Prematurity is one of the most perplexing problems in perinatal care. Since the discovery of a surfactant deficiency as the primary etiology of hyaline membrane disease, attempts have been made to supplement surfactant in the premature infant. Surfactant replacement therapy is a promising new treatment for the premature infant with hyaline membrane disease. The history of surfactant replacement therapy, current trials of this therapy, nursing care of the infant receiving surfactant replacement, and future directions for nursing research in this area are discussed. Infants receiving this therap

There are skin rashes, lesions, and disorders in the neonatal period that require emergent assessment and consultation. A neonate presented with epidermolysis bullosa and provided an opportunity for learning about this condition and various dermatologic resources, primarily DermAtlas. The neonate with suspected epidermolysis bullosa that is delivered in a community setting usually requires transport to a neonatal intensive care unit with pediatric subspecialty care. A case study involving the transport of a neonate with this condition is reviewed, followed by a discussion of the etiology, inci

This retrospective study evaluated whether different reproductive biotechnologies influence neonatal behaviour parameters in foals. Data from 102 foalings in two commercial breeding farms were analysed, including foals conceived by artificial insemination (AI), conventional embryo transfer (ET) and intracytoplasmic sperm injection (ICSI). Neonatal parameters recorded were birth weight, time to achieve sternal recumbency, time to stand, time to nurse and time to meconium elimination. Mixed statistical models were applied to assess the influence of reproductive technique, breed, sex and farm. Si

Minimally invasive surgery is now an option to patients as light as 1.2 kg, extending the benefits of small incisions, faster recoveries, decreased pain, and more precise procedures in critically ill neonates. This article provides a review of the historical development of minimally invasive surgery and an overview of the equipment involved, identifies the roles of the neonatal and perioperative nurse, and briefly describes several minimally invasive surgery procedures currently performed in the neonatal population.

BACKGROUND: Educational practices and national guidelines for best practices of providing palliative care to children and their families have been developed and are gaining support; however, the dissemination of those practices lags behind expectations. Incorporating education for pediatric palliative care into nursing pre-licensure programs will provide guidelines for best practices with opportunities to enact them prior to graduation.
OBJECTIVE: To evaluate the effect of an integrated curriculum for palliative care on nursing students' knowledge.
DESIGN: Matched pretest-posttest.

BACKGROUND: Planetary health is an emerging field of growing global importance. Environmental changes increasingly threaten maternal and child health, while nurses and midwives play central roles across care processes. Despite these opportunities, planetary health education and training in nursing and midwifery are underdeveloped and a mapping of current approaches is critically needed.
OBJECTIVE: To map existing planetary health education and training initiatives for nurses and midwives and examine their reported educational and practice outcomes related to maternal and child health.

While overall neonatal mortality rates are improving in Ghana, the Ashanti Region has the highest mortality rate in the country. The clinical causes of newborn deaths are well known, yet local beliefs about illness aetiology, cause of death and care-seeking are less well understood. This exploratory qualitative study sought to understand how community members perceive and respond to neonatal illness. Researchers worked with public health nurses, community health nurses and opinion leaders in the Ashanti Region of Ghana to identify women who had lost a baby, either during pregnancy or after del

BACKGROUND: Continuing education is essential for healthcare workers. Education interventions can help to maintain and improve competency and confidence in the technical skills necessary to address adverse events. However, characteristics of the health provider such as age (related to more critical and reflexive attitude); sex (relationship with gender socialization), profession and work conditions might have an influence on the effect of continuing education efforts.
METHODS: A training in the management of obstetric and neonatal emergencies (PRONTO, Spanish acronym for Neonatal and Obstetri

OBJECTIVE: To assess the number of children born with microcephaly in the State of Paraíba, north-east Brazil.
METHODS: We contacted 21 maternity centres belonging to a paediatric cardiology network, with access to information regarding more than 100 000 neonates born between 1 January 2012 and 31 December 2015. For 10% of these neonates, nurses were requested to retrieve head circumference measurements data from delivery-room books. We used three separate criteria to classify whether a neonate had microcephaly: (i) the Brazilian Ministry of Health proposed criterion: term neonates (gestation

Neonatal listeriosis is rare and detecting more than one case together would be unlikely without a causal link. Thirty-five instances of neonatal listeriosis where cross-infection occurred in the UK and Ireland were reviewed together with 29 other similar episodes reported elsewhere. All instances comprised an infant who was ill at or within one day of delivery and who had direct or indirect contact with a second infant, or in the minority, two or more infants, who then usually developed meningitis 6 to 12 days later. In most instances, the infants were nursed on the same day in obstetric unit

BACKGROUND: Most parents would agree that the admission of an infant to the neonatal intensive care unit (NICU) is stressful. Existing research is focused on white mothers, with little known about the experiences of Spanish-speaking, Mexican American mothers who have an infant in the NICU.
PURPOSE: The purpose of this qualitative case study was to describe the experiences of a Spanish-speaking, Mexican American mother whose infant had been in the NICU.

PURPOSE: To determine whether developmental care interventions reduce neurodevelopmental delay, poor weight gain, length of hospital stay, length of mechanical ventilation, physiologic stress, and other clinically relevant adverse outcomes in preterm infants.
SUBJECTS: Infants born at less than 37 weeks postconceptional age. This review consisted of 31 studies in 4 categories of developmental care interventions, 19 subgroups, and multiple clinical outcomes. The total sample sizes in the individual studies ranged from 16 to 259; the sample size in 18 of the studies was less than 50.

BACKGROUND: There is renewed interest in second-stage labor practices as recent evidence has challenged historical perspectives on safe duration of second-stage labor. Traditional practices and routine interventions during second-stage have uncertain benefit for low-risk women and may result in cesarean birth.
PURPOSE: The purpose of this quality improvement project was to implement an interdisciplinary second-stage practice bundle to promote safe outcomes including method of birth and women's birth experience.
METHODS: Standardized second-stage labor evidence-based practice recommendations

Neonatal hypothermia poses an increased risk of infection, hypoglycemia, metabolic dysfunction, and mortality, particularly in preterm or low birthweight (LBW) infants. However, early detection of hypothermia and prompt thermoregulation can mitigate these effects thus, the need for continuous neonatal temperature monitoring. The BEMPU TempWatch is a small bracelet designed for continuous temperature monitoring for neonates. When the body temperature falls below 36.5˚C, the bracelet generates an alarm sound and flashes an orange light, indicating hypothermia. This study aimed to assess the vali

With the legalization of both medical and recreational use of marijuana in some U.S. states, nurses and other clinicians should be prepared to care for pregnant women who have used marijuana during pregnancy. This column describes the prevalence of cannabis use among women, the effect cannabis has on the body and the potential maternal, fetal and neonatal effects of marijuana use during pregnancy.

The perinatal nurse, in collaboration with physicians, can use deep tendon reflexes as a powerful tool in determining the need to start, adjust, or stop magnesium infusion. Toxicity can be detected using physical manifestations as a guide. Clinical signs may be a better indicator than serum levels of tissue levels of magnesium. Whether magnesium is given to prevent seizures or for tocolysis, patients in both situations are at risk for developing toxicity and must be assessed regularly to ensure patient safety.

BACKGROUND: Nasal continuous positive airway pressure (NCPAP) therapy is widely used in neonates, but the clinical practice varies. However, nursing practice differs among individuals, and an inappropriate application method may delay the respiratory therapy, influence the beneficial effect of NCPAP, and increase complications. We introduced a quality improvement project to expedite the application of NCPAP therapy and decrease the incidence of nasal trauma.
METHODS: A new strategy of mobile NCPAP cart with prepacked fixation kits and a written protocol was implemented from April 2006. All me

AIM: To describe the current state of evidence on the care of neonates with neonatal abstinence syndrome in countries with low to virtually no medical opioid analgesic consumption.
BACKGROUND: While access to opioids for medical use improves globally, misuse grows as a health concern. One unintended consequence has been an increase in the incidence of neonatal abstinence syndrome. Because most evidence is generated in countries with adequate opioid analgesic consumption, a picture of evidence in lower opioid-consuming countries is not available.
DESIGN: A scoping review methodology was emplo

Umbilical catheters have been used in NICUs for drawing blood samples, measuring blood pressure, and administering fluid and medications for more than 25 years. Complications associated with umbilical catheters include thrombosis; embolism; vasospasm; vessel perforation; hemorrhage; infection; gastrointestinal, renal, and limb tissue damage; hepatic necrosis; hydrothorax; cardiac arrhythmias; pericardial effusion and tamponade; and erosion of the atrium and ventricle. A review of the literature provides conflicting accounts of the superiority of high versus low placement of umbilical arterial

Central hypoventilation syndrome (CHS) is a disorder characterized by little or no ventilatory or arousal sensitivity to hypercapnia and variable reactivity to hypoxemia, with little or no hypoxic arousal responsiveness. CHS may be congenital or acquired and can be idiopathic or secondary to a known central nervous system abnormality. Infants often present with life-threatening apnea during quiet sleep and develop severe respiratory acidosis because of the inadequate response to hypercapnia and hypoxia. Long-term treatment usually involves use of mechanical ventilation during sleep.
