Primary and secondary orbit surgery: the transconjunctival approach.

The transconjunctival approach to the orbit is underutilized because of concern regarding inadequate exposure and higher postoperative rates of lower eyelid shortening and ectropion. All patients who had a transconjunctival incision performed for orbital surgery over the last 6 years (1990 to 1996) were studied. Patients who had a transconjunctival blepharoplasty were excluded. A total of 35 patients, average age 32 years, had 45 transconjunctival incisions performed. Lateral canthotomy or cantholysis was not done. Operations fell into three categories: fracture plating alone, 10 (22 percent);…

Osteosarcoma of the orbit associated with Paget disease.

Osteosarcoma is a common primary bone malignancy most often involving the long bones and occurring in the second decade of life. Orbital involvement in this disease process is rare and is usually due to extension of the tumor from an adjacent sinus cavity. Orbital osteosarcoma arising from preexisting Paget disease is exceedingly rare. We report the case of a 78-year-old female patient with Paget disease of the forehead who presented with rapidly increasing bilateral proptosis and visual loss for several months. Examination showed bilateral aphakia, decreased motility, proptosis with resistanc…

Wooden foreign particles in the orbit--spontaneous recovery.

A six-year-old boy consulted an ophthalmologist, because another boy had hit his right eye with a tree branch. There were slight corneal erosions, and a superficial small wound in the oedematous upper lid. The erosions healed in 2 days, but the lid showed progressive ptosis and oedema. Computed tomography revealed a hyperdense, tight structure, which was suspected to be a foreign particle. An orbital exploration was made three times, but the foreign body was not found. After prolonged suppuration, two wooden particles appeared spontaneously 4 and 5.5 months after the trauma. After that, the su…

Bronchopulmonary atypical carcinoid tumour metastatic to the orbit.

BACKGROUND: Bronchopulmonary carcinoid tumours metastatic to the orbit are rare. A case is presented demonstrating presentation, histopathology and treatment. METHODS: A 64-year-old man with a history of bronchopulmonary atypical carcinoid presented with an orbital mass. The nature of this metastasis was confirmed with histology and it was surgically excised and, in addition, local radiotherapy and chemotherapy were administered. RESULTS: There was a good response to the treatment for the orbital mass but other metastases developed, causing death. CONCLUSIONS: In this case of atypical carci…

Color Doppler imaging of the eye and orbit.

1. The use of color doppler imaging (CDI) in ophthalmology is a new and exciting area in vascular imaging. The physiologic information obtained and the anatomy evaluated has been inaccessible by any other noninvasive or invasive technique. 2. Although CDI for ophthalmic use does not, as of this time, have FDA approval, clinical trials have begun. 3. It is our expectation that CDI will be an accurate, reproducible, noninvasive method to assist ophthalmologists in the diagnosis and treatment of patients with orbital and ocular vascular disease.

[Colobomatous cyst of the orbit in congenital microphthalmos].

Colobomatous orbital cyst with microphthalmos is an anomaly of the orbital contents occurring during embryogenesis. In this study, we report the case of a 3 year-old child referred with the initial diagnosis of voluminous orbital angioma. Radiological investigations and histological study of the ablated mass established the correct diagnosis of colobomatous cyst. Pathogeny, diagnostic and therapeutic problems are discussed.

Grease gun injuries to the orbit and adnexa.

The grease gun injury is an uncommon injury to the orbit. However, high-pressure injection of grease, oil, or paint is frequently reported in injuries of the hand and usually affects the fingers or palm, which require complicated surgical and medical care. Two cases of grease gun injury to the orbit are reported: one, a catastrophic injury, and the other resulting in satisfactory recovery after surgical debridement and adjuvant medical treatment. Presentation, investigation, and therapy are discussed.

Color Doppler ultrasonography of the eye and orbit.

Color Doppler imaging is the recent development in ultrasonography that allows simultaneous two-dimensional structural imaging in Doppler evaluation of blood flow. With this technique, it is possible for the first time to indirectly display the fine orbital vessels such as the ophthalmic artery and its branches, the central retinal artery, the posterior ciliary artery, and the lacrimal artery. Also, the display of venous structures such as the superior ophthalmic vein, the vortex vein, and the central retinal vein is possible. In addition to the qualitative display of those vessels, the hemody…

Tumours of the eye and orbit in Ibadan.

Four hundred and forty ocular and orbital tumours seen in Ibadan over 11-year-period (1980-1990) are reviewed. Three quarters of the 252 patients who had enucleation had malignant neoplasms, of which 55% were retinoblastomas. The mean age of patients with retinoblastomas (3.2 years) was less than that of patients with carcinoma and malignant melanomas (47 years). Malignant melanomas were rare neoplasms in Ibadan. The commonest orbital lymphoma was Burkitt's lymphoma. Only 24% of the enucleated eyes had inflammatory conditions, of which 38% gave a positive history of trauma. Inflammatory lesion…

Metastatic tumors to the orbit--management and prognosis.

In a retrospective study the records of 34 consecutive cases with tumors metastatic to the orbit were reviewed with special attention to the management and prognosis of these patients. The primary tumor site was the breast in 20 patients, prostate in 5, kidney in 2 and skin in 2. The remaining 5 patients had other primary tumour sites. The symptoms of orbital metastasis preceded the detection of the primary tumor in 8 cases (24%). Twenty patients died after a mean interval of 25 months following the diagnosis of the orbital metastasis. This study gives a summary of the clinical features of the…

CT and MR imaging of the pediatric orbit.

Orbital abnormalities encountered in the pediatric population differ substantially from those found in adult patients. Retinoblastoma, the most serious intraocular tumor, is often difficult to diagnose, but use of computed tomography (CT) (which reveals the characteristic focal calcification) and magnetic resonance (MR) imaging allows this tumor to be differentiated from pseudogliomas, such as Coats disease, and retrolental fibroplasia. CT and MR imaging help in the differentiation of orbital cellulitis from preseptal, lacrimal, and eyelid infectious processes and of orbital pseudotumor from G…

Isolated peripheral nerve sheath tumours of the orbit.

Fifty-four cases of peripheral nerve sheath tumours within the orbit are reviewed. Benign neurilemmomas or neurofibromas occurred in 93% of patients and had an equal incidence. There was a family history or other signs of systemic neurofibromatosis in one-quarter of those with solitary neurofibromas, but in none of the patients with neurilemmoma. Symptoms and signs were generally those of an orbital mass lesion and pain or sensory loss was unusual, occurring more frequently with malignancy. Within the orbit, most peripheral nerve sheath tumours affect the first division of the trigeminal nerve…

[Congenital serous cyst of the orbit with anophthalmos].

The authors reported a case of orbital congenital serous cyst with anophtalmos. It was a third type of Duke Elder classification. Although it looked sporadic, this malformation is hereditary and due to a disharmony between optic cup growth and differenciation. Ultrasonography and scanner could confirm the diagnosis and helped to discard orbital and eyelid angioma and meningocele. After excision, a prosthesis must be adapted in the orbit.

Concomitant lymphangioma and arteriovenous malformation of the orbit.

An 8-year-old girl had an orbital-adnexal lymphangioma and ipsilateral orbital and middle cranial fossa arteriovenous malformations. High-resolution magnetic resonance image scanning, orbital ultrasonography, and digital subtraction angiography were used for diagnosis and preoperative assessment. Complications related to this vascular neoplasm included amblyopia, acute hemorrhage with proptosis, exposure keratitis, cosmetic deformity, and recurrent preseptal cellulitis. The girl was treated with both embolization and orbital surgery for recurrent hemorrhage and proptosis. We postulated that th…

[Injuries of the orbit caused by foreign body].

Four cases of injury of the orbit by a big foreign body without damage to the bulbus are described. The bodies were extracted and the healing was without complications. Two of these bodies penetrated into the brain and in one case surgical intervention was necessary. In both instances vision was lost because of damage to the optic nerve.

Epidermoid cyst and cholesterol granuloma of the orbit.

Epidermoid cyst and cholesterol granuloma have been confused, but these are different lesions. A case of recurrent orbital epidermoid cyst and another case of orbital cholesterol granuloma are presented in order to describe their distinct pathological, clinical and imaging characteristics and their surgical management. Advanced investigation using our custom-designed three-dimensional imaging allowed a better appreciation of the exact location, and optimal planning of the surgical treatment. The same surgical approach was used for both lesions: total resection via a coronal incision and immedi…

0.6 T magnetic resonance imaging of the orbit.

Magnetic resonance (MR) imaging of the orbit was performed with a 0.6 T superconducting imaging system in 100 patients with normal orbits who were being evaluated for brain pathology and in 21 additional patients with a variety of orbital lesions to determine the efficacy of MR imaging in displaying orbital abnormalities. Usually, MR studies were performed using a multislice technique with multiple spin-echo pulse sequences and 30, 60, and 90 msec echo times and 500, 1500, and 2000 msec repetition times. Using section thicknesses of about 8 mm, imaging was performed in the transaxial, coronal,…
