Effect of truncal vagotomy on parietal cell mass and antral gastrin cell mass in dogs.

Vagotomy is known to reduce acid secretion and to increase serum gastrin concentrations. However, there is minimal information on the effect of vagotomy on parietal cell mass or gastrin cell mass. Basal and maximal acid secretions and fasting serum gastrin concentrations were measured in 22 gastric fistula dogs with pyloromyotomy before and up to 56 days following complete bilateral truncal vagotomy (n = 11) or sham vagotomy (n = 11). Dogs underwent total gastrectomy on postoperative days 9 (n = 5 per group) or day 56 (n = 6 per group). Parietal cells were stained with Luxol fast blue and pari…

Impact of myotomy length on per-oral endoscopic myotomy (POEM) outcomes for achalasia: a meta-analysis of randomized trials.

INTRODUCTION: Peroral endoscopic myotomy (POEM) has been established as a safe and effective intervention for Type I and II achalasia. Studies have shown that short POEM is non-inferior to long POEM, with some evidence of decreased incidence of gastroesophageal reflux disease (GERD). This meta-analysis aims to systematically review and analyze randomized controlled trials (RCTs) data comparing clinical and safety outcomes for long and short POEM procedures while further investigating differences in GERD and other long-term clinical outcomes. METHODS: We conducted a comprehensive search in Pub

Pyloric drainage interventions for gastroparesis: a comparison of laparoscopic pyloroplasty and gastric peroral endoscopic myotomy (G-POEM) outcomes.

BACKGROUND: The endoscopic approach to the surgical management of gastroparesis is gaining popularity. However, data comparing endoscopic myotomy to traditional laparoscopic pyloroplasty are limited. This study aimed to compare outcomes between gastric-per-oral endoscopic myotomy (G-POEM) and pyloroplasty. METHODS: Gastroparesis patients who underwent pyloroplasty or G-POEM from 2013 through 2023 at our institution were reviewed. Pre/postoperative gastroparesis cardinal symptom index (GCSI), resolution of the predominant gastroparesis symptom, and gastric emptying scintigraphy (GES) were asse

Endoscopic and Surgical Treatments for Gastroparesis: What to Do and Whom to Treat?

Gastroparesis is a complex chronic debilitating condition of gastric motility resulting in the delayed gastric emptying and multiple severe symptoms, which may lead to malnutrition and dehydration. Initial management of patients with gastroparesis focuses on the diet, lifestyle modification and medical therapy. Various endoscopic and surgical interventions are reserved for refractory cases of gastroparesis, not responding to conservative therapy. Pyloric interventions, enteral access tubes, gastric electrical stimulator and gastrectomy have been described in the care of patients with gastropar

Delayed extubation and hypertrophic pyloric stenosis: what are the predictive factors?

INTRODUCTION: Prolonged time to extubation after general anesthesia has been defined as a delay of more than 15 min from the end of surgery to tracheal extubation. This incident is frequently seen in infants operated on for hypertrophic pyloric stenosis (HPS), which can lead to inefficient use of operating rooms and delayed care for other patients. AIM: To evaluate the frequency of "delayed extubations" in infants who have received an extramucosal pyloromyotomy under general anesthesia and to identify the predictive factors of this incident. METHODS: We report a retrospective and comparative…

Peroral endoscopic myotomy (POEM) as a treatment for pediatric achalasia: multicenter study and first results.

INTRODUCTION: Laparoscopic Heller miotomy of achalasia has been classically recognized as the gold standard management in children. There is increasing experience with the peroral endoscopic miotomy (POEM) approach in pediatrics, although the series published are scarce. The objective of this study was to present our experience in primary or secondary treatment of pediatric achalasia by POEM and its clinical success rate. METHODS: We performed a retrospective review of pediatric patients with achalasia who underwent POEM in some national centers from October 2016 to January 2023. We evaluated

Peroral endoscopic myotomy provides effective palliation in type III achalasia.

BACKGROUND: Type III achalasia outcomes have historically been met with limited success after conventional laparoscopic Heller myotomy (LHM) and pneumatic dilation. Peroral endoscopic myotomy (POEM) has emerged as a promising alterative for a multitude of reasons. Our objective was to investigate POEM outcomes in palliating type III achalasia. METHODS: A retrospective analysis of a prospectively maintained database was conducted in a tertiary care institution between April 2014 and July 2019. The primary outcome was postoperative Eckardt score. We also explored the effect of lower esophageal

Gastroparesis.

Gastroparesis is characterized by symptoms suggestive of, and objective evidence of, delayed gastric emptying in the absence of mechanical o … ncluding prokinetics and anti-emetics; and interventions such as gastric electrical stimulation and pyloromyotomy. These are discussed briefly, and comment is added on the potential for individualized treatments in the future, based on optimal gastric emptying measurement and objective documentation of the underlying pathophysiology causing the gastroparesis.

Risk factors for unsuccessful atropine therapy in hypertrophic pyloric stenosis.

BACKGROUND: I.v. atropine (IA) for infantile hypertrophic pyloric stenosis (IHPS) is a good alternative to pyloromyotomy but has not been broadly accepted. The lower success rate is one of the greatest disadvantages of IA. We investigated the risk factors for unsuccessful results following IA for IHPS. METHODS: Medical records of patients with IHPS who were admitted to Kimitsu Chuo Hospital between 2002 and 2016 and were initially given atropine sulfate were retrospectively reviewed. Atropine was given i.v. (0.1 mg/kg/day in eight divided doses). Oral feeding of milk was started with a small …

Ergonomic laparoscopic spreader for infantile hypertrophic pyloric stenosis.

BACKGROUND: Laparoscopic pyloromyotomy (LP) for Infantile Hypertrophic Pyloric Stenosis (IHPS) is associated with an increased risk of mucosal tears or incomplete myotomy when compared with open myotomy. Keeping these risks in mind, a new ergonomically designed spreader for LP was developed and investigated in IHPS. METHODS: The new Pyloric Spreader was designed as a 3.5 mm instrument specifically for minimal access application taking into consideration the specific needs of LP. RESULTS: LP was performed using this spreader in 25 infants with IHPS during a 5-year period from 2006-2012. After…
