Autonomic and peripheral nerve function in early diabetic neuropathy. Possible influence of a novel aldose reductase inhibitor on autonomic function.

Autonomic and peripheral nerve functions as well as the possible short-term effect of a novel aldose reductase inhibitor (ARI) on neuropathy were evaluated in 30 male type I diabetics (age 25-44 years, mean 34; duration of diabetes 10-20 years, mean 34) with neurographic signs of peripheral neuropathy (PN). Autonomic neuropathy (AN) was established by the heart rate reactions to deep breathing (E/I ratio = vagal function) and to tilt (acceleration index = sympathetic and vagal functions; the brake index = vagal function). Twenty-nine patients, 13 with AN, completed the study. Among neurographi

Potentiation of antiaggregating activity of adenosine by a phosphodiesterase inhibitor, EG626 (oxagrelate), in human platelets in vitro.

EG626 (oxagrelate), a specific inhibitor of cyclic AMP phosphodiesterase, produced in vitro a concentration-dependent inhibition of platelet aggregation induced by collagen and ADP in human platelets. When adenosine was added to the platelet rich plasma (PRP) in the presence of a threshold concentration of EG626, the potency of adenosine in inhibiting platelet aggregation was markedly potentiated. This potentiating effect of EG626 proved to be synergistic, but not additive and was accompanied by a marked accumulation of cyclic AMP in the platelets. The antiaggregating and cyclic AMP increasing

Genomic profiling identifies actionable DNA-repair defects in a new cervical cancer model.

Genetic profiling of a new HPV-negative cervical cancer model identified pathogenic variants in genes implicated in oncogenic signaling, cell cycle regulation, and DNA damage repair pathways, including homologous recombination, non-homologous end-joining, and mismatch repair. Deficiencies in BRCA2, RAD51 and MLH1 were among these actionable targets. The newly described cervical cancer model revealed sensitivity towards the PARP inhibitor olaparib, which was further augmented upon combination with platinum-based chemotherapeutics. In contrast, BRCA1/2-proficient cervical cancer cells exhibited

Heterozygous BRCA1 SNPs in the Central Functional Region are Associated With Increased Sensitivity to PARP Inhibition in Breast Cancer Cells.

INTRODUCTION: Poly (ADP-ribose) polymerase (PARP) inhibitors demonstrate efficacy in breast cancer patients harboring pathogenic BRCA1/2 mutations. However, certain BRCA1 single nucleotide polymorphisms may alter protein function without complete loss of activity. This study investigates whether specific heterozygous BRCA1 single nucleotide polymorphisms within the central functional region of BRCA1 affect cellular sensitivity to PARP inhibition and explores the underlying transcriptomic mechanisms. MATERIALS AND METHODS: Two breast cancer cell lines were employed: CAMA-1, carrying heterozygo

Impact of prior olaparib use on subsequent platinum-based therapy in recurrent ovarian cancer.

OBJECTIVE: Epithelial ovarian cancer (EOC) has the highest mortality among gynecologic malignancies, with frequent recurrences despite initial responsiveness to platinum. Poly(ADP-ribose) polymerase (PARP) inhibitors, such as olaparib, have improved progression-free survival in BRCA-mutated and homologous recombination-deficient EOC. However, emerging evidence suggests that prior exposure to PARP inhibitors may reduce the efficacy of subsequent platinum-based chemotherapy, raising concerns about treatment sequencing. To evaluate whether prior olaparib use affects the sensitivity to subsequent

Exposure-safety analyses of talazoparib in combination with enzalutamide in patients with metastatic castration-resistant prostate cancer (mCRPC) in the TALAPRO-2 trial.

The TALAPRO-2 trial (NCT03395197) showed that the addition of talazoparib, a potent PARP inhibitor, to enzalutamide significantly improved radiographic progression-free survival in patients with mCRPC. Due to adverse events (AEs), approximately 62% of patients experienced dose interruption and 53% of patients experienced dose reduction of talazoparib in TALAPRO-2 trial. Hematologic AEs such as anemia, thrombocytopenia, and neutropenia were the most common AEs leading to dose interruptions or reductions. We investigated the relationship between talazoparib exposure as well as baseline patients/

A Gene Expression Signature that Predicts Gastric Cancer Sensitivity to PARP Inhibitor Therapy.

BACKGROUND/AIM: Biomarkers indicating sensitivity to poly ADP-ribose polymerase (PARP) inhibitors have not yet been identified in gastric cancer. PARP inhibitors target homologous recombination deficiency (HRD); however, homologous recombination (HR) induces complex changes in gene expression, which makes it difficult to identify reliable biomarkers. In this study, we identified a multi-gene expression signature as a marker of PARP inhibitor sensitivity in gastric cancer. MATERIALS AND METHODS: Seven gastric cancer cell lines were evaluated for susceptibility to PARP inhibitors using a growth

Using PARP Inhibitors in the Treatment of Patients With Ovarian Cancer.

Use of poly(ADP-ribose) polymerase (PARP) inhibitors has greatly increased over the past 5 years. With several new Food and Drug Administration (FDA) approvals, three PARP inhibitors have entered into standard of care treatment for epithelial ovarian cancer (including ovarian, fallopian tube, and primary peritoneal cancer). Olaparib and rucaparib currently have indications for treatment of recurrent BRCA mutant ovarian cancer. Olaparib, rucaparib, and niraparib all have indications for maintenance therapy in recurrent platinum-sensitive ovarian cancer after response to platinum-based therapy.

Double-blind, placebo-controlled trial of reformulated azelastine nasal spray in patients with seasonal allergic rhinitis.

BACKGROUND: Azelastine nasal spray is a topical antihistamine with a distinctive taste that may be objectionable to some patients. The primary objectives of this clinical trial were (1) to determine if a reformulated azelastine nasal spray (Astepro) with sucralose as a taste-masking agent provides comparable efficacy to the original formulation (Astelin) and (2) to evaluate dose-response relationships between groups. METHODS: Eight hundred thirty-five patients with seasonal allergic rhinitis were randomized to six treatment groups: (1) original azelastine nasal spray, 1 spray/nostril b.i.d.;

Duration of action of topical antiallergy drugs in a Guinea pig model of histamine-induced conjunctival vascular permeability.

The topical application of 0.1% olopatadine has been shown to provide significant attenuation of histamine-induced conjunctival vascular permeability (CVP) within 5 min and for as long as 24 h following a topical administration. The duration of the action of olopatadine was compared to that of epinastine, azelastine, and ketotifen. Male Hartley outbred guinea pigs (weighing 250-300 g) were administered a drug or vehicle as single O.D. topical drops, at times ranging from 4 to 24 h prior to histamine challenge. One (1) h prior to histamine challenge, the animals were administered 1 mL of Evans

A new target for shigellosis: rational design and crystallographic studies of inhibitors of tRNA-guanine transglycosylase.

Eubacterial tRNA-guanine transglycosylase (TGT) is involved in the hyper-modification of cognate tRNAs leading to the exchange of G34 at the wobble position in the anticodon loop by preQ1 (2-amino-5-(aminomethyl)pyrrolo[2,3-d]pyrimidin-4(3H)-one) as part of the biosynthesis of queuine (Q). Mutation of the tgt gene in Shigella flexneri results in a significant loss of pathogenicity of the bacterium, revealing TGT as a new target for the design of potent drugs against Shigellosis. The X-ray structure of Zymomonas mobilis TGT in complex with preQ1 was used to search for new putative inhibitors wi

TET2 loss triggers ssDNA gap accumulation and heightened PARP inhibitor sensitivity.

Somatic mutations in the de novo methyltransferase DNMT3A and cytosine demethylase TET2 are common in age-related clonal hematopoiesis and hematopoietic neoplasms. TET2 mutations, unlike DNMT3A mutations, increase sensitivity to PARP inhibitors (PARPi). We find that TET2 loss, but not DNMT3A loss, alters DNA replication fork progression upon treatment with PARPi Olaparib, resulting in longer DNA replication tracts and increased DNA breaks. This reveals a new link between abnormal fork elongation and PARPi sensitivity in TET2-deficient cells. The elongated forks of TET2-deficient cells contain

A small molecule disrupts G4-STAT1 interaction and synergizes with olaparib to drive cancer cell death.

Bloom syndrome protein (BLM), a RecQ family DNA helicase, is consistently overexpressed in multiple malignancies, yet its therapeutic potential remains largely unexplored. Herein, we focused on targeting the BLM promoter G-quadruplex (BLM-G4) to inhibit the BLM signaling pathway. We first characterized the parallel BLM-G4 in the BLM promoter region. Subsequently, it is shown for the first time that BLM-G4 recruits phosphorylated signal transducer and activator of transcription 1 (pSTAT1) to activate BLM expression. Importantly, two natural alkaloids, berberine (BER) and coptisine (COP), compet

Molecular classification identifies aggressive gastrointestinal stromal tumor subtype targetable by PARP inhibitors.

PURPOSE: Gastrointestinal stromal tumors (GISTs), the most common gastrointestinal sarcomas, experience a high rate of resistance to standard treatment tyrosine kinase inhibitor (TKI) imatinib, leading to tumor progression. Identifying non-responders and offering precise alternatives are crucial, however, multi-omics studies of GIST are limited, which hinders GIST biology understanding and novel drug development. METHODS: We performed whole-exome and transcriptomic sequencing on 106 primary GIST patients. Subtype-specific molecular features and therapeutic responses were further validated in

Cleavage and inactivation of poly(ADP-ribose) polymerase in peripheral blood mononuclear cells of patients with acute respiratory distress syndrome.

Olaparib, the first clinically approved poly (ADP-ribose) polymerase (PARP) inhibitor, may be repurposed for non-oncological conditions such as acute respiratory distress syndrome (ARDS), where PARP-1 inhibition has shown benefits in preclinical models. We investigated the expression and functional status of PARP-1 and the effects of olaparib in peripheral blood mononuclear cells (PBMCs) from ARDS patients and healthy controls. PBMCs from healthy volunteers (N = 8) and ARDS patients (N = 8) were isolated via Ficoll gradient. PARP-1, cleaved PARP (cPARP), and PAR polymers were assessed by Weste

Targeting homologous recombination deficiency with intensified chemotherapy versus standard chemotherapy followed by olaparib in stage III breast cancer (SUBITO): an open-label, randomised, controlled, phase 3 trial.

BACKGROUND: Patients with stage III, human epidermal-growth-factor-receptor 2 (HER2; also known as ERBB2)-negative breast cancer with homologous recombination deficiency (HRD) had a 4-year overall survival of 35% after anthracycline-based chemotherapy versus 78% after intensified alkylating chemotherapy with autologous stem cell rescue (IACT) in a post-hoc analysis of an earlier randomised controlled trial. In this study, we aimed to prospectively assess 4-year overall survival with IACT and establish whether this approach remains superior to a contemporary HRD-targeting regimen in patients wi

GNL3 SUMOylation is essential for DNA double-strand break repair by homologous recombination.

DNA double-strand break repair via homologous recombination is critical for maintaining genomic integrity and requires proper DNA end resection. Here, we identify GNL3, a nucleolar GTP-binding protein, as a key regulator of homologous recombination in human cells via SUMOylation-dependent control of DNA end resection. Expression of wild-type GNL3, but not the SUMO-defective K196R mutant, abolished DNA damage induced by knockdown of endogenous GNL3. GNL3 interacts with the BLM-DNA2 helicase-nuclease complex, promoting DNA end resection and subsequent RPA and RAD51 loading. This interaction requ
