CLINICAL ISSUE: Monofocal and multifocal bone lesions in children and adolescents may result from various diseases. Apart from inflammatory entities malignancy is the most important issue to be ruled out.
STANDARD TREATMENT: Various diseases. Apart from inflammatory origin malignancy is the most important issue to be ruled out. Imaging in particular magnetic resonance imaging (MRI) has an important role for the diagnostic process.
DIAGNOSTIC WORK-UP: Osteomyelitis and septic arthritis on the one hand and nonbacterial osteomyelitis and synovitis, acne, pustulosis, hyperostosis, and osteitis s…

Clostridium cadaveris, named following its identification in human corpses, is an unusual pathogen. We report the first case of C. cadaveris osteomyelitis. This case highlights the importance of deep tissue sampling and appropriate culture to correctly identify causative pathogens and guide targeted antimicrobial therapy in difficult-to-treat infections like chronic osteomyelitis.

Chronic recurrent multifocal osteomyelitis (CRMO), also called chronic nonbacterial osteomyelitis (CNO) or nonbacterial osteomyelitis (NBO), is a rare autoinflammatory bone disease of unknown etiology. However, the number of patients properly diagnosed would increase with better knowledge of the disease. In this regard, whole-body magnetic resonance imaging (WB MRI) has been found to be a better predictor of active lesions than clinical examination. Importantly, the RINBO index (radiologic index for NBO) quantifies the involvement based on the WB MRI. Further, a chronic nonbacterial osteomyeli…

Bacterial osteomyelitis of the scapula is rare in pediatric patients. When it occurs, it usually affects young infants and patients often exhibit fever and functional limitation of the extremity. We present a case of a 12-year-old boy with subacute bacterial scapular osteomyelitis and a normal physical examination. The patient was briefly treated with intravenous antibiotics and then transitioned to oral antibiotics to complete a 4-week course for methicillin-sensitive Staphylococcus aureus osteomyelitis.

Chronic osteomyelitis is a relapsing, persistent, low-grade inflammation of bone caused by various infectious agents. The present study, conducted over a two-year period, on specimens received from cases of chronic osteomyelitis was, to determine the frequency of isolation of aerobic and anaerobic bacteria and to analyse their antimicrobial susceptibility pattern. Specimens were processed for Gram stain, aerobic and anaerobic culture, and were identified according to standard techniques. Significant growth was observed in 102/204 specimens, in which aerobic growth was observed in 62 (60.8%) an…

OBJECTIVES: This study aimed to develop a suitable osteomyelitis model for pharmacokinetic/pharmacodynamic (PK/PD) evaluation and to investigate the target PK/PD values of vancomycin and tedizolid against methicillin-resistant Staphylococcus aureus (MRSA) osteomyelitis.
METHODS: An osteomyelitis model was established by implanting an MRSA-exposed sterilized suture in the tibia of normal mice and mice with cyclophosphamide-induced neutropenia. The suitability of the osteomyelitis mouse model for PK/PD evaluation was assessed using vancomycin as an indicator. The target PK/PD values for tedizol…

Journal of oral pathology & medicine : official publication of the International Association of Oral Pathologists and the American Academy of Oral Pathology2022eng
BACKGROUND: There are relatively few reports on the histopathological characteristics of diffuse sclerosing osteomyelitis of the mandible (DSOM), which is difficult to distinguish from chronic suppurative osteomyelitis (CSO) and craniofacial fibrous dysplasia (CFD). This study aimed to summarize and compare the histopathological characteristics of DSOM, CFD, and CSO.
MATERIALS AND METHODS: In this study, hematoxylin and eosin-stained sections of patients with DSOM, CSO, and CFD at the Peking University Hospital of Stomatology from 2015 to 2020 were retrieved. The histopathological characteris…

INTRODUCTION: Chronic osteomyelitis is the bane of orthopedics. Along with being difficult to treat for surgeons, it also impacts the patient's life immensely, severely restricting their life in all the aspects. Treatment with antibiotic coated cement spacers, which are removed after a duration of 6-8 weeks for the definitive surgery, is a frequently done procedure in conjunction with an adequate debridement of the necrotic tissues and prolonged IV antibiotics. We report a case of acute osteomyelitis superimposed on the chronic osteomyelitis of the left proximal humerus.

PURPOSE: Chronic diffuse sclerosing osteomyelitis (CDSO) is a non-suppurative inflammatory bone disease diagnosed based on combined clinical, histopathological, and radiological findings. Accurate diagnosis is important since CDSO is more refractory to treatment than suppurative osteomyelitis. The purpose of this study was to determine the diagnostic efficacy of diffusion-weighted imaging (DWI) in the quantitative assessment of CDSO to distinguish it from acute suppurative osteomyelitis (ASO) and chronic suppurative osteomyelitis (CSO) of the mandible.
METHODS: Using a retrospective cohort st…

INTRODUCTION: Hemipelvectomy - surgical removal of half of the pelvis including the ipsilateral limb, is a rare operation with a narrow indication spectrum, most often performed for malignant tumors in the proximal femur area with cranial spread. Other, less frequent indications include devastating trauma to the pelvic area and deep soft tissue infections involving the pelvic bones. In our two case reports, we present deep decubitus ulcers complicated by osteomyelitis of the pelvic bones in paraplegic patients. Case report 1: We describe the case of a 33-year-old paraplegic who underwent right…

Teaching point: Detection of intraosseous gas with a bubbly or irregular pattern on imaging should raise a strong suspicion for emphysematous osteomyelitis and prompt urgent antimicrobial treatment, particularly in immunocompromised patients without a history of trauma or surgery.
