Metformin: an effective attenuator of risperidone-induced insulin resistance hyperglycemia and dyslipidemia in rats.

The use of atypical antipsychotics in the clinical management of schizophrenia and schizoaffective disorders has been associated with the development of insulin resistance. The present study evaluates the possible individual ameliorating effect of single daily oral treatments with 20 mg/kg/day of metformin and 0.1 mg/kg of glibenclamide in two groups of Wistar rats pretreated with 0.2 mg/kg of risperidone for 60 days. Two additional groups of rats were only treated with 0.2 mg/kg of risperidone and 10 mL/kg of distilled water, respectively, also for 60 days. Results showed that oral pre-treatm…

Moderators, mediators, and other predictors of risperidone response in children with autistic disorder and irritability.

OBJECTIVE/BACKGROUND: The National Institute of Mental Health (NIMH) Research Units on Pediatric Psychopharmacology (RUPP) Autism Network found an effect size of d = 1.2 in favor of risperidone on the main outcome measure in an 8-week double-blind, placebo-controlled trial for irritability in autistic disorder. This paper explores moderators and mediators of this effect. METHOD: Intention-to-treat (ITT) analyses were conducted with suspected moderators and mediators entered into the regression equations. MacArthur Foundation Network subgroup guidelines were followed in the evaluation of the r…

Galactorrhea - side effect of risperidone in combination with depakine chrono in a patient with bipolar disorder.

Risperidone, as all atypical antipsychotics, can cause hyperprolactinemia which can in turn lead to galactorrhea. Mood stabilizers, one of which is valproic acid and its derivate "Depakine Chrono", are rarely linked with symptomatic hyperprolactinemia and do not alter prolactin concentrations. This case is based around a patient suffering from a bipolar disorder that has been psychiatrically treated in an outpatient clinic during four years. Bipolar disorder treatment was started with carbamazepine, but later it was discontinued due to adverse events and extreme increase of liver transaminases…

Risperidone: new indication. Behavioural disorders in children with autism or mental disabilities: no progress.

(1) Sedative drugs are one option when autistic or mentally disabled children have behavioural disorders that place them (or other people) in physical danger. Among the classic neuroleptics, haloperidol is the drug with the best-documented efficacy and safety. Placebo-controlled trials have also shown lithium to be effective for this use. (2) Clinical evaluation of risperidone in children with mental disabilities includes 3 placebo-controlled double-blind trials, 2 of which involved 118 and 110 children aged from 5 to 12 years who were treated for 6 weeks. All 3 trials showed a partial behavio…

The atypical antipsychotics risperidone, clozapine and olanzapine differ regarding their sedative potency in rats.

The atypical antipsychotics risperidone, clozapine and olanzapine were studied for their ability to antagonise apomorphine-induced stereotypy and to affect electroencephalographic (EEG) activity in rats. The compounds antagonised apomorphine-induced stereotypy with ED(50)s of 0.15 mg/kg s.c. for risperidone, 0.42 mg/kg s.c. for olanzapine and 1.3 mg/kg s.c. for clozapine. At a dose close to that required for apomorphine antagonism (0.16 mg/kg s.c.), risperidone induced only minor changes in EEG power spectral activity. At a higher dosage (0.63 mg/kg s.c.) it increased the power density in the …

Effectiveness of antipsychotic therapy in a naturalistic setting: a comparison between risperidone, perphenazine, and haloperidol.

BACKGROUND: Therapeutic ineffectiveness and noncompliance with antipsychotic agents are major contributors to rehospitalization in patients with psychotic disorders. It is unknown whether risperidone's favorable side effect profile compared with that of the conventional antipsychotics results in improved compliance and reduced hospitalizations in a naturalistic setting. The purpose of this study was to test the hypothesis that treatment with risperidone reduces readmission rates and associated costs when compared with treatment with perphenazine or haloperidol. METHOD: Inpatients prescribed e…

Acute and longer-term effects of risperidone in a case of first-episode catatonic schizophrenia.

This case report documents the response of a young, first-episode, neuroleptic naive male with severe catatonic schizophrenia to the novel antipsychotic, risperidone. Initial assessments included the Positive and Negative Syndrome Scales, Global Assessment of Function and the Extrapyramidal Symptom Rating Scale. These were repeated at discharge from hospital and during a 3.5 year outpatient follow-up. Neuroimaging (computed tomography, magnetic resonance imaging and single photon emission tomography) along with electroencephalogram and laboratory examinations were completed. Response to risper…

Risperidone in children and adolescents with pervasive developmental disorder: pilot trial and follow-up.

Dopamine receptor antagonists, particularly haloperidol, have been the most effective medications in currently available double-blind placebo-controlled studies for treating the disruptive behaviors often associated with pervasive developmental disorder (PDD). The rationale for trying risperidone in this population includes its dopamine-blocking activity; its seemingly lower incidence of tardive dyskinesia when compared to standard neuroleptics; the possibility that risperidone may ameliorate the social withdrawal of PDD, as it does the negative symptoms in schizophrenia; and substantial effec…

Therapeutic effect and safety of increasing doses of risperidone (R 64766) in psychotic patients.

Risperidone (R 64766) was administered during 4 weeks in increasing doses to 17 psychotic patients, to evaluate the hematological and cardiovascular safety, the therapeutic effect, side effects, effects upon endocrinological parameters and the pharmacokinetic profile. Following a placebo wash-out period of 1 week, the initial dose was 10 mg daily, increasing with 5 mg per week until the maximal dose of 25 mg daily was reached during the 4th week of treatment. Doses up to 20 mg daily resulted in a significant improvement of the total BPRS score and of the different BPRS factor scores; with high…

Risperidone-Induced Priapism: A Systematic Review of Risk Factors, Comprehensive Assessment, and Management Recommendations.

BACKGROUND: Risperidone, a second-generation antipsychotic with strong alpha-1 adrenergic receptor affinity, has been associated with priapism, a rare but serious urological emergency. This adverse effect remains underrecognized in psychiatric settings. We synthesized published reports on risperidone-induced priapism, summarized clinical characteristics, and highlighted preventive and management strategies. METHODS: A systematic literature search was conducted across PubMed, EMBASE, Scopus, Medline, PubMed Central, Google Scholar, PsyArXiv, and PsycINFO from 1957 to 2024 using combinations of…
