Paediatric Acute Lymphoblastic Leukaemia: A Narrative Review of Current Knowledge and Advancements.

PURPOSE OF REVIEW: This review aims to provide an update on current knowledge regarding paediatric acute lymphoblastic leukaemia (ALL), focusing on recent advancements in diagnosis and treatment, as well as future directions in the field. RECENT FINDINGS: ALL is the most frequently diagnosed paediatric malignancy, with advances leading to a 90% survival rate. The heterogeneity of childhood ALL requires a precise diagnostic algorithm incorporating morphological, immunophenotypic, and cytogenetic analyses. Research is exploring next-generation sequencing and artificial intelligence-aided techni

Case Report: Pediatric myeloid/lymphoid neoplasm with eosinophilia and PDGFRA rearrangement: The first case presenting as B-lymphoblastic lymphoma.

According to the latest WHO classification of hematopoietic malignancies, myeloid and lymphoid neoplasms with eosinophilia and gene rearrangements include three specific rare diseases and one provisional entity. Myeloid/lymphoid neoplasms with platelet-derived growth factor receptor alpha (PDGFRA) rearrangements are the most frequent of these disorders and are usually present in adult males with a median age of the late 40s. Patients usually have chronic eosinophilic leukemia but can occasionally manifest as acute myeloid leukemia or extramedullary T- or B-lineage lymphoblastic lymphoma. We re

HIV-1 Rev-binding protein accelerates cellular uptake of iron to drive Notch-induced T cell leukemogenesis in mice.

Somatic activating mutations in Notch1 contribute to the pathogenesis of T cell acute lymphoblastic lymphoma (T-ALL), but how activated Notch1 signaling exerts this oncogenic effect is not completely understood. Here we identify HIV-1 Rev-binding protein (Hrb), a component of the clathrin-mediated endocytosis machinery, as a critical mediator of Notch-induced T-ALL development in mice. Hrb was found to be a direct transcriptional target of Notch1, and Hrb loss reduced the incidence or delayed the onset of T-ALL in mouse models in which activated Notch1 signaling either contributes to or drives

Long-term follow-up of CD19-CAR T-cell therapy in children and young adults with B-ALL.

The tremendous successes of CD19-directed CAR T cells in children and young adults with B-cell acute lymphoblastic leukemia (B-ALL) has led to the more widespread use of this important treatment modality. With an ability to induce remission and potentially lead to long-term survival in patients with multiply relapsed/chemotherapy refractory disease, more children are now receiving this therapy with the hope of inducing a long-term durable remission (with or without consolidative hematopoietic cell transplantation). While overcoming the acute toxicities was critical to its broad implementation,

Transient Differentiation-State Plasticity Occurs during Acute Lymphoblastic Leukemia Initiation.

Leukemia is characterized by oncogenic lesions that result in a block of differentiation, whereas phenotypic plasticity is retained. A better understanding of how these two phenomena arise during leukemogenesis in humans could help inform diagnosis and treatment strategies. Here, we leveraged the well-defined differentiation states during T-cell development to pinpoint the initiation of T-cell acute lymphoblastic leukemia (T-ALL), an aggressive form of childhood leukemia, and study the emergence of phenotypic plasticity. Single-cell whole genome sequencing of leukemic blasts was combined with

Complete remission in advanced blastic NK-cell lymphoma/leukemia in elderly patients using the hyper-CVAD regimen.

Although its cell of origin is still controversial, the blastic NK-cell leukemia/lymphoma clearly represents a distinct type of hematopoietic neoplasm that is particularly clinically aggressive when it occurs in elderly patients as a disseminated, multi-organ disease. Consistently effective treatments have not been developed for this malignancy. The present report describes two elderly patients with widespread blastic NK-cell leukemia/lymphoma involving the skin, bone marrow, peripheral blood, lymph nodes, and viscera. In both cases the malignant cells were CD56+, CD2+, and terminal deoxynucle

[Immunophenotyping of acute lymphoblastic leukemia in Mexican children].

PURPOSE: To analyse the immunophenotype of leukaemic cells in a group of children diagnosed of lymphoblastic leukaemia in order to assess the frequency of the different immunologic subtypes. PATIENTS AND METHODS: In the period comprised between APR 1987 and MAR 1995, 402 Mexican children were studied in a prospective way. Conventional immunological markers were used, either associated to or specific for B, T, myelo-monocytic or megakaryocytic-platelet cell populations. RESULTS: Five major immunologic subtypes were disclosed, showing a series of specific surface markers: null-ALL, 5%; early p

[Therapeutic approaches to hematological malignancies in adolescents and young adults].

Hematological malignancies (HM) comprise the major proportion of the cancer incidence and mortality in the adolescent and young adult (AYA) age group. Even though age-related differences in tumor biology have been reported in HM, further biological studies are warranted for elucidating age-related differences in AYAs. Hemato-oncologists and pediatric oncologists frequently follow various treatment strategies for HM, as the treatment strategy that is more suitable for this age group remains unclear. Recently, the involvement of pediatric oncologists has increased because the advantage of pediat

Distribution of ABO blood groups in acute leukaemias and lymphomas.

We studied the distribution of ABO blood groups in Hodgkin's lymphoma, non-Hodgkin's lymphoma, acute myeloid leukaemia and acute lymphoblastic leukaemia, in children up to the age of 12 years, in a hospital-based retrospective study. Blood group data were recorded from the case records of all the patients in a tertiary care centre with the diagnosis of Hodgkin's lymphoma, non-Hodgkin's lymphoma, acute myeloid leukaemia and acute lymphoblastic leukaemia, during the period 1987-1997. There were 63 Hodgkin's lymphoma, 78 non-Hodgkin's lymphoma, 116 acute myeloid leukaemia and 522 acute lymphoblas

Salmonelloses in children with proliferative haematological diseases.

The infections caused by salmonella organisms in children with acute leukaemias are reported. Immunity depression caused by the main diseases as well as by immunosuppressing drugs lead in such cases to generalized infections with these organisms or local infections which were found in the reported cases in the skeletal system, or central nervous system. The course of these infections was very severe, often with lethal outcome.

Primary laryngeal manifestation in precursor T-cell acute lymphoblastic leukemia.

Primary involvement of the larynx in non-Hodgkin's lymphoma (NHL) is rare. Early symptoms are non-specific and thus, it is difficult to diagnose. In the present study, the case of a 52 year-old male with hoarseness due to diffuse T-cell lymphoma as the first manifestation of precursor T-cell acute lymphoblastic leukemia is presented. Subsequent to treatment with three cycles of the etoposide and cytosine arabinoside (EA) and one cycle of the EA+L-asp chemotherapy regimens, the patient achieved complete remission. A series of consolidation therapy courses were performed subsequently. At present

Induction of cell surface interleukin 2 receptor alpha chain expression on non-T lymphoid leukemia cells.

We examined nine cases of adult non-T lymphoid leukemia to investigate the cell surface inducibility of interleukin 2 receptor alpha chain (IL-2R alpha) and beta chain (IL-2R beta) after in vitro culture with and without recombinant human interleukin-1 beta (rhIL-1 beta). Induction of IL-2R alpha was observed in four of six cases with precursor B-cell acute lymphoblastic leukemia (pre-B ALL) and in all of three cases with B-cell mature lymphoid neoplasm (two chronic lymphocytic leukemia and one leukemic phase of non-Hodgkin's lymphoma). All of the IL-2R alpha-inducible cases could express this

Multiplex PCR for TCR delta rearrangements: a rapid and specific approach for the detection and identification of immature and mature rearrangements in ALL.

The preferential occurrence of immature T-cell receptor (TCR) delta rearrangements (i.e. incomplete Ddelta2-Ddelta3 and Vdelta2-Ddelta3) in B-cell precursor acute lymphoblastic leukaemia (BCP ALL) and of predominantly mature rearrangements (incomplete Ddelta2-Jdelta1, complete Vdelta1, Vdelta2, Vdelta3 to Jdelta1) in T-lineage ALL prompted us to establish two separate multiplex PCR systems for the identification of clonal TCRdelta rearrangements. PCR products of the expected size for the specific rearrangements were detectable from a dilution of 100-1000 clonal cells in 150000 polyclonal cells

[Recent advances in individualized treatment for pediatric high-risk B-cell acute lymphoblastic leukemia].

B-cell acute lymphoblastic leukemia (B-ALL) is the most common hematologic malignancy in children. High-risk B-ALL, due to factors such as poor early treatment response and adverse genetic features, is prone to drug resistance and relapse, resulting in an unfavorable prognosis and posing a major clinical challenge. In recent years, risk stratification based on molecular subtyping has driven optimization of therapeutic strategies, and precision, individualized treatment has become the focus of clinical research and practice. This review summarizes the research progress in individualized therapy

Overt and covert genetic causes of pediatric acute lymphoblastic leukemia.

Pediatric acute lymphoblastic leukemia (pALL) is the most common childhood malignancy, yet its etiology remains incompletely understood. However, over the course of three waves of germline genetic research, several non-environmental causes have been identified. Beginning with trisomy 21, seven overt cancer predisposition syndromes (CPSs)-characterized by broad clinical phenotypes that include an elevated risk of pALL-were first described. More recently, newly described CPSs conferring high risk of pALL are increasingly covert, with six exhibiting only minimal or no non-cancer features. These 1

The Meaning and Value of Palliative Rehabilitation Through a Case Report Analysis.

PURPOSE: Palliative rehabilitation is defined as the process of helping individuals with a progressive, often advanced or incurable disease reach their physical, psychological, and social potential consistent with physiologic and environmental limitations and life preferences. However, the evidence on this topic is very heterogeneous in the medical scientific literature and studies diverge in their results depending on both the pathologies treated and the outcome measures studied as well as the types of interventions brought to the patients. Additionally, little is known about the role of the

Eradicating measurable residual disease via immunotherapy to cure acute lymphoblastic leukemia: current status and future directions.

INTRODUCTION: Eradicating measurable residual disease (MRD) may represent the final hurdle in curing ALL. The introduction of immunotherapies alone or in combination with other agents to therapies, including induction therapy, consolidation therapy, maintenance therapy, preemptive therapy and salvage therapy, was superior to chemotherapy alone in eradicating MRD during the course of treatment, leading to improved survival. AREAS COVERED: We provide a concise overview of the techniques used for MRD detection. We focus on the application of immunotherapies for MRD eradication at different treat
