The iron metabolism has been studied in 11 patients with porphyria cutanea tarda. Despite significantly increased amounts of iron in the serum and liver, the porphyria patients absorb not less but significantly more radioiron than normal controls. Whereas phlebotomy-induced iron deficiency stimulates the absorption in controls, no further stimulation is found in the porphyria patients. The plasma iron turnover is high in the porphyria patients, and some of this iron is probably deposited in the liver. In 40 patients suffering from porphyria cutanea tarda the sera were examined for the existence of HBAg and HBAb. Hepatitis was known in the history of 6 patients, in 3 of 23 bioptically examined livers a chronic aggressive hepatitis could be proved. When the transmigration electrophoresis after Pesendorfer and coworkers was used, in no case HBAg and HBAb could be proved.

The clinical and biochemical findings in two cases of porphyria cutanea tarda (PCT) are presented. The cases are unique in their early age of onset (24 and 22 years) and their apparent precipitation by low-dosage oral contraceptives.Review of the literature reveals these to be the two youngest patients with estrogen-induced PCT. Only one other case of PCT induced by low-dosage oral contraceptives has been reported. In most cases of PCT induced by estrogens, high dosages have been taken by patients over age 35.
