Use of home sleep studies for diagnosis of the sleep apnoea/hypopnoea syndrome.

BACKGROUND: A study was undertaken to test the hypothesis that unsupervised domiciliary limited sleep studies do not impair the accuracy of diagnosis when used to investigate the sleep apnoea/hypopnoea syndrome (SAHS) and can be cheaper than laboratory polysomnography. METHODS: For validation, 23 subjects with suspected SAHS underwent laboratory polysomnography and a home study (EdenTec 3711) on successive nights. All subjects with > 15 apnoeas + hypopnoeas (A + H)/hour on polysomnography showed > 30 A + H/hour on their home study. Thereafter, in a prospective trial 150 subjects had a home st…

Insomnia among methadone-maintained individuals: the feasibility of collecting home polysomnographic recordings.

Sleep disturbance among methadone-maintained patients is highly prevalent. A full understanding of sleep disturbance requires polysomnographic measures along with subjective sleep quality measures. The goal of this study was to describe the authors' experiences in performing at-home unattended polysomnography in this population. Participants had a Pittsburgh Sleep Quality Index score of six or higher, which indicates clinically significant insomnia, and 65% of eligible individuals agreed to enroll. Among 88 participants (53% female, 82% white, and a mean methadone dose of 105 mg/day), each und…

Evaluation of a portable device for diagnosing the sleep apnoea/hypopnoea syndrome.

Waiting times for hospital-based monitoring of the obstructive sleep apnoea/hypopnoea syndrome (OSAHS) are rising. This study tested whether Embletta, a new portable device, may accurately diagnose OSAHS at home. A synchronous comparison to polysomnography was performed in 40 patients and a comparison of home Embletta studies with in-laboratory polysomnography was performed in 61 patients. In the synchronous study, the mean difference (polysomnography-Embletta) in apnoeas+hypopnoeas (A+H) x h(-1) in bed was 2 h(-1). In comparison to the apnoea/ hypopnoea index (AHI) x h(-1) slept, the Embletta…

High night-to-night variability in childhood obstructive sleep apnea severity.

STUDY: Overnight in-laboratory polysomnography is the gold standard test for childhood obstructive sleep apnea (OSA). Night-to-night variability is a well-documented phenomenon in adult literature but is poorly characterized in children. We evaluated the test-retest reliability of polysomnography in children with moderate-severe OSA. METHODS: This is a secondary analysis of two pediatric trials evaluating OSA therapies for children aged 4-18 years old with moderate-severe OSA. The baseline diagnostic polysomnography data were compared to repeat polysomnography data in trial. RESULTS: Twenty-…

Vallecular cyst as a cause of obstructive sleep apnea in an infant.

A 3-month-old baby was diagnosed with obstructive sleep apnea (OSA) on polysomnography (PSG) with a high apnea hypopnea index (AHI). On further investigations he was found to have a vallecular cyst that was successfully treated. We discuss the clinical presentation of vallecular cysts and the importance of polysomnography in identifying this rare condition.

[Assessment of the MESAM and Biox 3700 early warning methods for the diagnosis of sleep-related respiratory disorders in young men].

We can use pulse oximetry as a method for diagnosis of the sleep apnoea syndrome, as well as L-EKG, MESAM II and the actigraph. In this study MESAM II and pulse oximetry were combined for finding out discrete sleep-related breathing disorders. The results were compared with polysomnography. 5 (15.1%) of 32 patients had an index of apnoea (AI) greater than 10, whereas 4 (12.1%) patients showed an AI greater than 5. All of the patients with positive signs in MESAM II/pulse oximetry had an AI greater than 5 in polysomnography, whereas non of the patients with negative signs in MESAM II/pulse oxim…

Normal Neonatal Sleep Defined: Refining Patient Selection and Interpreting Sleep Outcomes for Mandibular Distraction.

BACKGROUND: Although polysomnography is paramount when evaluating neonatal airway obstruction, "normal" published references do not exist. We present normative polysomnography data for newborns age 0-1 month. We compare this reference to pre and postoperative sleep data from infants undergoing mandibular distraction osteogenesis (MDO) at this same age. METHODS: Following IRB approval, normative subjects were recruited from our neonatal intensive care unit to undergo nap polysomnography. One blinded sleep physician read all studies. From 2016 to 2019, we prospectively collected sleep data for …

Interpretation of the adult polysomnogram.

Polysomnography provides information on the physiological changes occurring in many different organ systems in relation to sleep stages and wakefulness. It allows qualitative and quantitative documentation of abnormalities of sleep and wakefulness, of sleep-wake transition, and of physiological function of other organ systems that are influenced by sleep. Polysomnography is considered to be the "gold standard" for diagnosing sleep disordered breathing (SDB) and other sleep disorders; however, as with most other diagnostic tests, polysomnography is not ideal, but is rather the best available me…

Who should be tested in the sleep laboratory?

Sleep laboratory testing with nocturnal and daytime polysomnography is an indispensable procedure for the evaluation of patients with sleep-disordered breathing, pathological excessive daytime sleepiness, and complex motor disorders. Titration of positive airway pressure apparatuses should be performed in conjunction with polysomnography. A sleep laboratory test yields best results when the procedure is based on proper clinical evaluation of the disorder.

Asymptomatic sleep abnormalities are a common early feature in patients with Huntington's disease.

Huntington's disease (HD) is a fatal neurodegenerative disease characterized by motor, cognitive, and psychiatric disturbance. In this article, we used polysomnography, actigraphy and a variety of validated questionnaires to ascertain the extent to which sleep changes are identifiable and measurable in mild stage HD, and importantly, to see whether patients are negatively impacted by the changes in their sleep. We found significant differences in sleep architecture and sleep efficiency in patients compared with controls using polysomnography. However, patient scores on the Functional Outcomes …
