The authors report the case of a 67-year-old woman with metastatic renal cell carcinoma (RCC) without prior nephrectomy, who had received long-term exposure (38 months) to the oral multi-targeted tyrosine kinase inhibitor (TKI), sunitinib. She had a sustained clinical and radiological response to her therapy, but had this therapy discontinued due to the rare development of nephrotic syndrome.

The use of implants for the growing child is not routinely recommended. The concerns about placing implants for patients in this age group are related to jaw growth. However, not all children with missing teeth need to wait for growth to be completed prior to implant placement. In this paper, the authors will discuss the indications for implant placement in the growing child. The decision for implant placement is based not only on growth, but also the number and location of the missing teeth.

Bladder tamponade is an uncommon clinical symptom among men suffering during sexual intercourse. The authors report on a 46-year-old man for whom this symptom appeared 4-5 years prior to hospitalization. Angiography confirmed the presence of left pudendal and obturator arterial bleeding, and embolotherapy of the internal pudendal and obturator arteries was performed. There was no mortality, or limb loss or sexual potency loss at follow-up.

The authors discuss the role of intravenous anesthetic agents in brain protection. The newer intravenous anesthetics, etomidate and propofol, have been proposed as neuroprotective agents. Thiopental remains the drug of choice, however, for use prior to intraoperative ischemic events. The anesthetic ketamine presents surprising similarities to other N-methyl-D-aspartate receptor inhibitors, but remains controversial in its use in neurologically compromised patients.

The authors were investigating the histochemical and electron microscopic characteristics of thyreocytes in pinealectomized rats treated with melatonin, prior to irradiation. They observed that the animals that did not receive melatonin show a higher degree of the destruction appearing at the level of all ultrastructural organelles and a very low expression of the DNA and RNA histochemical reaction. These results suggest the role of melatonin in the determination of the thyreocyte behaviour under given irradiation conditions.

Four patients undergoing cervical spinal manipulations for nonneurologic diseases and with no previous neurologic signs or symptoms all developed significant neurologic deficits, one fatal, following manipulations of the cervical spine. Both the literature and the authors' series show that a number of patients have a prodrome prior to the onset of neurologic changes. There is no established therapy for the syndrome. Perhaps prevention is the best means of reducing neurologic injury.

Clinico-anatomical examination of 547 patients with chronic opisthorchiasis has revealed the dislocation of intrahepatic biliary ducts onto the gallbladder wall and into the prolonged left triangular ligament of the liver. The authors believe that in order to prevent biliary peritonitis after operation on the organs of the upper part of the abdominal cavity, the ducts should not be injured during the operation, they should be ligated prior to cholecystectomy and section of the left triangular ligament.

Twelve cryptorchid patients had sonographic evaluation of the inguinal region prior to surgical orchiopexy. When the testis was within the inguinal canal, the gonad was localized preoperatively by sonography in eight of nine patients. One patient had an associated inguinal hernia in which loops of bowel concealed the gonad. There were no false positive cases. The authors were not able to show abdominal or pelvic maldescended testes.

Cranial chronic subdural hematoma (CSH) occurs secondary to trauma or just spontaneously. In this clinical report, the authors presented a 39-year-old male patient who was admitted with cranial CSH. Results of treatment of four patients are described. All of the patients had pervasive developmental disorder (PDD), a tic disorder, and a characteristic pattern of speech and language impairment. The patients were treated with haloperidol for their tic disorders, and concomitant with the reduction of the frequency and severity of tics was marked improvement in language. Acquired hyperthyroidism is most commonly autoimmune in etiology. In the setting of allogeneic bone marrow transplantation (BMT), the use of radiotherapy (total body irradiation) as part of the regimen prior to BMT is known to cause endocrine dysfunction, especially hypopituitarism and hypothyroidism, but hyperthyroidism is rare. The authors report this unusual and late complication in a young boy after BMT for relapsed childhood lymphoblastic leukemia and discuss the possible etiologies.
