In order to determine the prevalence of secondary hyperparathyroidism in patients with Paget's disease of bone, we measured serum parathyroid hormone levels (N-terminal assay) in 39 patients with a wide range of pagetic activity. All patients had normal serum calcium levels. A total of 30 patients were either untreated or had received no treatment for 6 months or longer when studied; the other 9 were receiving either salmon calcitonin (3) or EHDP (6). The results showed that in 7 of the 39 patients (18%) parathyroid hormone levels were increased above normal. These were among the most severely

(Asu1,7) E-CT (carbocalcitonin-elcatonin) is the aminoasuberic analogue of eel calcitonin differing from the natural hormone in that an ethylene bridge replaces the disulphide bridge in position 1-7. This modification preserves the conformation of natural calcitonin but enhances the physical, chemical and biological stability of the molecule. This in its turn facilitates maximum purification of the product as well as enhancing its resistance to degradation both in vitro and in vivo. The specific activity of carbocalcitonin is equivalent to 5000 U.MRC/mg and it possesses all the pharmacological

After biochemical and radiographic studies, enzyme replacement therapy in three patients with the infantile form of hypophosphatasia was attempted by weekly intravenous infusions of bone alkaline phosphatase-rich (BAP) plasma from patients with Paget bone disease. Subsequently, circulating BAP activity was substantially increased in each patient, and in one was maintained in the normal range for nearly 2 months. Despite partial or complete correction of the deficiency of circulating BAP activity, we observed no radiographic evidence for arrest of progressive osteopenia or improvement in rachit

Several patients with Paget's disease have been reported to have monoclonal IgM proteins in their serum. We have systematically studied 26 patients with Paget's disease severe enough to require diphosphonate therapy. Five of these patients were found to have isolated elevations of serum IgM which were well outside the normal range for their age, sex and race. An additional patient had elevations of both IgG and IgM. These patients seem to represent a subset of Pagetics. They do not differ from the remainder of the group with respect to severity or extent of disease, response to therapy, presen

Imaging of the vault of the cranium via CT must be considered as an additional examination. It is possible to assess the CT image already stored during examination of the brain, without additionally exposing the patient, merely by changing position and breadth of the window. In our opinion, primary indication for CT of the vault of the cranium exists only if diagnosis based on survey remains doubtful. However, examination will always be combined with a representation of the individual strata of the brain. Our studies have also shown that in many cases CT assessment of the vault of the cranium

The two most commonly used biochemical markers of bone turnover are the serum alkaline phosphatase and the urinary excretion of peptide-bound hydroxyproline, both of which are increased in Paget's disease. Serum alkaline phosphatase is assumed to be derived from osteoblasts during the process of bone formation, whereas small peptides containing hydroxyproline are excreted in the urine following the degradation of bone collagen. The alkaline phosphatase is probably the more useful measurement for diagnosis and for following response to treatment, whereas hydroxyproline, although very sensitive,

The value of bone scintigraphy in the prediction and detection of a relapse of Paget's disease of bone after treatment, as well as the pattern of such a recurrence, were studied in a group of 40 patients. Thirty of these received a combination of calcitonin and HEDP, ten were treated alternately with calcitonin and HEDP. Scintigraphic deterioration is reliable evidence for a recurrence of Paget's disease of bone; one third of all recurrences was noted first on the bone scintigram. In another third of the cases of recurrence, however, the scintigram showed virtually no signs of deterioration. S

Urinary glycosaminoglycan and hydroxyproline excretion was studied in 11 patients with clear evidence of Paget's disease of bone. Urinary hydroxyproline, cetyl pyridinium chloride (CPC)-precipitable uronic acid and CPC-precipitable hexosamine were expressed as ratios to urinary creatinine. Urine samples were concentrated x 1000 by vacuum dialysis and the glycosaminoglycans examined by electrophoresis on cellulose acetate followed by staining with alcian blue. All the cases studied showed markedly raised hydroxyproline excretion, whereas the uronic acid excretion was normal or only slightly rai

The hypercalcemia observed during the acute calcitonin test reflects the size and the activity of the osteoclastic population throughout the entire skeleton. Several calcitonins of animal origin (pork, salmon) have already been used for this test in human pathology, but the results can be flawed by the presence of anti-calcitonin antibodies. The authors demonstrate that human synthetic anti-calcitonin in man has a hypocalcemia effect identical to salmon synthetic calcitonin, with an equipotential dose for the rat. The systematic study of acute salmon calcitonin in various osteopathies makes it

Severe dementia and ataxia in an elderly woman were related to hydrocephalus resulting from Paget disease of the skull, with basilar impression. Ventriculoatrial shunting resulted in dramatic improvement. This procedure, rather than suboccipital decompression, is probably the operation of choice, and should be carried out at an early stage.

The intra nuclear and intra cytoplasmic inclusions described in osteoclasts in PAGET's bone disease are morphologically similar to those observed in subacute sclerosing panencephalitis. Immunological techniques using different specific immune sera demonstrate the presence of an antigenic structure of viral origin in osteoclasts in PAGET's bone disease. A measles or a measles like virus is most likely to be involved and may play a role in the etiology of the disease.

Ethane-1-hydroxy-1, 1-diphosphonate (EHDP) was administered in a dose of 20 mg/kg/d to 21 patients with symptomatic Paget's disease. All patients were treated for 6 months and then followed for an additional 6 months. There was a striking decline in serum alkaline phosphatase and urinary hydroxy-proline excretion observed after 3 months of therapy which was not significantly improved in the succeeding 3 months. Concomitantly there was marked improvement in clinical symptoms and bone scans. Following cessation of therapy, continued biochemical and clinical evidence of remission persisted. Sever

Archeological bone often lends itself to histological analysis. Sections from bone samples approximately one thousand years old may show as much structural preservation as those only a few hundred years old; hence, it appears that the degree of preservation is not necessarily affected by time. Enough structure may be preserved to permit the diagnosis of metabolic disorders of bone which might go undetected by other methods. This type of analysis can be utilized to accept or reject individual remains suspected of being pathological on the basis of other less precise diagnostic techniques.

The authors report an observation on a man 68 years of age suffering from Paget's disease in the pelvi-rachidian region. In four years the lumbar lesions evolved to form an L2-L4 vertebral block. The intensity of the bone radiotransparency of the vertebral block indicated some similarity to the localized forms of osteolysis well known in the skull and the long bones.
