A rare subtelomeric deletion syndrome: Wolf Hirschhorn syndrome.

Wolf-Hirschhorn syndrome is caused by a deletion of the distal portion of the short arm of chromosome 4, and is characterized by psychomotor retardation, seizures, congenital malformations, and typical facial appearance including 'Greek warrior helmet' appearance of the nose. The form and the severity of clinical manifestations vary according to the size and location of the deletion. Major complications are severe growth retardation, developmental delay, seizures, feeding difficulties due to hypotonia, and predisposition to respiratory infections. Patients will benefit from supportive therapy

Embryonal rhabdomyosarcoma of the tongue.

We report a 10-month-old girl who, at 4 months, developed a small, reddish, plaquelike lesion on her tongue. This lesion began to enlarge rapidly, resulting in difficulty in swallowing and breathing. On physical examination, there was a large, red, friable, hard in consistency, irregular tumor over the distal portion of her tongue. The histopathology and immunohistochemical findings were consistent with an embryonal rhabdomyosarcoma. The patient was assigned clinical stage I, group III, and began treatment according to the Intergroup Rhabdomyosarcoma Study guidelines, on triple-agent chemother

De novo development of a lesion with the appearance of a cavernous malformation adjacent to an existing developmental venous anomaly.

A case is presented in which a complex multicystic hemosiderin-containing lesion developed adjacent to a previously documented developmental venous anomaly (venous angioma). This lesion had the characteristic MR imaging appearance of a cavernous malformation. Follow-up MR imaging demonstrated a decrease in both the size and complexity of this lesion, which suggests at least a portion of the lesion was due to sequelae of hemorrhage. This case further supports the association of a de novo, hemosiderin-containing lesion in association with developmental venous anomaly. Implications of these findi

[Intrathoracic neurofibroma originating in the left vagus nerve].

A 20-year-old man was admitted because of an abnormal mass shadow on chest X-ray film. Computed tomography (CT) and magnetic resonance imaging (MRI) disclosed a mass lesion in the superior portion of the left mediastinum. CT scans showed a well-defined mass with low density. Axial MRI rendered the mass lesion with intermediate signal intensity on T1-weighted images and high signal intensity on T2-weighted images. The preoperative diagnosis was bronchogenic cyst. Video-assisted thoracic surgery revealed that the tumor originated in the truncus of the left vagus nerve. The resected tumor was 90

Community mental handicap nursing services in Northern Ireland: a profile of clients and selected working practices.

The aim of this survey of community mental handicap nursing in Northern Ireland (NI) was, among other things, to construct a profile of clients of the service as well as to describe selected working practices. A 25-item questionnaire was developed and administered to all 50 community nurses working with people with learning disabilities in NI. The number of responses was 36, giving a rate of 72%. Fifty-six percent of the clients were female. Young to middle-aged adults made up the largest portion of the caseload. Over 80% of the clients had no history of hospital admission. Caseload size also

Long-term follow-up of the residual intracanalicular tumours after subtotal removal of acoustic neurinomas.

We examined growth potential of residual intracanalicular tumours left from subtotal removal of large acoustic neurinomas. Eleven patients were followed-up by magnetic resonance (MR) imaging. The interval between surgery and MR study ranged from 12 to 29 years (median, 16 years). MR images of two patients showed no evidence of tumour remnant, and in six a small tumour was localized in the internal auditory canal. The other three showed an intracanalicular tumour protruding slightly towards the intracranial portion. This result suggests that the intracanalicular residual tumours have less risk

Nitric oxide synthase immunoreactive neurons in the rat kidney.

The presence of neurons with nitric oxide synthase (NOS) immunoreactivity was investigated in the rat kidney. Whole kidneys were examined by means of serial sections. The indirect immunocytochemical technique using polyclonal antibody raised against rat brain type Ia NOS and the histochemical technique for nicotinamide adenine dinucleotide phosphate (NADPH)-diaphorase are used in this study. NOS-immunoreactive (NOS-IR) neurons varied in size and were observed: (1) associated with nerve bundles at the hilus of the kidney, (2) in the proximity of the lower or middle portion of the interlobar art

Reverse Distally Based Abductor Hallucis Muscle Flap for Soft Tissue Coverage of the First Metatarsophalangeal Joint Wounds.

Loss of soft tissue coverage distally around the foot poses threats of amputation of the exposed boney structures. An amputation of a portion of the foot leads to loss of the biomechanical structural integrity of the foot. This promulgates an imbalance with its inherent risks of developing new ulcers. This in turn potentiates the limb loss cycle. The reverse abductor hallucis muscle flap is ideally suited for small to moderate-sized defects in the vicinity of the first metatarsophalangeal joint based on its arc of rotation. In this article, we present cases of 5 patients who failed local wound

Evidence of a direct projection from the medial terminal nucleus of the accessory optic system to lobule IX of the cerebellar cortex in the tree shrew (Tupaia glis).

Injections of a wheat germ agglutinin-horseradish peroxidase conjugate were placed in representative areas of the cerebellar cortex of adult tree shrews. Retrogradely labeled cells were found in the medial terminal nucleus (MTN) of the accessory optic system (AOS) when ventral portions of the uvula were involved in the injection site. These labeled cells are small to medium sized and have round or oval somata and few primary dendrites. The AOS functions to stabilize the eyes and head in space. The present observations provide evidence of a direct projection from the MTN of the tree shrew AOS t

Testing fundamental hypotheses of colonization success in the ferns.

BACKGROUND AND AIMS: Non-native species are now ubiquitous members of regional floras. The factors that lead to establishment and dominance of non-native species are continuously debated. Fundamental hypotheses about drivers of invasion success include the role of phylogeny, polyploidy, genome size, and rapid niche evolution. These hypotheses have been tested in the seed plants, but ferns, the second largest group of vascular plants, have rarely been considered in these analyses, despite making up a non-trivial portion of non-native floras. METHODS: We compiled a dataset of global non-native

Painless giant angioleiomyoma in the subfascia of the lower leg.

Angioleiomyoma is rare, benign vascular smooth muscle tumor originating from the tunica media of the veins and arteries. It usually presents as a slow-growing, small, and painful mass arising from the cutaneous or subcutaneous tissue. We report an unusual case of angioleiomyoma that was located in the subfascia of the lower leg and had grown to massive size (≤5 cm in diameter) during a 3-year period without pain. A 57-year-old female presented with a 3-year history of a slowly growing pain-free mass on the anterior portion of her right lower leg, just above the ankle joint. Magnetic resonance

Microcephaly and hydrocephalus in a sheep fetus infected with Neospora caninum in Southern Brazil - Short communication.

A case of non-communicating hydrocephalus and microcephaly in a sheep fetus infected with Neospora caninum from Lages, Santa Catarina, Brazil, is reported. Macroscopically, there was moderate flattening and narrowing of the skull, and the portion of the cerebral hemispheres was markedly reduced in size, measuring 3.5 × 3.5 × 0.5 cm, with marked diffuse flattening of the brain gyri and dilation of the lateral ventricles. Cerebrospinal fluid samples were positive to N. caninum detection by PCR. Histologically, there was discrete focal lymphoplasmacytic necrotising encephalitis on the floor of th

Magnetic resonance imaging of the pediatric mediastinum: updates, tips and tricks.

Magnetic resonance imaging (MRI) of the pediatric mediastinum is challenging for the practicing radiologist. Many confounding factors add to the complexity of pediatric mediastinal MRI including small patient size, broad spectrum of mediastinal pathologies, motion artifacts and the need for sedation in a significant portion of children. However, with special attention to motion-reduction techniques and knowledge of pediatric-specific considerations, pediatric radiologists can help to provide accurate and timely diagnosis and also prevent multimodality imaging where MRI might be all that is nee

Segmental Aplasia of Bilateral Internal Carotid Arteries Accompanied by Intracranial Aneurysms: A Case Report.

A 68-year-old woman presented with segmental aplasia of bilateral internal carotid arteries accompanied by unruptured intracranial aneurysms. The abnormality was discovered incidentally at the age of 44 years. Cerebral angiography showed occlusion of bilateral internal carotid arteries, and the carotid territory was supplied by each posterior communicating artery with small intracranial aneurysms. Endovascular treatment for the intracranial aneurysms was planned. However, the patient did not want to undergo the endovascular procedure because of the increased risk due to the associated bilatera

A case of symptomatic Rathke's cyst.

Rathke's cleft cyst is a benign growth found on the pituitary gland in the brain, specifically a fluid-filled cyst in the posterior portion of the anterior pituitary gland. It occurs when the Rathke's pouch does not develop properly, and ranges in size from 2 to 40 mm in diameter. Asymptomatic cysts are common, detected during autopsies of 2-26% of individuals who have died of unrelated causes. Symptomatic cysts are rare and only approximately 150 cases have been reported. Females are twice as likely as males to have a cyst. Symptomatic cysts can trigger visual disturbances, pituitary dysfunct

18q deletions: clinical, molecular, and brain MRI findings of 14 individuals.

We studied 14 individuals with partial deletions of the long arm of chromosome 18, including terminal and interstitial de novo and inherited deletions. Study participants were examined clinically and by brain MRI. The size of the deletion was determined by segregation analysis using microsatellite markers. We observed that the phenotype was highly variable, even in two families with three 1st degree relatives. Among the 14 individuals, general intelligence varied from normal to severe mental retardation. The more common features of 18q-deletions (e.g., foot deformities, aural atresia, palatal

[A case of idiopathic fibrosing mediastinitis presented with bilateral pleural effusion].

A case is reported of a 56-year-old male who presented with bilateral pleural effusion as an initial manifestation of idiopathic fibrosing mediastinitis. The patient showed shortness of breath with severe loss of vital capacity and weight loss. A mediastinal mass surrounding the thoracic aorta and bilateral pleuritis was identified by the chest CT scan. The mass extended, along the abdominal aorta, to the upper portion of retroperitoneum. Laboratory data showed elevated levels of C-reactive protein (CRP), erythrocyte sedimentation ratio (ESR), and IgG. Biopsy of the mediastinal and the pleural

[Mediastinoscopic resection combined with a sternum lift technique in a case of thymic cyst].

We report on a case of thymic cyst which was successfully treated with mediastinoscopic resection combined with a sternum lift technique. A 62-year-old man was admitted to our hospital with a thymic cyst, which was causing chest discomfort and was increasing in size. The tumor was located above the pulmonary artery in the anterior mediastinum. He underwent mediastinoscopic resection while lifting the inferior portion of the sternum with a Laparolift. The cyst was resected sharply and bluntly with the Harmonic Scalpel without rupture. The patient was discharged on the 6th postoperative day in a

Sonographic detection of tears of the anterior portion of the rotator cuff (subscapularis tendon tears).

We studied the value of ultrasonography in identifying tears of the most anterior portion of the rotator cuff (subscapularis tendon tears). Shoulder sonography was performed on 1640 patients with shoulder symptoms, 435 of whom underwent surgery. Among the patients who were operated on, 17 had predominantly or exclusively subscapularis tendon tears; 14 of these were full-thickness and three were partial-thickness tears. Ultrasonography demonstrated 82% (14 of 17) of the subscapularis tendon tears; 86% (12 of 14) of the full-thickness tears and 67% (2 of 3) of the partial-thickness tears were co

Retrocardiac pneumomediastinum: radiographic finding and clinical implications.

Fourteen neonates with posterior mediastinal air collections (retrocardiac pneumomediastinum) are described. In the majority of the infants (13 of 14), the mediastinal air developed as a complication of assisted ventilation. It is suggested that the retrocardiac pneumomediastinum represents air trapped in a potential space in the mediastinum posterior to the heart and not within the pulmonary ligament. The collections have a variable shape and size and rarely may produce a tension phenomenon elevating the posterior, inferior portion of the heart off of the diaphragm. There was a strong associa
