Baker's cysts are located in the posteromedial region of the knee between the medial belly of the gastrocnemius muscle and semimembranosus tendon. In adults, these cysts are related to intra-articular lesions, which may consist of meniscal lesions or arthrosis. In children, these cysts are usually found on physical examination or imaging studies, and they generally do not have any clinical relevance. Ultrasound examination is appropriate for identifying and measuring the popliteal cyst. The main treatment approach should focus on the joint lesions, and in most cases there is no need to address…

AIMS: We report the natural course of Baker's cysts following total knee arthroplasty (TKA) at short- and mid-term follow-up.
METHODS: In this prospective case series, 105 TKA patients were included. All patients who received surgery had a diagnosis of primary osteoarthritis and had preoperatively presented with a Baker's cyst. Sonography and MRI were performed to evaluate the existence and the gross size of the cyst before TKA, and sonography was repeated at a mean follow-up time of 1.0 years (0.8 to 1.3; short-term) and 4.9 years (4.0 to 5.6; mid-term) after TKA. Symptoms potentially attrib

Cystic adventitial disease (CAD) of the popliteal artery is a rare vascular disease of unknown etiology in which a mucin-containing cyst develops in the adventitial layer of the artery. Early recognition and treatment of this condition will prevent progression of the popliteal thrombosis and critical ischemia. However, diagnosis of the condition is difficult. The combination of contrast-enhanced magnetic resonance (MR) angiography and the excellent soft-tissue resolution of MR imaging allow for detailed evaluation of patients with CAD and facilitate appropriate management decisions. We report

A hydatid cyst is a parasitic disease which is caused by a cestode named Echinococcus and often located in the liver. Localization in the bone and soft tissue is rarely seen. In this article, we present a 64-year-old male case with a hydatid cyst located in the soft tissue of the left anterolateral aspect of the thigh. The hydatid cyst starting from the gluteal region proximally into the pelvis, the femoral head and the hip joint leading to the spread of secondary protrusio acetabuli and extending distally to the popliteal region formed a massive lesion. In addition to medical treatment follow

INTRODUCTION: Synovial cyst is composed by a fibrous wall; lining by a thin layer of synovial cells containing synovial fluid, the prototype of these, in the knee is the Baker's cyst, which is located abnormally in the gastrocnemius semimembranous bursa. Baker's cyst prevalence ranges from 5 - 38%. Clinical diagnosis is supported by the presence of increased volume of soft tissues located in the popliteal region.
CLINICAL CASE: A 74 year-old woman with longstanding active rheumatoid arthritis who developed a large, recurrent Baker's cyst. The Baker's cyst had two flare-ups of pain and soft ti

Venous aneurysms are rare and seldom give rise to specific symptoms. We describe a case of a popliteal vein aneurysm in a 55-year-old patient in whom the lesion was at first misinterpreted as being a Baker's cyst. The lesion was successfully treated by segmental resection and venous interposition grafting.

Multiple hereditary exostosis (MHE) is a rare autosomal-dominant disorder marked by multiple osteochondromas arising from the metaphyses of long bones. Parameniscal cysts, often linked to meniscal tears, can also expand and compress nearby vessels, posing a potential vascular risk. We present a 28-year-old male with MHE and a parameniscal cyst causing popliteal artery compression. The patient's intermittent claudication resolved completely following surgical cyst decompression and meniscal repair. This case report highlights the importance of considering vascular compression in patients with M

The syndrome of pseudothrombophlebitis is a well-known complication of popliteal cysts. We report the case of a patient with a neuropathic arthropathy of the shoulder in whom pseudothrombophlebitis of the upper extremity subsequently developed. To our knowledge, this is the first such case that has been reported.

We report two cases of cystic adventitial degeneration of the popliteal artery in young sportsmen. The pathogenetic role of microtrauma is suggested by the localisation of the adventitial cysts, the clinical symptoms, as well as the immunohistochemical demonstration of foamy macrophages instead of endothelial cells in the wall of the cysts. In one case the cause of the intermittent claudication could be verified by duplex sonography and comparison of the arterial pressure of both legs with a Doppler method only after physical exercise. As expected, an angiodilatation has only a temporary effec

RATIONALE: Peroneal neuropathy is the most common type of peripheral neuropathy in the lower extremities. The peroneal nerve is usually compressed at the lateral aspect of the fibular head. Compression by ganglion cysts are one of the numerous underlying etiologies for peroneal nerve neuropathy and are most frequently located around the fibular neck and proximal tibiofibular joint. To the best of our knowledge, this is the first report of an extraneural ganglion cyst located at the level of the distal thigh that resulted in compressive peroneal neuropathy.
PATIENT CONCERNS: We report a case o

Adventitial cystic disease of the popliteal artery is explored. The results of correspondence with authors reporting this condition are elaborated upon. This has provided an opportunity to discuss the history of the condition, the findings in 115 cases which have come to the attention of the Correspondence Office dealing with this entity, and the results of treatment. A discussion of the suspected etiology of the condition is presented. The condition remains one of unknown etiology which can be treated by cyst evacuation or aspiration when the popliteal artery is patent and which is best treat

Type 1 neurofibromatosis is one of the most common autosomal-dominant disorders and often includes orthopedic manifestations. We report the case of a 48-year-old woman with hemarthrosis caused by a popliteal cyst infiltrated by a diffuse neurofibroma associated with angiodysplasia. Surgical resection of this tumor was followed by postoperative hemorrhagic complications.

Adventitial cystic disease (ACD), also known as cystic mucoid or myxomatous degeneration, is a rare vascular disease seen mainly in arteries. It is very unusual for these cystic masses to develop in a vein. We report the case of a 56-year-old woman with leg swelling caused by ACD arising in the popliteal vein. The swelling appeared after a long period of standing. Magnetic resonance imaging (MRI) showed a popliteal cystic mass and venography showed disrupted venous flow. We resected the cyst wall without venous reconstruction, after which venous blood flow normalized and her symptoms subsided.…

The MR images of the knee joint of 100 successively examined patients were evaluated in retrospect with regard to periarticular cystic lesions. There were 18 popliteal cysts, 8 meniscal cysts and 4 ganglion cysts. Localisation and internal structure of the lesions were of main importance in respect of differential diagnosis. All meniscal cysts were associated with horizontal meniscal tears. Spin-echo and 3D gradient-echo images did not enable differentiation because of signal intensity values. A spin-echo sequence with long repetition time in double-echo technique was best suited to assess the…

Popliteal or Baker's cysts, often found incidentally during examination of the knee, are common. The authors report the magnetic resonance imaging (MRI) manifestations of a leaking Baker's cyst that initially presented as thrombophlebitis in a patient who had undergone reconstruction of the anterior cruciate ligament 4 months before; no cyst was present at the time of the surgery. Marked inflammatory changes apparent in the surrounding muscle and subcutaneous tissues were best appreciated in T2-weighted and fat-suppressed inversion recovery images. Although the results of ultrasonography sugge

The Authors have studied, by means of the arthrographic technique, 24 cases of knee osteoarthrosis in various clinical phases. In the diagnosis of the early phases of the disease, this technique as been helpful in showing alterations, as the widening of the intercartilaginous space, the irregularities of synovial outlines, the enlargement of the normal popliteal space, which common techniques, unusually cannot display. The assay is not needed in the most advanced phases of the disease but it could here as well supply further diagnostic data (i.e. popliteal cyst, etc.). Therefore the authors su…

Baker's cysts as a clinical entity are among the most infrequent cause of peripheral nerve entrapment and usually produce a strong positive pressure within the knee resulting in the rupture of the joint capsule. An unusual case with muscular and sensorial weakness due to compression of the peroneal nerve around the fibular head by a Baker cyst is presented in this article. Clinical and electromyographical findings shown peroneal nerve entrapments in the popliteal fossa. Peroneal nerve decompression by synevectomy resulted in clinical and electromyographical improvement.
