IFNM during total intravenous anesthesia (TIVA) vs sevoflurane anesthesia have not been investigated. METHODS: We conducted a randomized controlled trial in which 98 patients undergoing ear surgery were allocated to either the TIVA or sevoflurane group. We used quantitative neuromuscular monitoring of train-of-four (TOF) responses to assess achievements of IFNM-feasible conditions, and recorded the TOF cou…

[Anesthetic management of patients with neuronal ceroid lipofusucinosis].

Neuronal ceroid lipofuscinoses (NCL) are in a group of autosomal recessive inherited neurodegenerative diseases characterized by the accumulation of autofluorescent storage material in many cell types. Clinical manifestations of NCL are progressive mental and motor deterioration, seizures, and visual loss. We report anesthetic management for two siblings with NCL. Placement of percutaneous endoscopic gastrostomy tubes were scheduled for both 31-year-old woman and her 29-year-old sister with NCL. Although they were treated with several anticonvulsants, the grand mal and myoclonic seizures persi

Benefit of Salbutamol for the Treatment of Neuromuscular Junction Dysfunction in Patients With Purine-Rich Element Binding Protein A (PURA) Syndrome.

Purine-rich element-binding protein A (PURA) syndrome is a rare neurodevelopmental disorder caused by pathogenic variants in the PURA gene and is characterized by neonatal hypotonia, feeding difficulty, respiratory dysregulation, and severe developmental impairment. The PURA gene has recently been identified as a cause of congenital myasthenic syndrome. We report two unrelated infants with genetically confirmed PURA syndrome who were treated with neuromuscular junction-directed therapy and reviewed three additional published cases. All five patients had neonatal hypotonia and apnea or respirat

Massive Valproic Acid Overdose With Confirmed Pharmacobezoar Formation and Neuromuscular Toxicity: A Case Report and Literature Review.

We present a 42-year-old man who arrived obtunded to our emergency department (ED) after ingesting 60 g of extended-release valproic acid (VPA). Imaging and endoscopy confirmed a pharmacobezoar, a rare complication of overdose. The patient was managed with early airway protection, gastrointestinal (GI) decontamination, intravenous (IV) levocarnitine (L-carnitine), naloxone, and continuous renal replacement therapy (CRRT). His initial hospital course was stable but later complicated by delayed neuromuscular toxicity with extrapyramidal symptoms (EPS) requiring inpatient rehabilitation and multid

Bilateral dilated nonreactive pupils secondary to rocuronium infusion in an ARDS patient treated with ECMO therapy: A case report.

RATIONALE: Pupil monitoring for neurologic examination has become a regular clinical practice during extracorporeal membrane oxygenation (ECMO) therapy. Sudden dilation of pupils always indicates a severe cerebrovascular event. However, bilateral dilated nonreactive pupils secondary to neuromuscular blockade are uncommon and widely ignored in adult acute respiratory distress syndrome (ARDS) patients. This is the first case report of bilateral dilated nonreactive pupils caused by rocuronium in an ARDS patient receiving ECMO treatment. PATIENT CONCERNS: Bilateral dilated nonreactive pupils were

Ellen Grass Memorial Lecture: Clinical Neurophysiology in the Treatment of Disease.

Clinical neurophysiology has a long-standing history and value in the diagnosis of neurologic diseases. Because of their unique ability to assess physiology and function of the nervous system, electroencephalography (EEG), evoked potentials, electromyography (EMG), and polysomnography have long been used in the diagnostic evaluation of epilepsies, demyelinating disorders, neuromuscular disorders, sleep disorders, and other diseases affecting the central and peripheral nervous systems. The role of these tests and clinical neurophysiology in general has usually ended upon diagnosis. New applicat

Ultrasound assessment of the diaphragm: Preliminary study of a canine model of X-linked myotubular myopathy.

INTRODUCTION: We tested the feasibility of using neuromuscular ultrasound for non-invasive real-time assessment of diaphragmatic structure and function in a canine model of X-linked myotubular myopathy (XLMTM). METHODS: Ultrasound images in 3 dogs [wild-type (WT), n=1; XLMTM untreated, n=1; XLMTM post-AAV8-mediated MTM1 gene replacement, n=1] were analyzed for diaphragm thickness, change in thickness with respiration, muscle echogenicity, and diaphragm excursion amplitude during spontaneous breathing. RESULTS: Quantitative parameters of diaphragm structure were different among the animals. W

Antagonism of phase II succinylcholine block by neostigmine.

The neuromuscular effect of neostigmine, 1.25 mg/70 kg, was assessed in 40 adult patients 10 min after cessation of a succinylcholine infusion. The patients had received a thiopental-nitrous oxide anesthetic supplemented by halothane or fentanyl during which they were given at least 5 mg/kg succinylcholine over more than 90 min. Train-of-four monitoring was used. Neostigmine accelerated recovery of neuromuscular function in all patients. The degree of recovery was directly related to the train-of-four ratio, and the results in patients who had received halothane were no different from those wh

Anesthesia information system-assisted detection and effective management of sugammadex-induced anaphylaxis: a case report.

BACKGROUND: Sugammadex, a neuromuscular blockade reversal agent, has a reported incidence of severe anaphylaxis of approximately 0.005% in Japan, typically occurring within 5 min of administration. However, detailed case reports describing the full clinical course and associated changes in monitoring parameters—such as vital signs, capnographic waveforms, and airway pressure—remain limited. CASE PRESENTATION: We report the case of an 80-year-old man who underwent laparoscopic partial hepatectomy under combined general and epidural anesthesia. Postoperatively, 200 mg of sugammadex was administe

[Research progress on biomarkers for the monitoring of Spinal muscular atrophy].

Spinal muscular atrophy (SMA) is the most common neuromuscular disease in children, which seriously affects children's health. At present, gene and molecular modification therapy for SMA have become hot spots. However, there are many uncertainties about when people with SMA should start treatment, how well the drugs can treat, and the prognosis. Therefore, reliable biomarkers for monitoring and evaluation are urgently needed. This review will summarize the progress made in SMA biomarker research in recent years.

Severe immune checkpoint inhibitor-induced 3M syndrome: a case report.

BACKGROUND: Immune checkpoint inhibitors (ICIs) have significantly improved survival rates in various advanced cancers, including cervical cancer. However, they can induce a range of immune-related adverse events (irAEs), with immune myocarditis being particularly dangerous, especially when combined with myositis and myasthenia gravis, forming the fatal 3M syndrome. Early symptoms are subtle, and delayed diagnosis and treatment often lead to severe consequences. CASE PRESENTATION: A 65-year-old female with stage FIGO stage IIIA cervical squamous cell carcinoma received one dose of anti-PD-1 m

Use of repetitive nerve stimulation in the evaluation of neuromuscular junction disorders.

Neuromuscular junction (NMJ) disorders are characterized by fuctuating muscle weakness. Acquired myasthenia gravis is the most common NMJ disorder with an overall prevalence in United States estimated at 60,000. Depending on the site of neuromuscular transmission failure, NMJ disorders have been classified as: (1) presynaptic (e.g., Lambert-Eaton myasthenic syndrome), (2) synaptic (e.g., cholinesterase inhibitor toxicity), and (3) post-synaptic (e.g., myasthenia gravis). Electrodiagnostic techniques used for investigation of NMJ disorders include repetitive nerve stimulation (RNS) and single f
