A Comparative Review of Mandibular Orthognathic Surgeries with a Focus on Intraoral Vertico-sagittal Ramus Osteotomy.

Severe dentofacial deformities require both orthodontics and surgical management to repair. Modern mandibular orthognathic surgery commonly … ats preoperative temporomandibular joint symptoms. This osteotomy type is especially applicable for prognathism with excessive flaring of the ramus and with temporomandibular joint dysfunction. The 'L-shaped' IVSRO or Choung's type I osteotomy is indicated for patients with condylar hyperplasia and high condylar process fractures.

A newborn with acanthosis nigricans: can it be Crouzon syndrome with acanthosis nigricans?

Crouzon syndrome is a craniosynostosis syndrome, characterized by cloverleaf skull, hypertelorism, exophthalmos, external strabismus, parrot-beaked nose, short upper lip, hypoplastic maxilla, and mandibular prognathism. The 5% of individuals with Crouzon syndrome who have pigmentary changes in the skin are said to have Crouzon syndrome with acanthosis nigricans (CAN). Choanal atresia, hydrocephalus and the cranial features of Crouzon syndrome should suggest the diagnosis of CAN even before acanthosis appears. We present a 10-hour-old newborn who presented with bilateral choanal atresia, cranio…

Bilateral Scissor Bite Treated by Rapid Mandibular Expansion Following Corticotomy.

Scissor bite is defined as buccal displacement of the maxillary posterior teeth, with or without contact between the lingual surface of the maxillary lingual cusp and the buccal surface of the buccal cusp of its mandibular antagonist. Here we report treatment of bilateral scissor bite in the posterior region using rapid expansion following corticot-omy. The patient was a boy aged 17 years and 11 months in whom skeletal maxillary prognathism with bilateral scissor bite was diagnosed. The distance between the tips of canines and the mesial buccal cusps of the first molars was increased by use of…

Dentomaxillofacial imaging in Proteus syndrome.

Proteus syndrome is a rare condition that involves atypical growth of the bones, skin and head and a variety of other symptoms. Only a few authors have reported on the craniofacial manifestations so far. The authors present a case of a 7-year-old girl with Proteus syndrome in which the facial skeleton showed unilateral overgrowth. The analysis of the radiological evaluation revealed a bialveolar prognathism, a skeletal class III, a dolicocephalic growth pattern and a left convex face scoliosis. On the left side, the lesser wing of the sphenoid was elevated and the ethmoidal cell complex was hy…

GAPO syndrome: a new case of this rare syndrome and a review of the relative importance of different phenotypic features in diagnosis.

The case of GAPO syndrome reported here is the 24th recorded case, 23 cases having been published previously. The 29-year-old male under discussion presents all the typical features of the syndrome, having short stature, dysmorphic craniofacial features, total alopecia and pseudoanodontia. Orally, the erupted primary dentition was extremely worn and on radiographic examination, the second mandibular molars were found to be unerupted, together with the entire permanent dentition. Cephalometry revealed the absence of facial pneumatisation, a deficient cranial base with diminished upper face heig

Clinical profile of Angelman syndrome at different ages.

We describe 47 patients with Angelman syndrome (AS) from Belgium and the Netherlands, including the anamnestic data, the clinical and the behavioral attributes at different ages. The clinical picture of AS is most distinct between the ages of 2-16 years. Most patients of this age group show at least 8 of the major characteristics (bursts of laughter, happy disposition, hyperactive behaviour, microcephaly, brachycephaly, macrostomia, tongue protrusion, mandibular prognathism, widely spaced teeth, stiff and puppetlike movements, typical stature, wide based gait) beside the mental retardation and…

Rabson-Mendenhall Syndrome Nearly Misdiagnosed as Type 1 Diabetes Mellitus: A Case Report.

Rabson-Mendenhall syndrome (RMS) is a rare genetic condition marked by severe insulin resistance, leading to persistent hyperglycemia that can sometimes be misdiagnosed as type 1 diabetes mellitus (T1DM). This case report details a 34-year-old male who was referred to a tertiary center for genetic evaluation to rule out insulin resistance syndrome. The patient had been diagnosed with T1DM since childhood, struggling to control his hyperglycemia despite high doses of insulin. Physical examination revealed acanthosis nigricans, prognathism, and other dysmorphic features. Genetic testing identifi…

Unilateral condylar hyperplasia treated with simultaneous 2-jaw orthognathic surgery and posterior segmental osteotomy.

A 25-year-old woman presented with left condylar hyperplasia, canting-type facial asymmetry, mandibular prognathism, and arch width discrepancy. Bone scintigraphy confirmed the inactive status of the condyle, and the temporomandibular joint functioned within the normal range; thus, orthognathic surgery without condylectomy was performed. To correct facial asymmetry successfully through orthognathic surgery, sufficient dentoalveolar decompensation must be achieved in the presurgical orthodontic phase. In cases of canting-type facial asymmetry, teeth on the nondeviated side are extruded as dento…

Dental patient with acromegaly: a case report.

Acromegaly is an acquired disorder related to excessive production of growth hormone after epiphyseal closure of bones. It is characterized by progressive somatic disfigurement (mainly involving the face and extremities) and systemic manifestations. The prevalence is estimated at 1:140,000-250,000. Although acromegaly rarely manifests in the orofacial regions, it must be considered in patients who develop malocclusion after adolescence. Here, we present a case of acromegaly with brief review of the literature.

Anesthetic management of a patient with Bartter's syndrome undergoing orthognathic surgery.

Bartter's syndrome is a rare disorder characterized by severe hypokalemic alkalosis, marked elevation in plasma renin activity, pressor insensitivity to angiotensin II, and normal or low values of plasma sodium, plasma chloride, and blood pressure. Many of the clinical features and biochemical abnormalities appear to be secondary to chronic hypokalemia. We managed a 22-yr-old man requiring orthognathic surgery for correction of facial asymmetry under general anesthesia.

[Functional disturbances of the inferior alveolar nerve following sagittal osteotomy of the ramus mandibulae: preventive surgical technic].

Permanent functional disturbances of the inferior alveolar nerve after sagittal osteotomy of the ramus of the mandible form an increasingly common feature of the reported series. Reference is made to earlier experimental research in a description of a preliminary series in which this complication was avoided by cleaving the two cortical substances with a thin cement spatula used as a scalpel. The results were extremely encouraging. There were no reports of permanent functional disturbances of the alveolar nerve, while the incidence of temporary defects was also reduced.
