Identification and Localization of Functioning Gastroenteropancreatic Neuroendocrine Tumors Using [18F]F-NOTA-octreotide PET/CT.

Accurate identification and localization are crucial for determining surgical strategies in patients with F-GEP-NETs, as these tumors often … lized or yielded undefined findings on contrast-enhanced CT (CE-CT). In both instances, [18F]F-NOTA-octreotide PET/CT subsequently enabled precise identification and anatomical localization of the primary tumors. This molecular imaging technique leverages the overexpression of somatostatin receptors on the surface of most neuroendocrine tumor cells, providin …

Targeting neuroendocrine tumors with octreotide and lanreotide: Key points for clinical practice from NET specialists.

Octreotide and lanreotide are the two somatostatin analogs (SSA) currently available in clinical practice. They have been approved first to control the clinical syndrome (mainly carcinoid syndrome) associated with functioning neuroendocrine tumors (NET) and later for tumor growth control in advanced low/intermediate grade NET. Although evidence regarding their role, especially as antiproliferative therapy, has been increasing over the years some clinical indications remain controversial. Solicited by AIOM (Italian Association of Medical Oncology) a group of clinicians from various specialties,…

Meta-analysis: somatostatin or its long-acting analogue, octreotide, for prophylaxis against post-ERCP pancreatitis.

BACKGROUND: Acute pancreatitis is a most serious complication following endoscopic retrograde cholangiopancreatography (ERCP). Previous meta-analyses and randomized controlled trials have shown conflicting results regarding the preventive efficacy of somatostatin or octreotide for this complication. The aim of this study was to resolve these conflicts. METHODS: A standardized comprehensive literature search was performed through September 2009. Depending on heterogeneity of outcomes, either random-effects model (REM) or fixed-effects model (FEM) was applied to calculate pooled estimates of dr…

[Small bowel pseudo-obstruction revealing an early scleroderma. Long-term efficacy of octreotide and erythromycin].

Skin abnormalities are the most common manifestations in patients with systemic sclerosis but many other organs can be involved. We report here the case of a 61 year-old-man in whom small bowel involvement was the initial presentation of limited systemic sclerosis. The clinical features consisted of recurrent small bowel obstruction without any organic lesion. The patient responded well to erythromycin (125 mg thrice a day before meals) and octreotide (50 microg subcutaneously at bedtime) with complete symptom relief. This permitted recovery of nutritional autonomy for two years. This observat…

Prolonged large bowel transit increases serum deoxycholic acid: a risk factor for octreotide induced gallstones.

BACKGROUND: Treatment of acromegaly with octreotide increases the proportion of deoxycholic acid in, and the cholesterol saturation of, bile and induces the formation of gallstones. Prolongation of intestinal transit has been proposed as the mechanism for the increase in the proportion of deoxycholic acid in bile. AIMS: To study the effects of octreotide on intestinal transit in acromegalic patients during octreotide treatment, and to examine the relation between intestinal transit and bile acid composition in fasting serum. METHODS: Mouth to caecum and large bowel transit times, and the pro…

Localization of metastatic gastroenteropancreatic tumours by somatostatin receptor scintigraphy with [111In-DTPA-D-Phe1]-octreotide.

Most gastroenteropancreatic tumours express somatostatin receptors, allowing imaging using radiolabelled somatostatin analogues. Octreotide can be modified by coupling a DTPA moiety to the N-terminal D-phenylalanine to allow labelling with In111. We studied the comparative effectiveness of this radiopharmaceutical in identifying tumour extent. Twenty-two patients with metastatic gastroenteropancreatic tumours were scanned using [111In-DTPA-D-Phe1]-octreotide. In 11 patients with the carcinoid syndrome, one of six primary lesions was identified by CT scanning and by [111In-DTPA-D-Phe1]-octreoti…

A case of congenital hyperinsulinism with ABCC8 variants treated with octreotide long-acting release for four years.

Congenital hyperinsulinism (CHI) is a rare disorder resulting in hypoglycemia due to increased insulin secretion and is subdivided into focal and diffuse forms. CHI is associated with neurological damage due to hypoglycemia, making adequate blood glucose management crucial. Although subcutaneous octreotide injection can effectively treat CHI, frequent octreotide administration may be burdensome for patients and their families. Here, we describe a boy with early neonatal-onset persistent CHI treated with octreotide long-acting release (LAR). At first, he received treatment with continuous gluco…

A long-acting octreotide analog formed by conjugation with a peptide linker modified with two fatty acids.

INTRODUCTION: Current long-acting somatostatin analogs control acromegaly and neuroendocrine tumor symptoms, but rely on depot formulations requiring large-bore needle administration, often associated with injection-site pain, nodules, and variable drug release. To address this limitation, we developed TE-8214, a depot-free octreotide analog conjugated to a soluble, dual-fatty-acid albumin-binding module designed to enable prolonged systemic exposure while maintaining a fully aqueous formulation suitable for subcutaneous administration through a fine 30-gauge needle. METHODS: TE-8214's bindin…
