[Enteroviruses serotypes isolated in years 2008-2009 in NIPH-NIH].

Enteroviruses are important etiologic agents of many human diseases such as diarrhea, self-limiting gastroenteritis, respiratory infections, conjunctivitis, hepatitis, aseptic meningitis, encephalitis, and paralysis. The aim of this study was the evaluation of the frequency of enteroviral infections in Poland in 2008-2009. Out of 178 clinical materials tested for the presence of enteroviruses, 24 samples (13,5%) were positive. In the case of 153 samples from patients suffering from acute flaccid paralysis (AFP), positive results were obtained for 6 samples (4%). Moreover, 25 samples coming fro

Diaphragmatic paralysis caused by malposition of chest tube placement after pediatric cardiac surgery.

Diaphragmatic paralysis is a recognized complication after pediatric cardiac surgery. It is universally acknowledged that direct phrenic nerve injury during surgery is the etiology. However, we experienced two unusual cases of diaphragmatic paralysis following malposition of chest tube placement after pediatric cardiac surgery. The malposition of too deeply placed chest tube with resultant phrenic nerve injury was presumably the underlying cause. One patient underwent successful diaphragmatic plication due to intractable respiratory distress. The other was asymptomatic. Our report highlights t

[Phrenic nerve paralysis of obstetrical origin: favorable course using continuous positive airway pressure].

INTRODUCTION: Isolated diaphragmatic paralysis due to obstetrical factors is rare and therapeutic management modalities are not quite clear. CASE REPORT: A neonate born by breech delivery presented with respiratory distress due to isolated paralysis of the right hemidiaphragm. The clinical course was progressive, his condition worsening with oxygen supplementation. Continuous positive airway pressure (CPAP) delivered via a nasal cannula was started in the one-month-old child, inducing gradual improvement towards recovery at the age of two months and a half. CONCLUSION: Non-invasive nasal CPA

Ventilatory control in ALS.

Amyotrophic lateral sclerosis (ALS) is a fatal, progressive neurodegenerative disease. ALS selectively causes degeneration in upper and lower (spinal) motor neurons, leading to muscle weakness, paralysis and death by ventilatory failure. Although ventilatory failure is generally the cause of death in ALS, little is known concerning the impact of this disorder on respiratory motor neurons, the consequences of respiratory motor neuron cell death, or the ability of the respiratory control system to "fight back" via mechanisms of compensatory respiratory plasticity. Here we review known effects of

Bilateral phrenic neuropathy as a presenting feature of multifocal motor neuropathy with conduction block.

Diaphragmatic paralysis has previously been reported as a result of diverse pathologic processes involving the peripheral nervous system. We report the clinical history, physical findings, and antibody profile of an atypical case of multifocal motor neuropathy with conduction block initially presenting with respiratory failure secondary to bilateral phrenic neuropathy.

Spontaneous rupture of an aneurysm of the superior thyroid artery: A rare cause of acute respiratory failure.

Ruptured aneurysm of superior thyroid artery is a very rare pathological event. Underlying causes such as trauma, malignancy or iatrogenic are not systematically found. Resulting cervical hematoma is life threatening and can lead to acute respiratory failure, dysphagia, vocal cord paralysis and hemomediastinum. Endovascular treatment combined with surgical drainage has been described as an effective treatment. In this case report, a 63-year-old man presented a spontaneous rupture of an aneurysm of superior thyroid artery resulting in cervical hematoma and acute respiratory failure.

Mutations in MEGF10, a regulator of satellite cell myogenesis, cause early onset myopathy, areflexia, respiratory distress and dysphagia (EMARDD).

Infantile myopathies with diaphragmatic paralysis are genetically heterogeneous, and clinical symptoms do not assist in differentiating between them. We used phased haplotype analysis with subsequent targeted exome sequencing to identify MEGF10 mutations in a previously unidentified type of infantile myopathy with diaphragmatic weakness, areflexia, respiratory distress and dysphagia. MEGF10 is highly expressed in activated satellite cells and regulates their proliferation as well as their differentiation and fusion into multinucleated myofibers, which are greatly reduced in muscle from individ

Diaphragmatic paralysis post COVID-19 treated with robot-assisted plication reinforced with acellular dermal matrix: a report of two cases.

Coronavirus disease 2019 (COVID-19) continues to be a disease of global importance, with an increasing array of sequelae attributed to infection by the severe acute respiratory syndrome coronavirus-2. One such complication that has been rarely documented thus far is diaphragmatic dysfunction. Here, we report the cases of 2 individuals who developed diaphragmatic paralysis post COVID-19, which failed to respond to conservative management. Both patients proceeded to undergo robot-assisted thoracoscopic plication of the diaphragm reinforced with a bovine acellular dermal matrix. In both cases, th

Cobras.

Human encounters with cobras in Africa and Asia are common with substantial mortality and primarily respiratory paralysis in untreated cases. First aid includes maintaining calm, placing a lymphatic retardant on the bitten extremity, and transportation to a medical facility. Medical management is antivenin. Physiologic monitoring and the ability to manage the compromised airway are important.

Anesthetic management for cesarean delivery in a Guillain-Barré syndrome patient -A case report-.

Guillain-Barré syndrome is an acute inflammatory demyelinating polyradiculopathy characterized by progressive motor weakness, areflexia, and ascending paralysis. Guillain-Barré syndrome is extremely rare in pregnant patients, and there are no established guidelines for delivery or safest anesthetic methods. We report a Cesarean delivery in the case of a 32-year old woman who was diagnosed with Guillain-Barré syndrome 18 weeks into gestation. Tracheostomy was performed due to progressive respiratory muscle weakness and respiratory failure, and ventilator support was required in the intensive ca

Vocal cord paralysis following lithium button battery ingestion in children.

This study reports on vocal cord paralysis caused by esophageal button battery (BB) ingestion in children. Medical records of children with vocal cord paralysis and esophageal BB ingestion treated at a tertiary referral institute between January 2016 and March 2020 were reviewed. Five patients aged 9-58 months were identified; three were male. Each patient had accidentally swallowed a 20-mm-diameter lithium battery (3 CR2032 type and 2 CR2025 type). One battery was removed within 4 h after ingestion, and three batteries were removed within 12 h. Removal of the battery was achieved with rigid e

Noninvasive respiratory management of high level spinal cord injury.

This article describes noninvasive acute and long-term management of the respiratory muscle paralysis of high spinal cord injury (SCI). This includes full-setting, continuous ventilatory support by noninvasive intermittent positive pressure ventilation (NIV) to support inspiratory muscles and mechanically assisted coughing (MAC) to support inspiratory and expiratory muscles. The NIV and MAC can also be used to extubate or decannulate 'unweanable' patients with SCI, to prevent intercurrent respiratory tract infections from developing into pneumonia and acute respiratory failure (ARF), and to el

Failure of observation and impedance respirography to detect active expiration in ventilated preterm infants.

Twenty preterm infants ventilated for the respiratory distress syndrome were studied on 44 occasions to identify the pattern of interaction between their spontaneous respiratory efforts and the ventilator, using three techniques: (1) an oesophageal balloon and pneumotachograph, (2) impedance respirography and (3) clinical scoring. The information was in agreement on 23 of the occasions. In 7 infants the balloon and pneumotachograph showed active expiration which was not detected by the other methods. Four of these infants sustained a pneumothorax, one after paralysis with pancuronium. This stu

Thyrotoxic Periodic Paralysis With Severe Hypokalemia Precipitated by Acute Alcohol Intoxication in a Patient With Graves' Disease.

We present a case of a 29-year-old male who presented with thyrotoxic periodic paralysis (TPP) precipitated by acute alcohol intoxication. TPP is an endocrine emergency that presents with an episode of acute flaccid paralysis with hypokalemia in the setting of thyrotoxicosis. Individuals who present with TPP are thought to have an underlying genetic predisposition. Overactivation of the Na+/K+ ATPase channel leads to large-scale intracellular shifts in potassium, leading to low serum levels and the clinical manifestations of TPP. Severe hypokalemia can lead to life-threatening complications su

Fatal rabies despite post-exposure prophylaxis.

Only sporadic reports of failure of post-exposure prophylaxis for rabies exist in the published literature. We are reporting such a case in a 3-year-old boy. The child had Category III dog bite on his right thigh. He presented with progressive ascending paralysis, finally developing quadriplegia and respiratory paralysis. Typical hydrophobia and aerophobia were absent. He received four doses of antirabies cell culture vaccine. He did not receive antirabies immunoglobulin. The boy succumbed on the 23 rd day of the dog bite. Diagnosis of rabies was confirmed in the laboratory by demonstration of…

Vocal cord positioning by selective denervation. Old territory revisited.

Vocal cord positioning produced by selective laryngeal denervation remains a controversial issue in clinical laryngology. Previous studies fail to arrive at uniform conclusions for two important reasons: 1) failure to mark a reference sagittal plane from which to assess the degree of vocal cord lateralization, and 2) failure to recognize the influence of tracheostomy and respiratory positioning of the vocal cords. The present study makes use of photographic and electromyographic documentation in the assessment of the paralyzed cord. Physiologic inactivation of the cricothyroid muscle by trache

Acute Flaccid Myelitis: A Clinical Review.

Acute flaccid myelitis (AFM) is an emerging disorder primarily affecting children that is characterized by acute flaccid paralysis accompanied by abnormalities of the spinal cord gray matter on magnetic resonance imaging. In most cases, prodromal fever or respiratory symptoms occur, followed by acute-onset flaccid limb weakness. Respiratory, axial, bulbar, facial, and extraocular muscles may also be affected. The clinical manifestations have been described as "polio-like," due to striking similarities to cases of poliomyelitis. The primary site of injury in AFM is the anterior horn cells of th
