Anti-TPO-Negative Subclinical Hypothyroidism in the First Trimester and Its Influences on Obstetric and Neonatal Outcomes.

The aim of this study was to assess the relationship between maternal anti-thyroid peroxidase (anti-TPO)-negative subclinical hypothyroidism (SCH) in the first trimester with complications of pregnancy and neonatal outcomes. The study was done at a maternity and children's research training hospital. First-trimester thyroid function tests (TFTs) (free thyroxine (FT4), thyroid stimulating hormone (TSH), anti-TPO) were checked for mothers who gave birth at the center, and their newborns were accepted for the study. Based on the results of the TFTs, two groups were formed, the normal thyroid funct

Risk Factors for Thyroid Function Test Abnormalities During Pregnancy: A Systematic Review of the Literature to Validate Current Risk Factors and Identify Novel Ones.

Background: International guidelines recommend that the indication to perform thyroid function testing during pregnancy is based on the presence of risk factors for thyroid function test abnormalities. However, the discriminative ability of currently recommended risk factors is questionable. To inform on an update of the American Thyroid Association Guidelines for the Diagnosis and Management of Thyroid Disease in Preconception, Pregnancy, and the Postpartum, we aimed to systematically review the literature to evaluate evidence for current risk factors and potential novel ones for thyroid func

Risk factors for postpartum depression: An evidence-based systematic review of systematic reviews and meta-analyses.

PURPOSE: Postpartum depression (PPD) is the most common psychiatric condition after childbirth which not only effects the mother's health, but also might have impact on child's development and parenting behaviors. Because the etiology of PPD has not been fully cleared, the efforts towards identification of risk factors are crucial for both the children and mother's health. METHOD: PubMed, EMBASE and PsycINFO databases were searched since inception until July 2019 to collect data about the risk factors of PPD and only systematic review and meta-analysis can be included. RESULT: To identify th

Reduced dopaminergic inhibition of thyrotrophin release in states of physiological hyperprolactinaemia.

We have tested the hypothesis that physiological puerperal hyperprolactinaemia may be secondary to reduced hypothalamic dopaminergic inhibition of prolactin (PRL) release. Nine post-partum females with physiological hyperprolactinaemia (aged 19-40 years; mean basal PRL +/- SE, 2099 +/- 257 mU/1, range 1002-3762 mU/1) were studied and results compared with fourteen normoprolactinaemic females (basal PRL less than 400 mU/1; aged 18-36 years). Puerperal hyperprolactinaemic females showed a reduced TSH response to dopamine (DA) receptor blockade with metoclopramide (10 mg i.v.) compared with norma

Association between maternal thyroid function and adverse outcomes of pregnant women and offspring: evidence from an umbrella review.

OBJECTIVE: To synthesize current evidence on the associations between maternal thyroid dysfunction and adverse outcomes in pregnant women and their offspring. METHODS: A systematic search of PubMed, Embase, Web of Science, and the Cochrane Database was conducted through September 2024. Systematic reviews and meta-analyses examining maternal thyroid function (i.e., subclinical/overt hypo- and hyperthyroidism, thyroid autoimmunity) and outcomes were included. The methodological quality and certainty of evidence were assessed using A Measurement Tool to Assess Systematic Reviews 2 (AMSTAR 2) and

[Studies on the relationship between reproductive phenomena and thyroid function--dynamics of the concentrations of thyroid hormones in pregnancy, parturition and puerperium].

Thyroid function during pregnancy, parturition and puerperium was studied by measuring the concentration of free thyroxine (f-T4), free triiodothyronine (f-T3) and reverse triiodothyronine (r-T3) in maternal and cord sera, and simultaneously metabolism of thyroid hormones in chorionic tissues was examined. Results obtained were as follows: In cases of normal pregnancy, maternal f-T4 and f-T3 levels tend to decrease during the course of pregnancy toward parturition. Pathological pregnancies such as intra uterine growth retardation (IUGR) and toxemia (H, h) showed a tendency for maternal f-T3 to

Pregnancy outcomes with thyroxine replacement for subclinical hypothyroidism: Role of thyroid autoimmunity.

OBJECTIVE: To study pregnancy outcomes in relation to thyroid peroxidase antibody (TPOAb) status with optimum thyroxine replacement for subclinical hypothyroidism. MATERIALS AND METHODS: Ninety-eight women with subclinical hypothyroidism were followed up until the end of their pregnancy. TPO antibody status was performed for 59 women (positive 20, negative 39). Levothyroxine was supplemented to maintain TSH between 0.3-3 mIU/l in all patients, irrespective of TPOAb status. Pregnancy outcomes were noted as pregnancy-induced hypertension (PIH), antepartum or postpartum hemorrhage, preterm deliv

Physiological Mechanisms in Pregnancy and Their Relevance to the Clinical Management of Perinatal Mental Illness.

Perinatal mental illness affects up to 20% of new mothers worldwide, yet despite a growing research interest over the past decade, the etiology is still not fully understood, and clinical treatment guidelines remain inconsistent across countries and services. In this review, recent findings on neurobiological processes and evolutionary mechanisms, as they occur during the menstrual cycle, pregnancy, birth, postpartum and breastfeeding, are discussed. The intention is to raise awareness of physiological changes in pregnancy that might be relevant to the differential diagnosis and clinical treat

Iodine supplementation for women during the preconception, pregnancy and postpartum period.

BACKGROUND: Iodine is an essential nutrient required for the biosynthesis of thyroid hormones, which are responsible for regulating growth, development and metabolism. Iodine requirements increase substantially during pregnancy and breastfeeding. If requirements are not met during these periods, the production of thyroid hormones may decrease and be inadequate for maternal, fetal and infant needs. The provision of iodine supplements may help meet the increased iodine needs during pregnancy and the postpartum period and prevent or correct iodine deficiency and its consequences. OBJECTIVES: To

Clinical evaluation of 3rd generation assay for thyrotropin receptor antibodies: the M22-biotin-based ELISA initiated by Smith.

Recently a new procedure for measuring serum TSH receptor (TSHR) autoantibody (TRAb) was reported by Smith et al. in which the autoantibodies inhibit binding of a human monoclonal thyroid stimulating antibody M22 (labeled with biotin) to TSHR-coated ELISA plate wells (pTRAb(3rd) assay). The aim of this study was to compare the performance of pTRAb(3rd) assay with pTRAb(2nd) assay based on inhibition of TSH-biotin binding to TSHR-coated ELISA plate wells. In addition, we evaluated the applicability of TRAb3rd assay to discriminate between untreated Graves' disease (GD) and painless thyroiditis

Thyroid in pregnancy: From physiology to screening.

Thyroid hormones are crucial for the growth and maturation of many target tissues, especially the brain and skeleton. During critical periods in the first trimester of pregnancy, maternal thyroxine is essential for fetal development as it supplies thyroid hormone-dependent tissues. The ontogeny of mature thyroid function involves organogenesis, and maturation of the hypothalamus, pituitary and the thyroid gland; and it is almost complete by the 12th-14th gestational week. In case of maternal hypothyroidism, substitution with levothyroxine must be started in early pregnancy. After the 14th gest

Thyroid function tests, incongruent internally and with thyroid status, both in a pregnant woman and in her newborn daughter.

INTRODUCTION: Thyroid function tests (TFT) are extensively used in daily clinical practice. Here, we described a case of incongruent TFT both in a pregnant woman and in her newborn. CASE PRESENTATION: A 32-year-old woman, diagnosed with autoimmune thyroiditis during her first pregnancy, was monitored during her second gestation. At week 5 + 2 days, thyroid-stimulating hormone (TSH) and free thyroxine (FT4) values (Dimension VISTA 1500, Siemens Healthineers) were within normal limits. At week 19 + 5 days, TSH remained normal while FT4 increased approximately by three-fold. FT4 inconsistency wa

Hyperthyroidism in pregnancy.

The prevalence of hyperthyroidism in pregnancy is about 0.2%. The most common cause is Graves' disease. Maternal, fetal, and neonatal morbidity and mortality may be reduced to a minimum with careful attention to the clinical symptoms and interpretation of thyroid tests. Ideally, hyperthyroid women should be rendered euthyroid before considering conception. The incidence of maternal and neonatal morbidity is significantly higher in those patients whose hyperthyroidism is not medically controlled. Even the incidence of thyroid storm is high in women who are under poor medical supervision in the

Pregnancy-specific Reference Intervals for TSH and FT4: Novel Insights From Preconception-gestation Longitudinal Data.

BACKGROUND: Pregnancy-specific changes in thyroid physiology have prompted the use of pregnancy-specific reference intervals to diagnose thyroid disease. However, such reference intervals are not widely available, and there is no evidence of their superiority over nonpregnancy reference intervals. Preconception data are understudied benchmarks to compare pregnancy-specific and nonpregnancy reference intervals. Moreover, the added value of free T3 (FT3) measurements, for example in cases of (subclinical) hyperthyroidism, remains to be quantified. METHODS: This study was embedded within Generat

Thyroid cancer and pregnancy: a systematic ten-year-review.

BACKGROUND: Thyroid cancer is the second most common malignancy during pregnancy, especially the well-differentiated thyroid cancer (well-DTC). Therefore, complex medical and social dilemmas arise, dealing with which requires deep knowledge of the nature and characteristics of the disease and pregnancy as a whole. The purpose of this review is to present the diagnostic and therapeutic strategies of thyroid cancer during pregnancy and the postpartum period. METHODS: Extended review of the literature [2011-2023] was performed. Two hundred ninety-six articles were found, from which 225 were excl

Onset of Graves' disease during pregnancy in a woman with established hypothyroidism.

BACKGROUND: Pregnancy strongly influences the thyroid gland and its function. Thyroid guidelines recommend a 30 to 50% increase of the preconceptional levothyroxine dose in women with hypothyroidism, when pregnancy is diagnosed. CASE: A 33 year-old, 8-week pregnant woman with hypothyroidism, presents with a 2-week history of palpitations, sweating, nervousness and fatigue. Physical examination shows tachycardia (108 bpm), distal tremors and diffuse goiter. After biochemical confirmation of hyperthyroidism, her levothyroxine dose is reduced and finally interrupted. Propylthiouracil is started
