Plastazote abduction orthosis in the management of neonatal hip instability.

Since 1987, 113 consecutive newborns with either Ortolani-positive or provocative-positive hip examinations in the newborn nursery have been treated with a Plastazote hip abduction orthosis when diagnosed and followed to determine if the method was safe, effective, and easy to use. Ortolani-positive hips often had bilateral abnormalities in the hip examination, were frequently associated with breech position, and were anatomically more dysplastic than those hips that were unstable by provocative testing. No patient developed ischemic necrosis during follow-up. Only two had additional treatment

Functional instability of the ankle and the role of neuromuscular control: a comprehensive review.

A review of current knowledge of the clinical syndrome of functional ankle instability is presented. Recent evidence has demonstrated that the majority of patients with functional instability of the ankle do not have mechanical hypermobility of the ankle joint. Functional instability of the ankle results from a loss of neuromuscular control. Components of neuromuscular control include proprioception, muscle strength, muscle reaction time, and postural control. Proprioceptive deficits lead to a delay in peroneal reaction time, which appears to be a peripheral reflex. Proprioception and eversion

The prevention of ankle sprains in sports. A systematic review of the literature.

To assess the published evidence on the effectiveness of various approaches to the prevention of ankle sprains in athletes, we used textbooks, journals, and experts in the field of sports medicine to identify citations. We identified 113 studies reporting the risk of ankle sprains in sports, methods to provide support, the effect of these interventions on performance, and comparison of prevention efforts. The most common risk factor for ankle sprain in sports is history of a previous sprain. Ten citations of studies involving athletes in basketball, football, soccer, or volleyball compared alt

Influence of external stabilizing devices of the ankle on sport-specific capabilities.

We compared sport-specific-skills in 23 athletes with functionally unstable ankle joints to those of 18 healthy volunteers by performing a Japan test as well as a specially designed single-leg jumping test. For external stabilization of the ankle, an Aircast brace, a Ligafix Air brace, a Malleoloc brace and a tape bandage were applied. For the Japan test in the group with uninjured ankle joints, the best results were obtained when wearing the Aircast brace, followed by tape bandage, Ligafix brace and no stabilizing device in descending order. The worst results were presented by the group with

Rib cage-spine coupling patterns involved in brace treatment of adolescent idiopathic scoliosis.

STUDY DESIGN: The three-dimensional (3-D) interrelations in the correction of the spine and rib cage produced by the Boston brace were analyzed in a group of adolescents with idiopathic scoliosis. OBJECTIVES: To investigate the coupling movements between the spine and rib cage initiated by brace wear (i.e., the displacements of the spine that take place in other directions than the ones generated by brace pressures on the thorax). SUMMARY OF BACKGROUND DATA: The effects of thoraco-lumbo-sacral orthosis in the frontal plane have been well documented, but they have never been studied in terms

Sever's disease and other causes of heel pain in adolescents.

Sever's disease, or apophysitis of the calcaneus, is a common but frequently undiagnosed source of heel pain in young athletes. This condition frequently occurs before or during the peak growth spurt in boys and girls, often shortly after they begin a new sport or season. Sever's disease often occurs in running and jumping sports, particularly soccer. Patients present with intermittent or continuous heel pain occurring with weight bearing. Findings include a positive squeeze test and tight heel cords. Sever's disease cannot be diagnosed radiographically. The condition usually resolves two week

The ballet technique shoe: a preliminary study of eleven differently modified ballet technique shoes using force and pressure plates.

Ballet dancers are vulnerable to various stress-related injuries; foot and ankle injuries are the most common. This study determined if the pressure across the foot could be more evenly distributed by wearing various modified technique shoes padded with PPT, Spenco, or Sorbathane, alone or in combinations. A professional ballet dancer performed a jump by taking a single step on his left foot, jumping and landing with his right foot on the force and pressure plates with each modified shoe plus one unmodified shoe. The results of this study revealed: (1) total force cannot be reduced, (2) center

Effect of standard and Aircast tennis elbow bands on integrated electromyography of forearm extensor musculature proximal to the bands.

Classic tennis elbow, or lateral epicondylitis, has been described as an overuse or misuse injury resulting in a tendinitis. The extensor carpi radialis brevis (ECRB) and extensor digitorum communis (EDC) have been implicated as primary culprits in this pathology. Tennis elbow has been treated using a constrictive band placed several centimeters distal to the origin of these two muscles. Aircast (Aircast Inc., Summit, NJ) has developed a new style of band that employs an air-filled bladder as the counterpressure element. This study tested the effect of both standard and Aircast bands on EMG ac

Rehabilitation management of children with spastic diplegic cerebral palsy.

Spastic diplegia has been recognized as the type of cerebral palsy most frequently associated with prematurity. Due to constantly improving neonatal care in developed countries, more and smaller premature infants are surviving, and the number of spastic diplegic children can be expected to increase. This paper reviews the incidence, pathophysiology, and associated handicaps of patients with this type of cerebral palsy. The role of the physiatrist and aspects of traditional management are discussed. Recent advances in treatment of spasticity and lower extremity bracing are stressed as they seem

Abnormal extension of the big toe as a cause of ulceration in diabetic feet.

Relief from external pressure and repetitive stress is a main concept in treating diabetic neuropathic ulcers. Ulcers on the tip of the big toe may be caused by abnormal extension of the big toe, which can be diagnosed only by observing the patient during barefoot walking. A flexor pad underneath the big toe eliminates the phenomenon, but in case of rigidity of the toe, extremely roomy toe boxes must be prescribed. Out of a series of 272 diabetic patients with skin lesions on the feet 18 suffered this phenomenon, in one case with bilateral ulcerations. Fifteen ulcers healed and below the knee

Most of these structural deformities can be corrected through the use of various orthotic devices. Specific guidelines are presented for using both soft and permanent orthoses, which offer the foot increased shock absorption and proper structural alignment.

An office-based orthotic system in treatment of the arthritic foot.

Patients with inflammatory arthritic disease of the feet invariably require conservative office management. The simplest method of obtaining relief has been to use commercially available extra-depth shoes combined with custommade, removable, closed-celled polyethylene foam arch supports. These supports are soft, light, and can be accurately adjusted to each patient's pathology. The technique and modifications used for relieving the commonest problems, such as anterior metatarsalgia and plantar heel pain, are discussed.

Treatment of congenital subluxation and dislocation of the hip by knee splint harness.

The results are reported from a study of 103 cases fitted with the knee splint harness (KSH) which is an orthotic device used for the treatment of congenital subluxation and dislocation of the hip. The knee splint harness consists of a harness attached to posterior plastic shells at the knee which prevent flexion beyond 90 degrees while permitting full and free knee extension. No case was encountered in a 10 year period which failed to reduce. In cases of hip subluxation, reduction was obtained within an average of 5.8 days. In cases of dislocation, reduction was obtained in 6.7 days on averag

Greater trochanteric pain syndrome and gluteus medius and minimus tendinosis: nonsurgical treatment.

Greater trochanteric pain syndrome (GTPS) affects 10-25% of people in developed countries. The underlying etiology for GTPS is most commonly the tendinosis or a tendon tear of the gluteus medius, minimus or both at the greater trochanter; the inflammation of the tendon is not a major feature. We critically evaluated conservative treatment, for which we reviewed 76 publications, grading them according to four levels of evidence. We identified a wide variety of conservative treatment options: home therapy (insoles, walking sticks/crutches, orthotic devices, stretching exercises and preventive me…
