Myelodysplastic Syndromes: 2026 Update on Diagnosis, Risk-Stratification and Management.

DISEASE OVERVIEW: The myelodysplastic syndromes (MDS) are a heterogeneous group of myeloid disorders characterized by peripheral blood cytopenias and increased risk of transformation to acute myelogenous leukemia (AML). MDS occurs more frequently in older males and in individuals with prior exposure to cytotoxic therapy. DIAGNOSIS: Diagnosis of MDS is based on morphological evidence of dysplasia upon visual examination of a bone marrow aspirate and biopsy. Information obtained from additional studies such as karyotype, flow cytometry, and molecular genetics is usually complementary and may he

Type 1 Diabetes and Cardiovascular Risk: The Current Management Strategies.

Cardiovascular disease (CVD) is the most common cause of morbidity and excess mortality in patients with diabetes. However, CVD risk varies across the different forms of diabetes mellitus owing to underlying pathobiological mechanisms. In the asymptomatic phase of prediabetes and early stages of type 2 diabetes mellitus (T2DM)-the most common form of diabetes-CVD may already be established; meanwhile, the first diagnosis of T2DM in some patients may be established only when the patients are evaluated for risk factors contributing to CVD. In contrast, type 1 diabetes mellitus (T1DM) typically p

Plasmablastic Lymphoma: 2026 Update on Diagnosis, Risk Stratification, and Management.

DISEASE OVERVIEW: Plasmablastic lymphoma (PBL) is a rare CD20-negative aggressive lymphoma with poor prognosis when treated with standard treatment options. Although PBL is associated with HIV infection and other immunosuppressed states, it can also affect immunocompetent individuals. DIAGNOSIS: The diagnosis of PBL requires a high clinical suspicion and pathological confirmation. EBV-encoded RNA (EBER) expression and MYC gene rearrangements are frequently detected in the malignant cells. The differential diagnosis includes EBV+ diffuse large B-cell lymphoma (DLBCL), extracavitary primary eff

Delirium in burn patients: Incidence, risk factors, and management strategies.

Delirium is an acute, fluctuating disturbance of attention, awareness, and cognition. Delirium is common and multifactorial in pediatric and adult burn patients. Delirium can be hyperactive, hypoactive, or mixed in phenotype. This is an important complication associated with prolonged hospitalization, increased morbidity and mortality, and possibly long term cognitive and psychological sequelae, highlighting the importance of prevention and early recognition. Burn patients are particularly vulnerable due to the combination of the intense systemic inflammatory response, disruption of the blood

Investigating Risk Factors for Laryngotracheal Reconstruction and Assessing Postoperative Management.

This study examines demographic and etiological factors influencing the need for laryngotracheal reconstruction (LTR) by analyzing clinical studies with clear outcome measures, pre- and post-treatment scores, and demographic data while excluding case reports, reviews, and abstracts. A literature search through PubMed and Embase identified 49 articles, which were narrowed to six after abstract analysis. The results indicate that sex, age, and ethnicity were not significant predictors of the need for LTR, whereas the underlying etiology (particularly trauma, prolonged intubation, and idiopathic

Cardiovascular Risk Factor Management in Medicare Advantage and Traditional Medicare.

BACKGROUND: Although cardiovascular disease is the leading cause of death in the United States among Medicare beneficiaries, management of modifiable risk factors remains suboptimal. Medicare Advantage (MA) enrollment has increased substantially; therefore, understanding the quality of cardiovascular risk factor management in MA is critical. In this study, we evaluated whether cardiovascular risk factor management was better among MA compared with traditional Medicare (TM) beneficiaries. METHODS: We linked physical examination and laboratory data from the National Health and Nutrition Examina

Cardiovascular Risk in Autoimmune Diseases: Mechanisms, Management, and Emerging Evidence.

Cardiovascular disease is a leading cause of morbidity and mortality among patients with autoimmune disorders. Chronic inflammatory conditions such as rheumatoid arthritis, systemic lupus erythematosus, psoriasis, and inflammatory bowel disease are associated with accelerated atherosclerosis and other vascular complications. Persistent inflammation, dysregulated cytokine networks, and autoantibody-mediated pathways contribute to endothelial dysfunction, pro-thrombotic states, and metabolic disturbances. In addition to traditional risk factors, disease-specific elements such as chronic glucocor

Risk of Cardiovascular Complications in Thyrotoxicosis: Thyroidectomy Versus Nonoperative Management.

INTRODUCTION: Thyrotoxicosis is a hypermetabolic state that can be managed with antithyroid drugs, radioactive iodine (RAI), or thyroidectomy. However, the long-term cardiovascular effects of these treatments remain unclear. This study evaluates surgical versus nonsurgical approaches on new-onset cardiovascular complications in thyrotoxicosis patients. METHOD: A retrospective analysis of electronic medical records (2005-2024) using TriNetX data identified patients diagnosed with thyrotoxicosis. Propensity score matching (1:1) balanced surgical and nonsurgical cohorts. Kaplan-Meier analysis wa

Comprehensive Peri-Operative Risk Assessment and Management of Geriatric Patients.

Background: As the population ages, the prevalence of surgical interventions in individuals aged 65+ continues to increase. This poses unique challenges due to the higher incidence of comorbidities, polypharmacy, and frailty in the elderly population, which result in high peri-operative risks. Traditional preoperative risk assessment tools often fail to accurately predict post-operative outcomes in the elderly, overlooking the complex interplay of factors that contribute to risk in the elderly. Methods: A literature review was conducted, focusing on the predictive value of CGA for postoperativ

Perforated Peptic Ulcer in Lagos. Risk Factors and Management Outcome.

BACKGROUND: Complications of peptic ulcers, such as bleeding, gastric outlet obstruction, and perforation, can be life-threatening and may require surgical intervention. Perforated peptic ulcer (PPU) presents mostly as a surgical emergency and is one of the leading causes of secondary peritonitis with significant morbidity and mortality. The clinical outcome of surgical treatment of peptic ulcers varies due to the varying patterns of presentations and interventions between countries. There are few documented series on PPU in Nigeria. This paper reviews the management outcome of PPU in our inst

Essential thrombocythemia: 2024 update on diagnosis, risk stratification, and management.

OVERVIEW: Essential thrombocythemia is a Janus kinase 2 (JAK2) mutation-prevalent myeloproliferative neoplasm characterized by clonal thrombocytosis; clinical course is often indolent but might be interrupted by thrombotic or hemorrhagic complications, microcirculatory symptoms (e.g., headaches, lightheadedness, and acral paresthesias), and, less frequently, by disease transformation into myelofibrosis (MF) or acute myeloid leukemia. DIAGNOSIS: In addition to thrombocytosis (platelets ≥450 × 109 /L), formal diagnosis requires the exclusion of other myeloid neoplasms, including prefibrotic MF,

Endoscopic retrograde cholangiopancreatography-related complications: risk stratification, prevention, and management.

Endoscopic retrograde cholangiopancreatography (ERCP) plays a crucial role in the management of pancreaticobiliary disorders. Although the ERCP technique has been refined over the past five decades, it remains one of the endoscopic procedures with the highest rate of complications. Risk factors for ERCP-related complications are broadly classified into patient-, procedure-, and operator-related risk factors. Although non-modifiable, patient-related risk factors allow for the closer monitoring and instatement of preventive measures. Post-ERCP pancreatitis is the most common complication of ERCP
