Validity and reliability of the measurement of the quardriceps femoris muscle strength with a hand-held dynamometer on the affected side in hemiplegic patients.

The purpose of this study was to investigate validity and reliability of measuring the quadriceps femoris muscle strength on the affected side in hemiplegic patients with a hand-held dynamometer(HHD) to prepare for a multi-center cooperative study. Measurements of the known weights(8.972 and 18.665 kg) with HHD were 8.98 +/- 0.00 kg and 18.57 +/- 0.00 kg, respectively, and measurements with HHD were almost consistent with those with KIN-COM (gold standard), because the Pearson's product-moment correlation coefficient was 0.99 (P < 0.001). There were significant differences in measurements of t

Quantitative gait analysis in patients with medial patellar instability following lateral retinacular release.

Medial patellar instability is a known possible complication following lateral retinacular release. Insufficient passive structures, muscle … t quadriceps mechanism and a centred patella was seen during loading (weight acceptance), while the quadriceps muscle was active. Abnormal medial translation of the patella was observed during unloading of the leg while the knee was bending in preparation for the swing phase. This is a phase in the gait cycle when the quadriceps muscle is silent and the patella pos …

Asymmetric calf hypertrophy of neurogenic origin.

A 47-year-old male presented with painful swelling of the right calf. His medical history was negative, except for a herniation of disc LIV-V 5 years before. Physical examination revealed unilateral calf hypertrophy with moderate weakness of plantarflexion, mild paresis of dorsiflexion. Electromyography showed a peripheral neurogenic lesion in the right anterior tibial muscle, but normal findings were obtained from the unaffected quadriceps muscle. Histological examination of the right gastrocnemic muscle showed neurogenic changes with typical targetoid fibers, but no pathological changes were…

Investigations of ultrastructure of damaged and regenerated skeletal muscle fibers.

Our investigations concerned the head of the parietal part of quadriceps femoris, and we based our investigation on observations of the ultrastructure of muscle fibers using an electron microscope. We observed tissue samples taken from patients (10 men) 25-35 years old, who had old damage of knee joint ligament (after about 6 week's immobilization). In the first group, segments of tissue of parietal head of quadriceps femoris were taken inter-operationally from patients in whom there was found old damage of knee joint ligament. The second group was of tissue segments of this muscle after surgi

Effect of quadriceps training at different levels of blood flow restriction on quadriceps strength and thickness in the mid-term postoperative period after anterior cruciate ligament reconstruction: a randomized controlled external pilot study.

BACKGROUND: More than 2 million anterior cruciate ligament (ACL) injuries occur worldwide each year. Most surgeons suggest that athletes and active persons with significant knee functional demands, including cutting motions, require and should be offered ligament reconstruction surgery. Despite concentrated rehabilitation efforts, deficits in quadriceps size and strength can persist for years after surgery. Blood flow restriction (BFR) training can help overcome disuse muscular atrophy in the mid-term postoperative period after anterior cruciate ligament reconstruction (ACLR) surgery. The purp

Acute botulinum toxin-induced muscle weakness in the anterior cruciate ligament-deficient rabbit.

We established botulinum type-A toxin (BTX-A) injections as a powerful tool to cause knee extensor weakness in New Zealand White (NZW) rabbits. The purpose of this study was to determine if BTX-A induced quadriceps weakness causes muscle dysfunction beyond that caused by anterior cruciate ligament (ACL) transection in the knee of NZW rabbits. Twenty animals were randomly divided into four study groups (n=5 each); uninjected controls, BTX-A injection alone, ACL transection alone, BTX-A injection and ACL transection combined. Isometric knee extensor torque, quadriceps muscle mass, and vertical a…

Hamstring/quadriceps ratios in college football players: a high velocity evaluation.

Hamstring/quadriceps ratios were assessed on 60 intercollegiate football players at functional speeds of 90, 180, and 300 deg/sec on the Cybex II. The ratio rose as velocity of exercise increased and the quadriceps was stronger than the hamstrings at all speeds. The flexor/extensor ratio differed bilaterally at all speeds with ratio lower for the dominant knee. The study helped establish normative data on hamstring/quadricep ratios on football players at functional speeds. Furthermore, the ratio provides a guide to observe muscle imbalances which could be a factor in injury.

[Lewis-Sumner syndrome presenting unilateral quadriceps amyotrophy as an initial symptom].

We report a 55-year-old man with a chief complaint of wasting and weakness of the left quadriceps muscle. At age 54, he noticed difficulty in running and weakness in the left thigh, which gradually progressed. On the first admission to our hospital, based on the nerve conduction studies (NCS), the muscle biopsy findings showing neurologenic changes, and no abnormality of spinal MRI, we diagnosed as unilateral quadriceps amyotrophy, which resulted from an atypical form of spinal progressive muscular atrophy. One year later, he showed the bilateral hand weakness, conduction blocks on the right m…

Identification of ultrasound predictors and development of a diagnostic scoring system for sarcopenia using LASSO regression: a cross-sectional study.

Ultrasound-based muscle assessment has emerged as a practical and noninvasive tool for evaluating sarcopenia; however, the relative diagnostic value of individual ultrasound-derived muscle parameters and their integration into a simple scoring system remain unclear. This cross-sectional study included 145 community-dwelling women aged 65-80 years. Eight ultrasound-derived quadriceps muscle parameters were evaluated: rectus femoris thickness, cross-sectional area, and echo intensity; vastus intermedius thickness; vastus lateralis thickness, fascicle length, and pennation angle; and quadriceps f…

Can real-time visual feedback during gait retraining reduce metabolic demand for individuals with transtibial amputation?

The metabolic demand of walking generally increases following lower extremity amputation. This study used real-time visual feedback to modif … rt rate increased with center-of-mass feedback. Metabolic rate, center of mass sway, and integrated quadriceps muscle activity were all not significantly different from controls. Attempts to modify gait to decrease metabolic demand may actually adversely increase the physiological effort of walking in individuals with lower extremity amputation who are young, activ …

Effects of Essential Amino Acid Supplementation on Clinical Outcomes in the Conservative Management of Knee Osteoarthritis.

BACKGROUND: The effect of supplementation of essential amino acids (EAAs) in knee osteoarthritis (OA) remains unclear. This study aimed to evaluate whether supplementation with EAA improves pain, patient-reported outcome measures, gait function, and quadriceps muscle volume in patients undergoing conservative treatment for knee OA. METHODS: A retrospective cohort study was conducted on outpatients undergoing physical therapy from April 2024 to March 2025. Inclusion criteria were patients who started physical therapy for knee OA, with exclusion of patients taking other supplements or having se…

Activating the somatosensory system enhances net quadriceps moment during gait.

Quadriceps muscle rehabilitation following knee injury or disease is often hampered by pain, proprioception deficits or instability associated with inhibition of quadriceps activation during walking. The cross-modal plasticity of the somatosensory system with common sensory pathways including pain, pressure and vibration offers a novel opportunity to enhance quadriceps function during walking. This study explores the effectiveness of an active knee brace that used intermittent cutaneous vibration during walking to enhance the peak knee flexion moment (KFM) during early stance phase as a surrog…

p38 mitogen-activated protein kinase is not activated in the quadriceps of patients with stable chronic obstructive pulmonary disease.

Quadriceps weakness is associated with a poor prognosis in COPD. Several data suggest that immobility and muscle wasting may be related through up-regulation of the p38 Mitogen associated protein kinase (MAPK) signalling pathway. We therefore hypothesised that this might occur in the quadriceps of COPD patients. We studied 105 stable COPD outpatients (mean FEV(1) 43.9% predicted) and 27 age and gender matched controls. We measured fat-free mass, quadriceps strength and daily physical activity using triaxial accelerometry. A quadriceps biopsy was obtained in which components of the p38-MAPK sig

Dysfunction of endogenous pain inhibition during exercise with painful muscles in patients with shoulder myalgia and fibromyalgia.

The aim of this study was to investigate how exercise influenced endogenous pain modulation in healthy controls, shoulder myalgia patients and fibromyalgia (FM) patients. Twenty-one healthy subjects, 20 shoulder myalgia patients and 20 FM patients, all females, participated. They performed standardized static contractions, that is, outward shoulder rotation (m. infraspinatus) and knee extension (m. quadriceps). Pressure pain thresholds (PPTs) were determined bilaterally at m. infraspinatus and m. quadriceps. During contractions PPTs were assessed at the contracting muscle, the resting homologo
