Pulmonary Paragonimiasis Presenting as Pneumothorax, Hemoptysis, and Severe Eosinophilia: A Case Report.

Paragonimiasis is a foodborne zoonosis transmitted mostly by the consumption of inadequately cooked crabs and crayfish harboring Paragonimus metacercariae. We present the case of a 38-year-old male with a history of crab ingestion who developed severe eosinophilia, transaminitis, and serositis. Initial presentation included fever, epigastric pain, weight loss, and pleural effusion. He was managed empirically with albendazole and corticosteroids along with other supportive measures. Two months later, he re-presented with hemoptysis and hypoxemia. HRCT showed multiple cavitary pulmonary lesions,…

Paragonimiasis in a child with massive pericardial effusion as the primary manifestation: a case report.

BACKGROUND: Paragonimiasis in children is rare as isolated pericardial effusion, especially when presenting despite the absence of elevated eosinophilia. We report such a case to highlight this atypical manifestation. CASE PRESENTATION: A 10-year-old male child was admitted to the hospital with “pericardial effusion found for 5 days”. The initial differential diagnosis included infectious, autoimmune, and neoplastic causes of pericarditis. Laboratory tests on admission showed normal serum and pericardial effusion eosinophils, and the pericardial effusion was positive for paragonimiasis IgG. Mu…

Computed Tomography and Magnetic Resonance Imaging in the Diagnosis of Cerebral Paragonimiasis in Children.

OBJECTIVE: To investigate the computed tomography (CT) and magnetic resonance image (MRI) manifestations of cerebral paragonimiasis (CP) in children and to improve the understanding of the disease. METHODS: The cranial CT and MRI data of 12 children with positive intradermal tests for Paragonimus-specific antigens were retrospectively analyzed. Additionally, the lesion locations, morphology, and imaging characteristics were analyzed. RESULTS: The lesions were located in the cerebral parenchyma in 12 cases, with 10 in the supratentorial area and two in the subtentorial area, among which three…

Neuroimaging findings in a woman with brainstem paragonimiasis: a case report and review of the literature.

INTRODUCTION: The most serious erratic migration of paragonimiasis is a cerebral paragonimiasis. Infection in the temporal and occipital lobes of the brain has been previously reported in cerebral paragonimiasis. Here, we report a case of chronic brainstem paragonimiasis. CASE PRESENTATION: A 29-year-old Chinese woman presented to our hospital complaining of a sudden onset, three days previously, of double vision and dizziness. A computed tomography scan and diffusion-weighted images looked like an ischemic stroke. However, conglomerates of multiple ring-like enhancements on contrast magnetic…

Characteristic MR and CT imaging findings of hepatobiliary paragonimiasis and their pathologic correlations.

BACKGROUND: Hepatobiliary paragonimiasis (HP) is not commonly encountered and may be confused with hepatobiliary tumors; however, computed tomography (CT) and magnetic resonance imaging (MRI) features of HP allow this entity to be distinguished from other diseases. PURPOSE: To present the CT and MRI findings in patients with HP and to describe some specific imaging findings along with their pathological correlations. MATERIAL AND METHODS: Imaging and clinical findings of 21 patients (9 boys/men and 12 girls/women; age range 3-67 years; mean age 40 years) who were diagnosed with HP were retro…

[The manifestations and clinical significance of chest CT in paragonimiasis at different clinical phases].

OBJECTIVE: To investigate the chest CT manifestations of paragonimiasis at different clinical phases. METHODS: Forty-eight cases of paragonimiasis confirmed by laboratory studies were retrospectively analyzed. There were 30 males and e18 females ageing 9 - 66 years (average, 31.4 years). Conventional chest CT was performed in all cases and contrast enhanced CT in some cases. RESULTS: The chest CT findings of paragonimiasis included: (1) peribronchial inflammation (n = 9); (2) infiltration (n = 13), manifested as unilateral or bilateral patchy infiltrates with hazy borders. The location and s…

Identification of the human paragonimiasis causative agent in Lao People's Democratic Republic.

To assess the species of human paragonimiasis in Lao People's Democratic Republic, 6 ovum samples from 6 native confirmed paragonimiasis patients were examined with polymerase chain reaction (PCR) amplifying the internal transcribed spacer 2 (ITS2). The PCR products were sequenced, and a homology search was performed using the GenBank. All 6 sequences were identical with Paragonimus heterotremus ITS2. Our work suggests that P. heterotremus may be the main etiological agent of human paragonimiasis in this locality.

Pulmonary paragonimiasis. Report of a case with diagnosis by fine needle aspiration cytology.

We report a case of paragonimiasis in a Nigerian woman evaluated for symptoms of chronic respiratory disease five years after chemotherapy for primary lymphoma of the breast. Fine needle aspiration of one of two fibrocavitary pulmonary lesions yielded thick, brown material in which ova diagnostic of Paragonimus westermani were identified cytologically. This disease is unusual in natives of North America but is seen in travelers and immigrants from Asia, Africa, and South and Central America, where it is endemic. The infection can be fatal, especially if it involves the central nervous system. …

[Purification of antigenic proteins of Paragonimus westermani and their applicability to experimental cat paragonimiasis].

This study was designed to evaluate the partially purified antigens which were fractionated from crude extract of Paragonimus westermani and to monitor the enzyme-linked immunosorbent assay (ELISA) in experimental cat paragonimiasis during the course of infection as well as before and after chemotherapy. Crude extract of 6-month-old adult P. westermani was fractionated to 5 antigens by successive applications of ammonium sulfate precipitation, ion exchange chromatography and gel filtration. And the cats, 10 in each group, were infected with 60, 30, 15, and 5 metacercariae, then the half of eac…

Pulmonary paragonimiasis: clinical features, diagnosis and treatment of 39 cases in Manipur.

Thirty-nine cases of pulmonary paragonimiasis due to Paragonimus westermani were identified in Manipur, India. Recurrent haemoptysis, often increased on exertion, was the most common symptom. The clinical features and radiological appearances simulated pulmonary tuberculosis and 23 cases were treated for tuberculosis for varying periods before the final diagnosis. Young males aged 11 to 30 years comprised two-thirds of the cases. The disease was acquired by eating raw or improperly cooked crabs (Potamon deham). Intradermal tests (IDT) employing a saline extract of adult Paragonimus were a simp…

Pathological and parasitological aspects of the first autochthonous case of human paragonimiasis in Venezuela.

A worm found in histopathologic sections of a lung piece of a young Venezuelan male was identified as a Paragonimus sp. Definitive identification of the species was not possible since only a deteriorated segment of the worm was recovered, nevertheless comparison with other known species is discussed. This is the first report of an indigenous case of human paragonimiasis in Venezuela.

[A Follow-Up Study To Evaluate The Efficacy Of Mass Chemotherapy For Control Of Paragonimiasis].

In the survey of l1,005 subjects in l3 villages of Che Ju lsland, 1,450 persons were egg-positive by repeated sputum examinations. The respo … up study, however, strongly indicate that continous and intense chemotherapy may be able to put the paragonimiasis under control. Further follow-up study on total population including survey on changes of environment and eating habits is planned.
