Radiological and functional outcomes 2.7 years following conservatively treated completely displaced midshaft clavicle fractures.

OBJECTIVES: It is unclear whether all completely displaced midshaft clavicle fractures require primary surgical intervention. The aim of this study was to elucidate the radiological and clinical outcomes after conservative treatment, and to identify subgroups at risk of an inferior outcome. DESIGN: Retrospective case series. SETTING: Level II trauma center. PATIENTS: Between 2005 and 2008, 122 patients were conservatively treated for a completely displaced midshaft clavicle fracture of whom 92 were eligible for inclusion in this study. Of these, 59 completed the study after a median of 2.7

Advances in intramedullary nail fixation in foot and ankle surgery.

Tibiotalocalcaneal arthrodesis for the treatment of complex foot and ankle deformities are extremely challenging cases. Technological advances in intramedullary nail fixation have improved the biomechanical properties of available fixation constructs in recent years. Nails designed specifically to accommodate hindfoot anatomy, advancement in the understanding of optimal screw orientation, fixed angle technology, the availability of spiral blade screws, and features designed to achieve compression across the arthrodesis site have provided the foot and ankle surgeon with a greater armamentarium

Migration of luque rods through a laminectomy defect causing spinal cord compression.

Internal fixation of traumatic spinal injuries has been associated with spinal canal stenosis, spinal cord compression, and nerve root impingement. We present a case of spinal cord/cauda equina compression due to migration of intact, anchored thoracolumbar Luque rods into the spinal canal through a laminectomy defect, leading to neurologic complications 10 years after the original operation.

[Percutaneous management of osseous avulsions of extensor tendons and lateral ligaments of the fingers with the hooked wire].

A new method of percutaneous treatment of some fractures of the fingers is being described. With the use of a magnification table, fitted onto the X-ray intensifier, evulsion fractures of extensor tendons or collateral ligaments can be reduced and fixed by means of a K-wire. After insertion, the end of the wire is being bent in the shape of a hook ("wire hook"), into which a pull-out wire is inserted. By anchoring the wire-hook at the opposite side of the finger under tension, the fragment is held in position by the hook. After four weeks the wire-hook is cut below its anchorage and can easily

Stabilization of thoracic and thoracolumbar fracture-dislocations with Harrington rods and sublaminar wires.

Seventeen unstable thoracic and thoracolumbar fracture-dislocations have been treated by Harrington double-distraction rod technique supplemented by doubled 18-gauge stainless-steel wires applied to two intact laminae above and below the injury. Fourteen of the 17 cases were grossly comminuted and translated beyond 50% of the width of the spinal column. No external bracing was used in the 13 patients with neurologic deficits. At the time of follow-up examinations (greater than 22 months in all patients) there was no loss of reduction or fixation in any patient. Solid fusion was achieved in all

Alternative indications for external fixation according to Wagner.

Wagner's technique of external fixation was originally developed for limb-lengthening procedures. With its one-dimensional configuration and great stability, however, it is also highly suitable for application to the upper extremity and thigh for other purposes and several alternative applications are described. The more important indications are infected defect-pseudarthroses of the femur, temporary immobilization of the knee-joint, and 'unmanageable' fractures of the distal radius.

Tantalum as Trabecular Metal for Endosseous Implantable Applications.

During the last 20 years, tantalum has known ever wider applications for the production of endosseous implantable devices in the orthopedic and dental fields. Its excellent performances are due to its capacity to stimulate new bone formation, thus improving implant integration and stable fixation. Tantalum's mechanical features can be mainly adjusted by controlling its porosity thanks to a number of versatile fabrication techniques, which allow obtaining an elastic modulus similar to that of bone tissue, thus limiting the stress-shielding effect. The present paper aims at reviewing the charact

Concomitant congenital heart defect repair and Nuss procedure for pectus excavatum.

The combination of complex congenital cardiac anomalies and pectus excavatum represents a technical challenge. Most concomitant repairs have been performed in adult patients. We report the uncomplicated concomitant repair of double-outlet right ventricle with absent pulmonary valve syndrome and the Nuss procedure for pectus excavatum in a 3-year-old child.

Cement augmentation of the navigated iliosacral screw in the treatment of insufficiency fractures of the sacrum: a new method using modified implants.

PURPOSE: Pelvis ring fractures of geriatric patients are currently a serious problem in orthopedic and trauma surgery. One controversy that remains is the insufficiency fracture of the sacrum with treatment options ranging from conservative to operative. We modified standard 7.3-mm screws with additional perforations at the tip to allow cement injection trough the screw to improve implant anchorage. METHOD: We describe a new method of treatment of sacral insufficiency fractures. We use modified cannulated 7.3-mm screws (Synthes) with additional perforations at the screw tip. This allows the i

Medial and lateral crossed pinning versus lateral pinning for supracondylar fractures of the humerus in children: decision analysis.

BACKGROUND: The choice of pinning techniques in supracondylar fractures of the humerus in children has been a debate regarding its fixation stability and risk of iatrogenic ulnar nerve palsy. This study was performed to determine as to which fixating method (medial and lateral crossed pinning vs. lateral pinning) is better for the displaced supracondylar fractures using a decision analysis tool in terms of function. METHODS: A decision analysis model was designed containing the probability of iatrogenic ulnar nerve palsy and malunion caused by unstable fixation for each of lateral pinning and

Frontal and sagittal balance analysis of late onset idiopathic scoliosis treated with third generation instrumentation.

As scoliotic curve is a rotational deformity, derotation maneuvre was used as the corrective factor, but recent studies demonstrated spinal imbalance and decompensation problems in patients treated with this method. This study evaluates 217 late onset idiopathic scoliosis patients surgically treated with third generation instrumentation (Texas Scotish Rite Hospital System - TSRH) from September 1991 to November 1996 with a minimum 2 years follow up. Preoperative and postoperative Cobb angles in the frontal plane and thoracic kyphosis and lumbar lordosis angles in the sagittal plane are measure

Deformity correction and arthrodesis of the midfoot with a medial plate.

Nine feet in eight patients undergoing tarsometatarsal (Lisfranc) or other midfoot arthrodeses for posttraumatic or degenerative arthritis were reviewed retrospectively. All patients were treated using a medial one-third tubular plate spanning the midfoot joints to be fused. Three feet underwent fusion in situ while six feet underwent correction of residual planus, planovalgus, or cavovarus deformity at the time of fusion. All patients achieved fusion within 12 weeks. A good or excellent result was achieved in seven of nine feet. There was no radiographic or clinical evidence of pseudarthrosis

[Clinical experience with recently produced devices for use in spinal surgery].

The authors reported their clinical experience with devices, which have been recently introduced for spinal surgery. The main roles of such devices are to correct malalignment of the spine and stabilize the spinal instability. Although Harrington's device has been widely used since 1962, its use is gradually decreasing due to its having several weak points, such as its low resistance to distorting forces and its imperfect ability to correct malalignment of the spine. In the present manuscript, the authors describe clinical experience with three kinds of spinal devices. They are Steffee's trans
