Prospective study of the psychiatric disorders of childbirth by self report questionnaire.

Two self report questionnaires, the Anxiety and Depression scales of Bedford and Foulds (SAD), and 10 visual analogue scales were each administered on several occasions during pregnancy and the puerperium to a representative sample of 425 childbearing women. Those women who each completed the analogue scales during pregnancy at 12 and 23 weeks and at 1 week and 5 months after childbirth (N = 230) showed no association between depression or anxiety and either the gravidity or method of delivery on any of the four occasions. Depression, anxiety and tearfulness on the visual analogue scales incre

[Clinical pathological analysis of peripartum maternal mortality (author's transl)].

All maternal deaths which occurred in relation to labour at the Department of Obstetrics and Gynaecology, University Hospital, Graz, between 1963 and 1978 were reviewed and analysed. 24 mothers died over this 15-year period. The maternal death rate was 0.34 per thousand. In all cases a post mortem examination was performed. The youngest woman was 19, the oldest 42 years old. Most of the decreased mothers were primiparae. The main cause of maternal death was post-partum haemorrhage, followed by infection of the uterus especially after Caesarean section. Three mothers died of liver distrophy. Th

Factors associated with postpartum complications: A systematic review to inform early detection strategies.

BACKGROUND: Complications during the puerperium present abnormal conditions that can jeopardize the health and well-being of both mother and newborn. These complications are significant contributors to maternal morbidity and mortality worldwide. This systematic review aims to identify and synthesize evidence on the factors associated with postpartum disease complications. METHODS: This systematic review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The protocol was registered in PROSPERO (CRD420251013692). A compre

A Rare Case of Postpartum-acquired Hemophilia A Presenting with Deep Vein Thrombosis.

Acquired hemophilia A (AHA) is a rare autoimmune disorder (1.5 per million) caused by autoantibodies against factor VIII, leading to coagulopathy. Postpartum AHA is uncommon, accounting for only 1-5% of cases, and typically presents with mucocutaneous or vaginal bleeding, while deep vein thrombosis (DVT) is an extremely rare manifestation. We report a case of a 33-year-old postpartum female presenting with left lower limb DVT and spontaneous bleeding. Initial management with heparin and direct oral anticoagulants (DOACs) exacerbated bleeding, prompting treatment revision. Persistently elevated

A case with rare complication of chronic puerperal uterine inversion and underwent operation reduction.

OBJECTIVE: Puerperal uterine inversion is a rare and severe complication and is associated with short cord, uncontrolled cord traction, placenta accreta, or uterine atony. CASE REPORT: A primigravida woman gave birth a 2770 gm newborn at term at our hospital, and clinically presented postpartum hemorrhage, hypovolemic shock, postpartum preeclampsia and urinary retention. She discharged 3 days postpartum, but she complained persist vaginal bleeding and lower abdominal pain for more than 1 month. Uterine inversion was diagnosed and laparoscope surgery for reduction was done. CONCLUSION: The no

Genetics of Peripartum Cardiomyopathy: Current Knowledge, Future Directions and Clinical Implications.

Peripartum cardiomyopathy (PPCM) is a condition in which heart failure and systolic dysfunction occur late in pregnancy or within months following delivery. Over the last decade, genetic advances in heritable cardiomyopathy have provided new insights into the role of genetics in PPCM. In this review, we summarise current knowledge of the genetics of PPCM and potential avenues for further research, including the role of molecular chaperone mutations in PPCM. Evidence supporting a genetic basis for PPCM has emanated from observations of familial disease, overlap with familial dilated cardiomyopa

Assessing the future risks of subsequent pregnancies in peripartum cardiomyopathy.

Peripartum cardiomyopathy is a myocardial disease process which occurs in young women either in late pregnancy or the early postpartum period. Due to the young age of women effected by this disease, many of these patients elect to pursue a subsequent pregnancy after their initial diagnosis. Currently, echocardiography is used to better elucidate the cardiovascular risks these young patients face when undergoing a subsequent pregnancy; however, the most accurate modality to determine these risks is debatable. In this review, we explore the current literature regarding the use and accuracy of re

Perinatal anxiety and depressive symptoms and perception of child behavior and temperament in early motherhood.

The perinatal period is a vulnerable time for the development of psychopathology, particularly mood and anxiety disorders. In the study of maternal anxiety, important questions remain regarding the association between maternal anxiety symptoms and subsequent child outcomes. This study examined the association between depressive and anxiety symptoms, namely social anxiety, panic, and agoraphobia disorder symptoms during the perinatal period and maternal perception of child behavior, specifically different facets of development and temperament. Participants (N = 104) were recruited during pregna

Abducens Nerve Palsy In Gestational Hypertension: A Case Report and Review of the Literature.

BACKGROUND: Headache, blurring of vision, and confusion are common neurologic symptoms of hypertensive disorders of pregnancy, whereas abducens nerve palsy during pregnancy is an extremely rare condition. CASE: A 28-year-old woman with gestational hypertension presented isolated sixth cranial nerve palsy after delivery. Neither simple resonance nor angioresonance showed alterations. No other specific pathology was found. Symptoms of abducens nerve palsy resolved spontaneously. CONCLUSION: Abducens nerve palsy is an unusual condition. We present a review of the literature finding eight docume

Evidence for the use of ceftiofur for treatment of metritis in dairy cattle.

Metritis is a cause of postparturient uterine disease in dairy cattle and is most commonly associated with watery fetid red-brown uterine discharge occurring in the first 21 days postpartum. The most severe form of metritis (puerperal metritis) often warrants antibiotic therapy. This article analyzes the current literature to determine the efficacy of ceftiofur in the treatment of metritis. Evidence-based review of the current literature suggests that there is evidence for the use of ceftiofur in the treatment of metritis. However, review of the literature also reveals the need for more studie

Gout in pregnancy: a case report and review of the literature.

While gout is a common inflammatory joint disease, its occurrence in women in their reproductive years is very rare. This is thought to be the result of the uricosuric effect of estrogen. The higher estrogen levels during pregnancy are believed to protect the mother against an acute gout flare. We report a case of a patient with gout who experienced a flare in the third trimester of her pregnancy and a review of the English literature on gout in pregnancy. In addition to this case, we identified 19 pregnancies in 8 women with a diagnosis of gout. Of those, 6 experienced an antepartum flare and

Peripartum cardiomyopathy: current understanding, comprehensive management review and new developments.

Peripartum cardiomyopathy (PPCM) is a rare and potentially fatal disease which presents with symptoms of heart failure primarily due to left ventricular (LV) systolic dysfunction in the last month of pregnancy and up to 5-6 months after delivery. PPCM is still regarded as a disease of unknown aetiology, although recent evidence suggests a role for a 16 kDa prolactin derivative produced by proteolytic cleavage of prolactin secondary to unbalanced oxidative stress present during late pregnancy and early puerperium. The medical management of PPCM is similar to other forms of non-ischaemic dilated

Role of dynamic MRI in surgical decision-making for a postpartum woman with a prolapsed degenerating uterine leiomyoma.

BACKGROUND: Surgical decision-making in the case of postpartum complications affecting fibroids can be difficult. We present a case that illustrates the potential role of MRI in assisting these decisions. CASE: A 28-year-old woman with a known uterine leiomyoma presented on postpartum day 10 with abdominal pain, fever, and foul-smelling vaginal discharge. Her condition improved after antibiotic therapy, but she subsequently returned to hospital with a liquefied fibroid filling the vagina and a solid mass palpable above the dilated cervix. Pelvic ultrasound did not conclusively identify the va
