OBJECTIVE: To assess the value of the second neonatal examination as a medical surveillance procedure.
DESIGN: Prospective survey of routine neonatal examinations and the abnormalities identified during 8 March-30 June 1988.
SETTING: Maternity unit with an annual birth rate of 5700.
SUBJECTS: For first neonatal examination: 1795 babies born in the unit during the 115 day observation period. For second routine examination: 1747 babies (97.3%) discharged from postnatal ward.

Eleven horses (3 mares, 7 stallions, 1 gelding) with clinical and biochemical evidence of hyperkalemic periodic paralysis were studied. For the first time in Bulgaria a case of hypokalemic periodic paralysis in thyrotoxicosis is described. Three periods of paralysis were observed and documented in a hospital. The thyrotoxicosis was well manifested with disturbed carbohydrate tolerance and responded favourably to methizole treatment. It is suggested that the mechanism for the development of the hypokalemic periodic paralysis is the intracellular blockade of potassium by the surplus of thyroid hormones.

A 50-year-old man with thyrotoxicosis complained of recurrent bouts of weakness. On admission all his voluntary muscles were paralyzed except for those of the face, and hypokalemia was found. Thyrotoxic, hypokalemic periodic paralysis is very rare among Caucasians. This is the first case to be reported from Israel.

Of 21 patients with clinically definite hypokalemic, hyperkalemic, or normokalemic periodic paralysis, 15 (71%) had a greater than normal increase in compound muscle action potential amplitude during 2-5 minutes of intermittent strong voluntary contraction of the muscle. This increase was followed by a progressive decline in amplitude, which was greater than in a control population and which was most rapid during the first 20 minutes after exercise. The authors report the case of an 8 year-old girl admitted for an erysipelas-like rash of the lower limbs and an episode of Henoch-Schönlein purpura. These manifestations were subsequently attributed to familial mediterranean fever. This case report illustrates the polymorphism of cutaneous manifestations in this disease.

With the acquisition of specialized laboratory techniques, as the determination of serum immune complexes and the auditory evoked potentials, which bring new knowledge, the study of syphilis must be reviewed, in particular the classification of the evolution periods. The authors consider latent syphilis and neuro-syphilis. Based on these facts the new ways of treatment as well as the evolution control must be revised.
