De Clérambault's Syndrome (Erotomania) and claims of psychiatric malpractice.

De Clérambault's Syndrome or Erotomania was originally described as a delusional disorder in which a woman believes that an older man of higher social status is passionately in love with her. The patient's relentless pursuit of the delusional love object, often with escalating intrusiveness, may eventually involve threats or overt acts of retaliation, in response to repeated rejection, unrequited love, or alleged betrayal. Cases from the literature are reviewed in which the delusional romantic attachment involves the patient's psychiatrist or another medical specialist. The authors present a c

Certainty and uncertainty in genetic counselling: a qualitative case study.

This study examines the communication that occurs during routine genetic counselling sessions. It involves the qualitative analysis of nine consultations with one member of a clinical genetics team. The consultations were characterised by a contrast between the sense of certainty apparent in how the clinicians talked about the power and promise of the new genetics and the uncertainty communicated regarding the actual information produced by genetic tests and their limitations in solving people's problems. There was also a contrast between how the clinician seemed to control the format and agen

Therapist and patient factors in the behavioural treatment of phobic patients.

Thirty-five phobic patients were assessed in order to investigate the specific therapist and patient factors operative within treatment. Although the factors were highly correlated, they did distinguish between those patients who benefited differentially from treatment.

Parent-led conferences as sites of medical work.

Conferences are novel sites for understanding medical work. Through describing styles of presentation that take place at conferences attended by patients and parents, this article highlights how clinicians on stage present ordinary and extraordinary aspects of medicine. Attention is drawn to the reaction of the parents in the audience. The power of the presenter to direct proceedings highlights the potential vulnerability of the audience. The relationship between clinician on stage and parents in the audience reflects the clinical relationship between doctor and patient. But through identifyin

Changing drug users' risk environments: peer health advocates as multi-level community change agents.

Peer delivered, social oriented HIV prevention intervention designs are increasingly popular for addressing broader contexts of health risk beyond a focus on individual factors. Such interventions have the potential to affect multiple social levels of risk and change, including at the individual, network, and community levels, and reflect social ecological principles of interaction across social levels over time. The iterative and feedback dynamic generated by this multi-level effect increases the likelihood for sustained health improvement initiated by those trained to deliver the peer interv

Treating perpetrators of child physical abuse: a review of interventions.

This systematic review analyzes eleven empirically based articles relevant to evaluating treatments and interventions for perpetrators of child physical abuse. The articles provide a clear picture of the limited research available in this area. The review describes current themes in research and interventions, common challenges in working with families in which child physical abuse has occurred, gaps in the current research, and suggestions for practice, policy, and future research.

Violent Behavior among hospitalized medical and surgical patients.

To characterize violent behavior in hospitalized medical and surgical patients, we reviewed documented violent incidents at the San Francisco General Hospital during a two-year period. Twenty-nine incidents of verbal and physical violence occurred. One patient was gravely ill and three were delirious. All the incidents were associated with increased levels of tension and loss of impulse control. In most cases, contention with the staff regarding pain medication or ward regulations was a precipitating event. Of the 28 patients with mental disorders, 19 were substance abusers, six had organic br

[International and Israeli physicians' health--information and action plan].

Physician health is a matter of interest for patients' physicians and their teams, managers and policy-makers. It has an impact on public health, physician impairment, patient safety, resource allocation and malpractice litigation. International medical literature, unlike Israel publications, is extensively preoccupied with the domain. Based on 2 MD thesis dissertations, Ministry of Health data and a literature search, the present review addresses many issues. It deals with: physicians' physical and mental health internationally and in Israel, prevention and health promotion, burn-out, the pro

Nurse-to-Nurse Boundaries: Transgressions, Grey Areas and Professionalism.

Nursing teams depend on trust, respect, and clear communication in their interactions, yet these ideals are continually negotiated within hierarchical structures, time pressures and competing demands. While much attention is given to nurse-patient boundaries, far less scrutiny is directed towards boundaries among nurses. The boundaries we maintain with colleagues are often taken for granted rather than openly discussed, even though they significantly shape the workplace culture we encounter each day and can reproduce subtle (and even overt) forms of exclusion, favouritism and harm. The consequ

"It's nothing personal": understanding barriers to relational harm reduction practices during inpatient hospitalization.

BACKGROUND: People who use drugs (PWUD) have a higher incidence of infectious complications that require medical interventions only available in hospital settings. Hospitalizations for PWUD are also more likely to result in patient directed discharge (PDD) before completing medical treatment. Stigma directed at PWUD pressurizes and shapes clinical encounters, leading to poor communication between patient and provider. We explore how the relational philosophy of harm reduction - the premise that building relationships rooted in compassion and respect, as opposed to a transactional encounter - c

The Productivity of Wh- Prompts in Child Forensic Interviews.

Child witnesses are often asked wh- prompts (what, how, why, who, when, where) in forensic interviews. However, little research has examined the ways in which children respond to different wh- prompts, and no previous research has investigated productivity differences among wh- prompts in investigative interviews. This study examined the use and productivity of wh- prompts in 95 transcripts of 4- to 13-year-olds alleging sexual abuse in child investigative interviews. What-how questions about actions elicited the most productive responses during both the rapport building and substantive phases

A promising approach for emergency departments to care for patients with substance use and behavioral disorders.

Millions of patients visit US emergency departments (EDs) each year because of substance use and behavioral disorders. Caring for these people is daunting, given the high patient volumes and increasing acuity of illness that EDs are experiencing. The nation's primary care system has limited capacity to treat these individuals, who are often uninsured, poor, and sick. A growing body of evidence suggests that screening, providing a brief intervention, and referring these patients to treatment-an approach known as SBIRT-can be effective in the ED. Typically requiring just five to ten minutes, SBI

Treatment of primary vaginismus: a new perspective.

This paper challenges the efficacy of a cognitive-behavioral treatment model for women with primary vaginismus and proposes a conceptual shift from a focus on behavior to a focus on differentiation. Primary vaginismus is viewed as a somatic boundary, a symbolic description of an opportunity for differentiation. Four relevant themes are considered: 1) mastery versus incompetence; 2) autonomy versus dependence; 3) boundary versus fusion; and 4) the effect on the therapy of the therapist's level of differentiation. Change at this particular time in history involves shifts in clinical focus from s

Reconceptualizing case management in theory and practice: a frontline perspective.

'Case management' in the health and social service sector is controversial, highly political and saturated with conflicting agendas among stakeholders. Research was conducted recently to explore the role of case managers in three evolving, Australian-based, long-term care, case management models. The findings have relevance to countries worldwide that contend with delivering quality care that is cost effective. The research captures the perspectives of 51 open-ended interviews with front-line case managers who have first-hand knowledge of the models' effect on their clients and on their own ro

Exploring young patients’ perspectives on rehabilitation care: methods and challenges of organizing focus groups for children and adolescents.

BACKGROUND: In research on quality of care, the experiences of children and (pre)adolescents are usually assessed by asking their parents. However, these young patients may have preferences of their own, and their experiences do not necessarily concur with those of their parents. Therefore, our research aimed to give young patients an opportunity to speak up for themselves. METHODS: Focus group meetings and online focus groups were organized for two age groups (8–11 and 12–15 years) of young patients from two Dutch rehabilitation centres, with the use of a tailored interview technique. The fe
