What is the lower limb length discrepancy after arthroplasty for proximal femoral fracture? A prospective, multicenter observational study of 590 hips.

INTRODUCTION: Lower limb length discrepancy (LLD) following hip arthroplasty after proximal femoral fracture (PFFA) is little studied. The aim of this work was to answer the following questions: 1) What are the incidence and mean values of LLD after PFFA? 2) What are the clinical consequences (tolerance) of LLD after PFFA? 3) Can we identify risk factors for LLD after PFFA? 4) Is there a significant difference in terms of LLD after PFFA to treat intra- versus extra-capsular fractures? HYPOTHESIS: LLD after proximal femoral fracture arthroplasty is rare but has good clinical tolerance, given

Silent iatrogenic pseudoaneurysm after intertrochanteric fracture fixation with proximal femoral nailing and cerclage wiring: case report and review of literature.

Pseudoaneurysm is a rare complication after intertrochanteric fracture fixation. Herein, we present a rare case of late development of a pseudoaneurysm with silent clinical symptoms. The case was a 91-year-old woman treated with proximal femoral nailing and cerclage wiring. Postoperatively, the patient was able to ambulate with a walker without abnormal symptoms. During the follow-ups, the radiographic images showed progressive cortical scalloping on the medial femoral shaft. Ultrasonography revealed a yin-yang sign, and a CT scan confirmed a pseudoaneurysm at the profunda femoris artery (PFA)

Validation and algorithmic audit of a deep learning system for the detection of proximal femoral fractures in patients in the emergency department: a diagnostic accuracy study.

BACKGROUND: Proximal femoral fractures are an important clinical and public health issue associated with substantial morbidity and early mortality. Artificial intelligence might offer improved diagnostic accuracy for these fractures, but typical approaches to testing of artificial intelligence models can underestimate the risks of artificial intelligence-based diagnostic systems. METHODS: We present a preclinical evaluation of a deep learning model intended to detect proximal femoral fractures in frontal x-ray films in emergency department patients, trained on films from the Royal Adelaide Ho…

Three-year experience with risedronate therapy for patients with an increased fracture risk: assessment of proximal femoral bone density and geometry by DXA.

BACKGROUND AND OBJECTIVE: We previously reported that risedronate improved the structural parameters of the proximal femur, as well as lumbar spine and proximal femoral bone mineral density (BMD), after 1 year of therapy by suppressing bone resorption in patients with an increased risk of fracture. Our practice-based observational study was subsequently extended to determine whether these effects were enhanced, maintained or attenuated after 3 years. METHODS: A total of 174 patients (nine men and 165 postmenopausal women) with a mean age of 67.8 years who had osteoporosis or osteopenia and cl

Functional Outcomes in Failed Osteosynthesis of Proximal Femoral Fractures Managed by Total Hip Replacement Using the Modified Hardinge Approach: An Ambispective Cohort Study.

Introduction Proximal femoral fractures are a major cause of disability, particularly in aging populations, with an increasing incidence. Although osteosynthesis remains the first-line treatment, failures are common due to various complications. Total hip arthroplasty (THA) is the preferred salvage procedure in such cases, despite its technical challenges. This study aims to evaluate functional outcomes following THA in patients with failed proximal femoral fracture fixation. Materials and methods This ambispective cohort study was conducted from 2022 to 2023 at King George's Medical Universit…

Comparative study of a novel proximal femoral bionic nail and three conventional cephalomedullary nails for reverse obliquity intertrochanteric fractures: a finite element analysis.

PURPOSE: Conventional cephalomedullary nails (CMNs) are commonly employed for internal fixation in the treatment of reverse obliquity intertrochanteric (ROI) fractures. However, the limited effectiveness of conventional CMNs in addressing ROI fractures results in significant implant-related complications. To address challenges associated with internal fixation, a novel Proximal Femoral Bionic Nail (PFBN) has been developed. METHODS: In this study, a finite element model was constructed using a normal femoral specimen, and biomechanical verification was conducted using the GOM non-contact opti

Novel anchor-type proximal femoral nail for the improvement of bone-fixation integrity in treating intertrochanteric fractures: an experimental and computational characterization study.

This study introduces a novel anchor-type proximal femoral nail (AT-PFN) to improve the bone-fixation integrity over the standard screw-type nail (SST-PFN). Quasi-static incremental cyclic load test was performed to investigate load-displacement, cumulative deformation energy, time-strain, and backbone curves. The finite element analysis (FEA) was implemented to identify the stress and strain distributions. Additionally, non-destructive dynamic tests were conducted, and the measurements were processed using statistical pattern recognition, based on vector autoregression and principal component

Proximal femoral nailing for intertrochanteric fracture combined with contralateral femoral neck local osteo-enhancement procedure (LOEP) for severe osteoporotic bone loss: An original Italian case series.

Proximal femoral fractures in elderly women are a major cause of morbidity and mortality worldwide and a public health concern. Although pharmacological therapies have shown potential in improving bone mineral density (BMD) and decreasing fracture risk, the current research effort is focused on developing a procedure that can ensure both immediate and long-term efficacy. A minimally-invasive surgical approach, known as AGN1 local osteo-enhancement procedure (LOEP), has been recently developed to promote bone augmentation. The procedure implies the preparation of an enhancement site, a specific…
