Nutritional support of the patient with cancer.

The role of nutritional support in the management of the critically ill patient has been a topic of much concern and research. Malnutrition experienced by patients with cancer can be related to the nutritional status of the patient before the development of cancer, to the tumor itself, and to cancer therapy. Aggressive treatment may increase the degree of malnutrition, and the combination of the effects of therapy and progressive malnutrition may be a frequent cause of death. The use of intensive nutritional support for some patients may promote weight gain and positive nitrogen balance, incre

Progressive Dilatation as a Successful Treatment for Y Duplication of Urethra: A case report.

Duplication of urethra is a rare congenital anomaly; a Y-shaped urethral duplication is the rarest variant and lacks a standardised treatment plan. We report a nine-year-old male patient with a Y-duplication of urethra diagnosed during neonatal age who presented to a tertiary care teaching hospital in India in 2018. The patient had undergone a vesicostomy on the seventh day of life for passing urine per anus and was lost to follow-up thereafter. At eight years of age, there was a failed attempt at disconnection of the duplicated urethral tract from the anus after colostomy. The patient was man

Case report: atrial septostomy as a bridge to lung transplantation in a patient with venovenous extracorporeal membrane oxygenation.

INTRODUCTION: Advances in critical care management have led to the recent increase in the use of extracorporeal membrane oxygenation (ECMO) as a bridge to lung transplantation (LT). Patients with respiratory failure requiring venovenous ECMO usually experience progressive right ventricular (RV) failure. Diagnosis and treatment of RV failure during ECMO are essential for improving the prognosis of patients. PATIENT CONCERNS: A 28-year-old female patient underwent allogeneic hematopoietic stem cell transplantation (HSCT) from a matched unrelated donor for acute myeloid leukemia presenting with

Clinical spectrum of mitochondrial DNA depletion due to mutations in the thymidine kinase 2 gene.

BACKGROUND: Mitochondrial DNA depletion syndrome is an autosomal recessive disorder characterized by decreased mitochondrial DNA copy numbers in affected tissues. It has been linked to 4 genes involved in deoxyribonucleotide triphosphate metabolism: thymidine kinase 2 (TK2), deoxyguanosine kinase (DGUOK), polymerase gamma (POLG), and SUCLA2, the gene encoding the beta-subunit of the adenosine diphosphate-forming succinyl coenzyme A synthetase ligase. OBJECTIVE: To highlight the variability in the clinical spectrum of TK2-related mitochondrial DNA depletion syndrome. DESIGN: Review of patient

Impact of Multidisciplinary Team-Based Nursing on Pulmonary and Psychological Outcomes in Acute Exacerbation of Chronic Obstructive Pulmonary Disease.

BACKGROUND We aimed to comprehensively assess the association of multidisciplinary team (MDT)-based stepped nursing intervention with dyspnea index, pulmonary function, psychological outcomes, quality of life, and nursing satisfaction in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). MATERIAL AND METHODS Participants (n=150) were retrospectively recruited from patients with AECOPD treated from April 2022 to December 2024 and assigned into an MDT group (n=76) and routine group (n=74) based on nursing intervention modes. Intergroup and intragroup comparisons

Idiopathic Pulmonary Fibrosis: Best Practice in Monitoring and Managing a Relentless Fibrotic Disease.

Idiopathic pulmonary fibrosis (IPF) is a fibrosing interstitial lung disease that is, by definition, progressive. Progression of IPF is reflected by a decline in lung function, worsening of dyspnea and exercise capacity, and deterioration in health-related quality of life. In the short term, the course of disease for an individual patient is impossible to predict. A period of relative stability in forced vital capacity (FVC) does not mean that FVC will remain stable in the near future. Frequent monitoring using multiple assessments, not limited to pulmonary function tests, is important to eval

Utility of Repeat Head Computed Tomography for Intracranial Hemorrhage After Trauma and Importance of Direct Patient Care.

At many institutions, it is common practice for trauma patients with traumatic intracranial hemorrhage (ICH) to receive routine repeat head computed tomographic (CT) scans after the initial CT scan, regardless of symptoms, to evaluate progression of the injury. The purpose of this study was to assess quantifiable risk factors (age, anticoagulation, gender) that could place patients at greater risk for progression of injury, thus requiring surgical intervention (craniotomy, craniectomy) for which serial CT scanning would be useful. From January 2014 to June 2015, a total of 211 patients present

Intractable Neck Pain in an Oncologic Palliative Care Setting: Is Cancer Always the Answer?

Refractory pain is a common manifestation in an oncologic palliative care setting and represents a major challenge for health care professionals involved in care provision. The underlying neoplasm and its dissemination are the foremost pathophysiologic mechanism for the development of pain in patients with advanced cancer. Nonetheless, other etiologies such as trauma and infections need to be considered by clinicians in this particular care setting. The authors present the case of a patient with a recent diagnosis of hepatocellular carcinoma, suffering from intractable neck pain, progressive w

Advanced multiple sclerosis: an exploratory study on a neglected patient population.

BACKGROUND: Multiple sclerosis (MS) is a chronic immune-mediated disease that can cause severe physical and cognitive disability. While modern therapies have improved outcomes in relapsing MS, patients with advanced disease remain underserved. In this stage, neurodegeneration dominates, treatment options are limited, and care becomes complex. Yet individuals with advanced MS are largely absent from trials, registries, and structured care pathways, leaving a major evidence gap. OBJECTIVES: To characterize the clinical, social, and treatment-related profile of patients with advanced MS in insti

Case Report: Family-mediated prognostic non-disclosure and psychological deterioration in terminal cancer.

Prognostic non-disclosure is a common practice in Indian palliative care, often driven by family members' intentions to protect patients from emotional distress. However, the psychological consequences of sustained non-disclosure remain insufficiently documented within psycho-oncology literature. This case report, structured according to the CARE guidelines, describes the psychological trajectory of a 34-year-old male police officer with metastatic gastric carcinoma whose terminal prognosis was withheld at his family's request. Initially, the patient remained hopeful following a total gastrect

When Omental Infarction Imitates Appendicitis: Advancing Diagnosis with POCUS.

A previously healthy 12-year-old boy presented to the paediatric emergency department on three occasions in the space of four days with progressive right iliac fossa pain. His presentation appeared consistent with appendicitis; however, blood tests remained normal at each presentation. Point of care ultrasound (POCUS) revealed significant inflammatory changes in the right lower quadrant along with free fluid. Due to worsening pain and advancing findings on POCUS, the patient underwent a diagnostic laparoscopy which identified a normal appendix but had features consistent with omental infarctio

Management of pulmonary arterial hypertension.

Pulmonary hypertension (PH) is common and may result from a number of disorders, including left heart disease, lung disease, and chronic thromboembolic disease. Pulmonary arterial hypertension (PAH) is an uncommon disease characterized by progressive remodeling of the distal pulmonary arteries, resulting in elevated pulmonary vascular resistance and, eventually, in right ventricular failure. Over the past decades, knowledge of the basic pathobiology of PAH and its natural history, prognostic indicators, and therapeutic options has exploded. A thorough evaluation of a patient is critical to cor

[The support of relatives of the demented elderly].

Relatives of patients suffering from senile dementia have to deal with multiple changes and difficulties. Emotional problems as well as the continuing care for the elderly demented patient may be a cause of progressive social isolation. The co-operation with professional helpers is often a source of considerable conflicts. These conflicts may still aggravate once the patient has been admitted into a specialized psychogeriatric ward. Conceptually some reactions of the relatives may be interpreted as manifestations of a mourning-process, where not only real losses play an important part, but als

Rhinophyma: A Report of Two Patients Managed With Electrosurgery.

Rhinophyma is a chronic cutaneous condition secondary to rosacea that primarily affects the nose, leading to a bulbous and enlarged appearance. We report two cases of rhinophyma in 65- and 76-year-old men with long-standing rosacea evolving for more than 10 years. Both patients presented with progressive nasal enlargement, erythema, and nodular hypertrophy that had worsened over the previous five to seven years, leading to significant cosmetic and psychosocial impairment. The diagnosis was clinical. Surgical treatment was performed in both cases, including electrosurgical excision in one patie

Toxic Epidermal Necrolysis Secondary to Post-surgical Analgesia for Posterior Vaginal Repair.

A 26-year-old woman with a history of epilepsy and drug hypersensitivity developed toxic epidermal necrolysis (TEN) following a posterior vaginal repair and levatoroplasty. She experienced fever, general malaise, and rapidly progressing blistering dermatosis, with areas of denuded epidermis and oral mucosal involvement. A dermatological assessment confirmed that her skin involvement exceeded 30%, leading to a clinical diagnosis of TEN. She was transferred to the intensive care unit, where treatment included discontinuation of the suspected drug, intravenous immunoglobulin, systemic corticoster

Clinical insights in enzyme replacement therapy for metabolic storage disorders: lessons from Pompe disease.

Metabolic storage disorders, including lysosomal storage disorders, pose complex challenges in management due to their progressive and life-threatening nature. Although enzyme replacement therapy has substantially improved outcomes for patients with lysosomal storage disorders, limitations of this therapy have become apparent throughout two decades of use. New clinical features of these diseases have emerged as patients live longer, leading to unresolved questions regarding ongoing treatment and long-term care. Innovative therapies are emerging that aim to improve targeting of tissues, particu

Health care resource utilization and costs across stages of amyotrophic lateral sclerosis in the United States.

BACKGROUND: People living with ALS (plwALS) experience motor control loss, speech/swallowing difficulties, respiratory insufficiency, and early death. Advancing disease stage is typically associated with a greater burden on the health care system, and delays in diagnosis can result in substantial health care resource utilization (HCRU). OBJECTIVE: To estimate HCRU and cost burden of plwALS across disease stages from a US payer perspective we assessed HCRU and costs in early-, mid-, and late-stage ALS. METHODS: Using insurance claims data from the IBM MarketScan Databases between January 2013

Impact of a Dedicated Noninvasive Ventilation Team on Intubation and Mortality Rates in Severe COPD Exacerbations.

BACKGROUND: Compared with usual care, noninvasive ventilation (NIV) lowers the risk of intubation and death for subjects with respiratory failure secondary to COPD exacerbations, but whether administration of NIV by a specialized, dedicated team improves its efficiency remains uncertain. Our aim was to test whether a dedicated team of respiratory therapists applying all acute NIV treatments would reduce the risk of intubation or death for subjects with COPD admitted for respiratory failure. METHODS: We carried out a retrospective study comparing subjects with COPD admitted to the ICU before (

Patient pathways for rare diseases in Europe: ataxia as an example.

BACKGROUND: Progressive ataxias are rare and complex neurological disorders that represent a challenge for the clinicians to diagnose and manage them. This study explored the patient pathways of individuals attending specialist ataxia centres (SAC) compared with non-specialist settings. We investigated specifically how diagnosis was reached, the access to healthcare services, treatments, and care satisfaction. The focus of this study was on early intervention, coordination of treatment to understand the care provision in different countries. METHODS: A patient survey was done in the UK, Germa
