2019 Update on Pediatric Medical Overuse: A Systematic Review.

IMPORTANCE: Medical overuse is common in pediatrics and may lead to unnecessary care, resource use, and patient harm. Timely scrutiny of established and emerging practices can identify areas of overuse and empower clinicians to reconsider the balance of harms and benefits of the medical care that they provide. A literature review was conducted to identify the most important areas of pediatric medical overuse in 2018. OBSERVATIONS: Consistent with prior methods, a structured MEDLINE search and manual table of contents review of selected pediatric journals for the 2018 literature was conducted …

Doing the Right Thing? General Practitioners' Considerations in Achieving a Timely Dementia Diagnosis.

OBJECTIVES: Timely detection and diagnosis of dementia are beneficial for providing appropriate, anticipatory care and preventing acute situations. However, initiating diagnostic testing is a complex and dynamic process that requires general practitioners (GPs) to balance competing priorities. Previously identified barriers, such as a lack of time, knowledge, and resources, may not fully represent the challenges involved in this process. Therefore, this study aimed to examine GPs' more implicit considerations on starting the diagnostic trajectory for dementia. METHODS: A qualitative study was

Self-Reported Suicidal Ideation as a Predictor of Suicidal Behavior Among Outpatients With Diagnoses of Psychotic Disorders.

OBJECTIVE: Individuals with psychotic disorders are at high risk of suicidal behavior. The study examined whether response to item 9 of the Patient Health Questionnaire (PHQ-9), which asks about thoughts of death or self-harm, predicts suicidal behavior among outpatients with diagnoses of psychotic disorders. METHODS: Electronic health records (EHRs) from seven large integrated health systems were used to identify all outpatient visits by adults with a diagnosis of schizophrenia spectrum psychosis or unspecified psychosis from January 1, 2009, to June 30, 2015, during which a PHQ-9 was comple

Quality of Care Transition and Patient Safety: An Investigation Into Patients After Bariatric Surgery.

BACKGROUND: High quality care transition (QCT) is key to mitigating patient harm after hospital discharge to home. Nevertheless, patients' experiences of QCT are rarely investigated, particularly in acute surgical settings. Given the high relevance of bariatric surgery as the gold standard in the treatment of severely obese patients, mitigation of patient harm is gaining traction. Therefore, we aimed to determine the association between patient-experienced QCT and safety incidents and health complications after bariatric surgery. METHODS: Cross-sectional survey study. Using standardized instr…

Undiagnosed myopathy before surgery and safe anaesthesia table.

Patients with muscle pathology are a challenge for anaesthesiologists because of possible life-threatening general anaesthesia complications. A review of the current medical literature on the issue clearly indicates that increasing awareness by anaesthesiologists in recent years has led to a reduction in the occurrence of adverse events in patients with diagnostically well-defined muscle disease. On the other hand, the current emerging aspect is that the great majority of complications concern subjects with clinically non-overt (silent to mildly symptomatic) and thus undiagnosed myopathy. With

Association between posttreatment gambling behavior and harm in pathological gamblers.

This study investigated the relationship between posttreatment gambling behavior and harm in a sample of treatment-seeking pathological gamblers. One year after initiating treatment, participants (n = 178) completed the Gambling Timeline Followback (D. C. Hodgins & K. Makarchuk, 2003; J. Weinstock, J. P. Whelan, & A. W. Meyers, 2004) and the South Oaks Gambling Screen (SOGS; H. R. Lesieur & S. B. Blume, 1987), both of which assessed gambling behaviors and problems over the prior 6 months. Based upon self-reports of gambling-related problems on the SOGS, participants were classified as problem

Self-mutilation and eating disorders.

Patients with eating disorders are at high risk for self-mutilation (e.g., skin cutting and burning), and vice versa. Evidence for this linkage comes from a literature review, from patient interviews, from responses to an instrument we have developed (the Self-Harm Behavior Survey), and from three instructive case reports. Even if the self-mutilation in these patients is regarded as a Borderline Personality Disorder symptom, DSM-IV should list it as an associated feature or a complication of Anorexia Nervosa/Bulimia Nervosa. In lieu of a dual diagnosis, we postulate that the combination of sel

Mobile clinics for opioid use disorders: What they do and how they do it. A qualitative analysis.

INTRODUCTION: People with substance use disorders (SUD) face many barriers to receiving evidence-based treatments including access to and cost of treatment. People who use drugs face stigma that limits access to traditional office-based clinics. With the goal of reducing morbidity and mortality, mobile clinics reduce many of these barriers by providing harm reduction and on-demand low-threshold medical care. METHODS: In 2020 Massachusetts Department of Public Health (DPH) Mobile Addiction Services Program expanded a program called Community Care in Reach building on its success in reducing ba

Patient safety in actioning and communicating blood test results in primary care: a UK wide audit using the Primary care Academic CollaboraTive (PACT).

BACKGROUND: Errors associated with failures in filing, actioning and communicating blood test results can lead to delayed and missed diagnoses and patient harm. This study aimed to audit how blood tests in primary care are filed, actioned and communicated in primary care, to identify areas for patient safety improvements. METHODS: UK primary care clinicians were recruited through the Primary Care Academic CollaboraTive (PACT). PACT members audited 50 recent sets of blood tests from their practice and retrospectively extracted data on blood test result coding, actioning and communication. PACT…

A novel model measuring the harm of transplanting hepatocellular carcinoma exceeding Milan criteria.

No empirical studies have defined the posttransplant survival that would justify expansion of the Milan criteria for liver transplantation of hepatocellular carcinoma. We created a Markov model comparing the survival benefit of transplantation for a patient with >Milan HCC, versus the harm caused to other patients on the waiting list. In the base-case analysis, the strategy of transplanting the patient with >Milan HCC resulted in a 44% increased risk of death and a utility loss of 3 quality-adjusted years of life across the pre- and posttransplant periods for a nationally representative cohort

The harm that wasn't: moral residue, near-miss events, and the ethics of harmless error in procedural medicine.

A surgeon performing a retroperitoneal procedure inadvertently begins dissecting the wrong kidney. The error is recognized intraoperatively, the correct structure is identified, and the procedure is completed successfully. The patient suffers no measurable clinical harm. The institution classifies the event as a near-miss, and no disclosure is initiated. This paper argues that the classification, while legally defensible, is morally inadequate. The absence of measurable clinical harm does not eliminate the ethical obligations a serious procedural error generates; it displaces them onto forms o

Self-harm, suicide and brain death: the role of the radiologist.

Suicide is a leading cause of death worldwide and takes many forms, which include hanging, jumping from a height, sharp force trauma, ingestion/poisoning, drowning, and firearm injuries. Self-harm and suicide are associated with particular injuries and patterns of injury. Many of these patterns are apparent on imaging. Self-harm or suicidal intent may be overlooked initially in such cases, particularly when the patient is unconscious or uncooperative. Correct identification of these findings by the radiologist will allow a patient's management to be tailored accordingly and may prevent future

When Nature Meets Oncology: Unraveling Herb-Drug Interactions in Cancer Therapy.

Natural product (NP) use by patients alongside their conventional cancer therapies is ubiquitous. This common, yet often hidden, practice can potentially contribute to significant patient harm, given the narrow therapeutic window of most anticancer drugs. This review takes on this challenge directly, moving past theoretical concerns to summarize current clinical evidence on interactions between widely used NPs and modern cancer treatments, including chemotherapy, targeted therapy, and immunotherapy. We break down the key pharmacokinetic (PK) mechanisms, such as the disruption of cytochrome P45…
