Solitary Rectal Ulcer Syndrome Is Not Always Ulcerated: A Case Report.

Solitary rectal ulcer syndrome (SRUS) is a benign and chronic disorder well known in young adults that is characterized by a series of symptoms such as rectal bleeding, copious mucus discharge, prolonged excessive straining, perineal and abdominal pain, a feeling of incomplete defecation, constipation and, rarely, rectal prolapse. The etiology of this syndrome remains obscure, and the diagnosis is easily confused with that of other diseases, contributing to difficulties in treatment. We present a case of a 37-year-old male with a nonulcerated rectal lesion grossly resembling a superficial depr

Non-operative management in low-lying rectal cancers undergoing chemoradiation.

PURPOSE: To evaluate the outcomes of post-neoadjuvant chemoradiation (NACTRT) wait-and-watch Strategy (WWS) in distal rectal cancers. MATERIALS AND METHODS: All consecutive patients from December 2012 to 2019 diagnosed with distal rectal tumors (T2-T4 N0-N+) having a complete or near-complete response (cCR or nCR, respectively) post-NACTRT and wishing for the non-surgical treatment option of WWS were included in this study. Patients were observed with 3 monthly magnetic resonance imaging (MRIs), sigmoidoscopies, and digital rectal examination for 2 years and 6 monthly thereafter. Organ preser

Unsuspected carcinoma found histologically after minor anorectal operations.

The unsatisfactory results of radical operations in advanced anal cancer could be bettered by earlier diagnosis of the malignant lesion. The possibility of cancer should always be kept in mind when treating any of the common chronic anal diseases. Preliminary biopsy studies should be done more frequently if suspicious lesions are present and all tissues removed during minor anorectal operations should always be examined by a pathologist. Each surgical specimen should be labeled to show the site from which it was removed. Follow-up care of the patient would be simplified if the exact site of ca

Mucinous adenocarcinoma in anorectal fistulae.

Mucinous adenocarcinoma arising in an anorectal fistula is an uncommon condition which gives rise to difficult problems of diagnosis and pathogenesis. The clinical history and pathology of seven patients are described and compared with six patients in whom anal fistulae were lined by normal rectal mucosa or 'misplaced glands'. In five of the cases granulomas were present which were a further cause of diagnostic difficulty. The evidence from this study suggests that the fistulous tracks are congenital duplications of the lower end of the hind gut lined by rectal mucosa which is prone to maligna

A case of rectosigmoid cancer metastasizing to a fistula in ano.

We herein report a case of rectosigmoid cancer metastasizing to a fistula in ano. A 53-year-old man complaining of anal bleeding consulted another hospital. He had been suffering from an anal fistula since 7 years. On the left upper side of the skin surface around the anus a fistula end was seen as a hole that tunneled down into the back passage, although no hard tumor was palpable on the hole. Complete colonoscopy revealed an ulcerative tumor in the rectosigmoid colon. On 5 February 2004, anterior resection and lymphadenectomy was performed. The post-operative pathological diagnosis was recto

Extended Total Mesorectal Excision Based on the Avascular Planes of the Retroperitoneum for Locally Advanced Rectal Cancer with Lateral Pelvic Sidewall Invasion.

INTRODUCTION: It has been considered difficult to achieve en bloc resection in cases of locally advanced rectal cancer with lateral pelvic sidewall invasion. The present study demonstrates a novel surgical procedure for these tumors. TECHNIQUE: There are 3 avascular planes of the retroperitoneum in the pelvic sidewall. Two visceral pelvic fasciae, namely the ureterohypogastric fascia and umbilical prevesical fascia, and the parietal pelvic fascia can be identified. In addition, the key structures of these fasciae, the ureter, umbilical artery, and external iliac vessels, can be identified tra

[Reoperation in rectal cancer (author's transl)].

Carcinoma of the rectum is reported to have a local recurrence rate of 3-48 percent. In 80 percent of cases, recurrence appears within 2 years after the first operation. The possible causes of its occurrence and its localization are discussed. Early diagnosis is important. Follow-up examinations are therefore necessary every 3 months in the first 2 years. Curative secondary operations are rare. In pelvic, perineal, and genital recurrence, radiation therapy is usually considered to be the treatment of choice.
