Analysis of emboli and blood flow in the ophthalmic artery to understand retinal artery occlusion.

Retinal artery occlusion (RAO) is a common ocular vascular occlusive disorder that may lead to partial or complete retinal ischemia with sudden visual deterioration and visual field defects. Although RAO has been investigated since 1859, the main mechanism is still not fully understood. While hypoperfusion of the ophthalmic artery (OA) due to severe stenosis of the internal carotid artery might lead to RAO, emboli are assumed to be the main reason. Intra-arterial thrombolysis is not a sufficient treatment for RAO, and current research is mainly focused on risk factors. In this study, a computa…

Combined occlusion of branch retinal artery and vein secondary to prepapillary arterial loops.

A middle-aged diabetic and hypertensive man presented with diminished vision in the left eye. Fundus examination revealed prepapillary arterial loops, but with features of venous rather than arterial occlusion. Fluorescein angiography and optical coherence tomography confirmed the presence of a branch retinal vein occlusion along with two branch retinal artery occlusions. The resultant macular edema responded well to intravitreal triamcinolone and laser photocoagulation though the visual improvement was moderate.

Coronary artery tortuosity: comparison with retinal arteries and carotid intima-media thickness.

BACKGROUND AND AIM: We conducted a prospective study to investigate the possible relationship between the tortuosity of coronary arteries (TCA) and carotid intima-media thickness (CIMT), and also compare TCA to retinal artery tortuosity. METHODS: One hundred and five participants with nonsignificant coronary plaque or normal coronary angiogram were included. To determine subclinical atherosclerosis, maximum CIMT was measured. Retinal tortuosity was evaluated ophthalmically. RESULTS: Among all demographic variables and risk factors, only female gender and height were significantly associated …

Intra-arterial thrombolysis for central retinal artery occlusion: two cases report.

Central retinal artery occlusion (CRAO) causes severe visual loss in affected eye and vision does not recover in more than 90% of the patients. It is believed that it occurs by occlusion of the central retinal artery with small emboli from atherosclerotic plaque of internal cerebral artery. Retina is a part of the brain, thus basically CRAO is corresponding to acute occlusion of intracerebral artery and retinal ischemia is to cerebral stroke. Therefore, intra-arterial thrombolysis (IAT) has been considered as a treatment method in CRAO. Recently, we treated 2 patients diagnosed as CRAO and cou…

[Retinal artery occlusion in Germany: results from the oregis registry].

BACKGROUND: The oregis registry is Germany's first nationwide platform for real-world ophthalmological data. Launched as a multicenter, hypothesis-free registry by the German Society of Ophthalmology (DOG), oregis continuously collects treatment data from outpatient practices and clinics. With currently more than 2.5 million patients, oregis offers an unprecedented database for ophthalmological health research. Despite its clinical relevance, epidemiological and real-world data on the treatment of retinal artery occlusion (RAO) remain limited and based on small cohorts. This leads to uncertain…

Incidence of central retinal artery occlusion peaks in winter season.

INTRODUCTION: Stroke incidence exhibits seasonal trends, with the highest occurrences observed during winter. This study investigates the incidence of central retinal artery occlusion (CRAO), a stroke equivalent of the retina, and explores its monthly and seasonal variations, as well as potential associations with weather and ambient air pollutants. METHODS: A retrospective search of medical records spanning 15 years (January 2008-December 2022) was conducted at the University Eye Hospital Tübingen, Germany, focusing on diagnosed cases of CRAO. Incidences were evaluated on a monthly and seaso…

Cilioretinal Sparing Central Retinal Artery Occlusion from Giant Cell Arteritis.

A 93-year-old woman presented with a 1-day history of sudden painless vision loss in her right eye with a background of new temporal headaches and jaw claudication. Dilated fundus examination of the right eye showed mild retinal whitening and edema throughout the right fundus with selective sparing in the distribution of the cilioretinal artery. A diagnosis of giant-cell arteritis (GCA) was made and confirmed with temporal artery biopsy. Oral prednisone was initiated and her vision improved to 20/200 in the right eye at 3-month follow-up. GCA-related cilioretinal artery sparing central retinal…

Differential artery-vein analysis in quantitative retinal imaging: a review.

Quantitative retinal imaging is essential for eye disease detection, staging classification, and treatment assessment. It is known that different eye diseases or severity stages can affect the artery and vein systems in different ways. Therefore, differential artery-vein (AV) analysis can improve the performance of quantitative retinal imaging. In this article, we provide a brief summary of technical rationales and clinical applications of differential AV analysis in fundus photography, optical coherence tomography (OCT), and OCT angiography (OCTA).

Simultaneous Bilateral Central Retinal Artery Occlusion following COVID-19 Infection.

Purpose: Coronavirus-19 disease (COVID-19) has been associated with a high risk of thrombotic complications. Here, we report the case of a patient who developed simultaneous bilateral retinal artery occlusion following COVID-19 infection.Case Report: A 42-year-old male with no systemic co-morbidities presented with sudden, painless loss of vision in both eyes. Fundoscopy showed retinal edema and cherry-red spots in both eyes. Fluorescein angiography showed reperfusion, absence of choroidal ischemia, and Optical Coherence Tomography showed thickened inner retinal layers suggestive of retinal ed…

Nephrotic Syndrome with Central Retinal Artery Occlusion: A Unique Presentation.

Childhood nephrotic syndrome is associated with significant morbidity because of recurrent relapses, infections, and episodes of thromboembolism. Thromboembolism in nephrotic syndrome may involve any major blood vessel. Timely recognition of symptoms and early initiation of anticoagulation therapy are important to avoid end-organ damage. We present here a case of a child with steroid-resistant nephrotic syndrome (SRNS) with bilateral central retinal artery occlusion (CRAO), whose vision improved with anticoagulation therapy.

Hemicentral Retinal Artery Occlusion: A Rare Complication of Orbital Cellulitis.

A 14-year-old boy presented with acute vision loss and swelling of the right eye. An anterior segment examination revealed a relative afferent pupillary defect, severe proptosis, and ophthalmoplegia in his right eye. The fundus examination revealed inferior hemicentral retinal artery occlusion. Although the authors proceeded with systemic medical treatment, the findings did not improve. An endoscopic orbital decompression was performed. With both medical and surgical treatment, the orbital cellulitis resolved and the patient's visual acuity improved. Assessing the risk of stroke development following retinal artery occlusion.

BACKGROUND: Retinal artery occlusion (RAO) can cause acute, painless, and irreversible loss of vision. Using a large sample population database, we investigated the possible impact of RAO and numerous other clinical and non-clinical factors on the risk of developing a subsequent cerebrovascular accident. METHODS: Cases of RAO were obtained from the National Inpatient Sample (NIS) database between 2002 and 2013 using ICD-9 codes. Associated morbidities and procedures were assessed in these cases with a primary hospital admitting diagnosis of stroke. Univariate and multivariate logistic regress…
