McCune Albright Syndrome is characterized by the triad of polyostostic fibrous dysplasia of bones, skin pigmentation (café au lait-spots) and sexual precocity. The authors report a case of a 7 years girl with this syndrome in whom the sexual precocity is due to an autonomous hyperfunctioning of luteinized follicular cysts of ovary. The pathogenic mechanisms of this autonomy are debate.

The authors relate the observation of a girl with precocious puberty diagnosed at 4 years, treated for 7 years with medroxyprogesterone and who presents now a Stein-Leventhal syndrome. They emphasize the unique character of this association and discuss the different pathogenetic hypotheses: hypothalamo-pituitary disorder, effect of medroxyprogesterone acetate, hyperinsulinism, casual association.

A study of 98 cases of true sexual precocity in girls shows a frequency three times higher between 5 and 7 years than before age 5. Detectable causes (intracranial lesion and Mac Cune-Albright syndrome) were found in 56% of cases before 5 years and only 18% after age 5. The plasma levels of estradiol and gonadotropins and the gonadotropins response to LHRH did not differ significantly from those found at the corresponding stages of normal puberty. Examinations of psychic and intellectual development were made in 9 girls with idiopathic precocious puberty.
