Priapism is a recognized side effect of antipsychotic therapy. Recently, new agents known as atypical antipsychotics, such as clozapine, have been introduced with the intent of ameliorating psychosis without the extrapyramidal side effects associated with standard antipsychotic therapy. Priapism has not been observed previously with atypical antipsychotic therapy. We report a case of veno-occlusive priapism associated with the use of clozapine. This priapistic episode was complicated by the development of recurrent post-ischemic priapism.

Cocaine abuse has escalated to epidemic proportions in the United States. In addition to the social and economic problems associated with the recreational use of this agent, a variety of medical complications have been reported. The occurrence of priapism with intranasal use has not been documented previously. We report a case of priapism secondary to intranasal cocaine abuse and describe the possible neurochemical etiology for this phenomenon.

A case of segmental priapism in a 24-year-old man with congenital spherocytosis is presented. Preoperative cavernosography and computer tomography clearly revealed thrombosis of the left proximal corpus cavernosum. Because the lesion had already been in existence for 5 weeks irrigation to flush out the thrombosis was no longer possible.

A case of spontaneous intermittent priapism in a patient with spinal canal stenosis is presented. Erection, as well as intermittent neurogenic claudication and urinary incontinence were provoked by physical exertion, mainly walking. The symptoms completely resolved after decompressive lumbar laminectomy.

Priapism can be successfully treated by unilateral or bilateral percutaneous transcatheter occlusion of the internal pudendal arteries. Occlusion should be reversible in order to avoid impotence. Embolization with an autologous clot satisfies this requirement because of clot lysis and consequent vessel recanalization. This treatment is the most specific therapy in 'high-flow' priapism. The authors describe extensively 5 cases which demonstrate the feasibility of the method by which sexual potency is preserved.

Idiopathic priapism is a diagnosis made by exclusion of identifiable etiologies and is found in 43 to 58 per cent of the cases. We report a case of priapism that was investigated exhaustively and classified as idiopathic. In addition, 3 brothers also reported similar episodes of priapism. Although a common abnormality exists presumably, we were unable to define an etiology. Therefore, this case was designated as familial idiopathic priapism.

Fourteen patients with priapism have been treated over the last 12 years. Two patients had a recurrence 2 and 5 years after the primary incident. Abuse of alcohol (often combined with psychotropic drugs) and the use of antihypertensive drugs were the most prominent aetiological factors. Three out of 9 patients treated conservatively and 5 out of 6 patients treated with a shunt procedure have preserved their erection ability. Our results indicate that conservative treatment is not justified and that a shunt procedure should be performed with the least possible delay.

For the surgical treatment of priapism we have created a percutaneous fistula between the glans penis and the corpus cavernosum with good results. This is our preferred method of management for all patients except those undergoing sickle cell crisis.

The aetiopathogenesis and clinical picture of 3 cases of priapism are described, along with the treatment employed. The first patient suffered from thalassaemia minor, whereas in the second case priapism was a manifestation of aneurysm of the abdominal aorta perforated in the cava. In the last case, erection was a sequela of surgery for hydrocoele performed under peridural anaesthesia.
