Chest wall reconstruction in a patient with Cantrell syndrome.

Cantrell syndrome is a very rare congenital anomaly with up to five features: a midline, upper abdominal wall abnormality, lower sternal defect, anterior diaphragmatic defect, diaphragmatic pericardial defect, and congenital abnormalities of the heart. This report describes our experience of performing a reconstruction of a chest wall defect in a Cantrell syndrome case with herniation of the heart. The patient was a 1-month-old female infant who received surgical patch repair of a ventricular septal defect (VSD) and atrial septal defect (ASD) at the Department of Cardiac Surgery. Subsequently,

The human voice aligns with whole-body kinetics.

Humans often vocalize while concurrently gesturing with their hands. Fluctuations in the intensity and tone of the voice have been shown to synchronize with gestural upper limb movement. This research provides direct evidence that (postural) muscle activity associated with arm movements predict these voicing fluctuations. We show that specific muscles (e.g. pectoralis major, erector spinae), associated with upper limb movement and their postural anticipations, are especially likely to interact with the voice. Adding a 1 kg mass to the upper limb increased this interaction. Ground-reaction forc

Modified Total Laryngectomy With Strap-Flap Technique in a Patient With Bovine Aortic Arch Anomaly: A Case Report and Literature Review.

BACKGROUND: Total laryngectomy is an essential surgical intervention in the management of advanced laryngeal carcinoma. However, anatomical variations such as the bovine arch can complicate this procedure. The atypical branching pattern of the bovine arch may position major vessels closer to the tracheostomy site, increasing the risk of tracheo-arterial fistulization. METHODS: We present the case of a 71-year-old woman with bovine arch anatomy who underwent total laryngectomy. Preoperative assessment revealed an aberrant right common carotid artery (RCCA) located close to the tracheostomy sit

A Review on Functional Electrical Stimulation in Tetraplegic Patients to Restore Hand Function.

The purpose of this collective review is to describe a new form of functional electrical stimulation called neuroprosthesis. This unique technology has been devised to produce lateral pinch and palmar grasp in persons with C5 and C6 motor level spinal cord injuries. This neuroprosthesis includes external as well as implanted components. First, a receiver is surgically implanted into the patient's chest above a pectoralis major muscle. The receiver stimulator is then connected to 8 surgically implanted epimysial or intramuscular electrodes. Restoration of upper extremity function can greatly im

[Circulatory assistance through cardiomyoplasty and aortomyoplasty. Experience and first clinical results].

The finding that skeletal muscles can be made resistant to fatigue by progressive electrical stimulation has been used as a means of providing circulatory support in cardiac surgery. The first application of this discovery was dynamic cardiomyoplasty, performed for the first time in man in 1985 at the Broussais Hospital, Paris. The latissimus dorsi muscle is transposed into the thorax, then attached around the heart and finally stimulated synchronously with the ventricular systole. So far, more than 200 patients in the whole world (including 57 at the Broussais Hospital) have undergone this op

[Role of musculocutaneous flaps in pelvibuccopharyngectomy].

Musculocutaneous flaps were used for reconstruction after excision of large base of tongue or lateral oropharyngeal wall tumors in 50 patients, the most frequently employed flap in both regions being one from pectoralis major. Results showed marked reliability for these types of flaps for repair in these two areas, and they have now supplanted conventional single pedicle skin flaps. Various flaps supplying different useful surfaces are required, the platysma or sternocleidomastoid muscles being used for small areas, the pectoral or trapezius muscles for medium sized gaps and the latissimus dor

Swimming as Treatment of Scapular Dyskinesis.

Scapular dyskinesis is quite frequent and can lead to shoulder pain. The diagnosis is essentially clinical. The main cause is muscle imbalance, between the trapezius, rhomboids, and pectoralis minor. In these cases, rehabilitation is the best treatment. We present a case of a young male patient with dyskinesis due to axonal involvement of the long thoracic nerve and paresis of the anterior serratus muscle. After a swimming program to increase muscular strength and imbalance, he experienced pain reduction and functional recovery of the upper limb, with reduction of the winged scapula.

Poland's syndrome and vascular malformations.

Unilateral hypoplasia of the breast and the pectoralis muscle with a missing anterior axillary fold as part of Poland's syndrome are of major concern, especially for women. The latissimus dorsi is one of the most suitable flaps for breast and anterior thorax reconstructions but it may be hypoplastic or absent. If so, a free tissue transfer of the contralateral latissimus dorsi muscle is the next possible option for reconstruction. As Poland's syndrome is additionally associated with vascular malformations of the diseased hemithorax such as hypoplastic or missing vessels, a preoperative angiogr

The abnormalities of trapezius muscle might be a component of Poland's syndrome.

Poland's syndrome is a rare unilateral congenital anomaly characterized by the absence of the pectoral muscle and hand anomalies. By the time, new components including the absence or hypoplasia of many muscles have been identified, however, the anomalies of trapezius muscle have not been reported in patients with Poland's syndrome. The accepted etiological theory is the temporary interruption of blood supply of the subclavian artery and its branches in the early gestational period. The artery of the trapezius muscle is also one of the branches of subclavian artery. Just because of that, it is
