The conflicting vessels in hemifacial spasm: Literature review and anatomical-surgical implications.

Since several decades, it has been established that so-called primary hemifacial spasm is linked to neuro-vascular conflicts in the facial nerve, especially its root exit zone (REZ). Based on our review of the detailed publications of literature (2489 patients), together with our own series (340 patients), the responsible vessels found at surgery were: the posterior inferior cerebellar artery in 47.2%, the anterior inferior cerebellar artery in 45.9%, the vertebro-basilar artery in 17.5%, another (smaller) artery in 11.7%. Participation of veins was very diversely estimated according to series

Subjective visual vertical in first time seafarers experiencing sea sickness.

OBJECTIVES: Sea sickness, a form of motion sickness, is a common condition among sailors on various sailing vessels, primarily due to their roll and pitch movements. While the exact neurobiological mechanisms remain unclear, the most widely accepted explanation is the sensory conflict theory, which identifies two main sources of conflict: discrepancies between visual and vestibular inputs and conflicts within the vestibular system itself. This study aims to evaluate otolith-mediated verticality perception in first-time seafarers on board a naval frigate ship using the Subjective Visual Vertica

The multiplicity of caregiving burden: a qualitative analysis of families with prolonged disorders of consciousness.

Objective: To understand the multiple and sometimes conflicting roles substitute decision makers (SDMs) of individuals in a vegetative state (VS), minimally conscious state (MCS), or with locked-in syndrome (LIS) perform while caring for a loved one and the competing priorities derived from these roles. Methods: We conducted semi-structured qualitative interviews using a constructive-grounded theory design. Twelve SDMs, who were also family members for 11 patients, were interviewed at two time points (except one) for a total of 21 in-depth interviews. Results: Participants described that caregiv

Moral dilemmas and conflicts concerning patients in a vegetative state/unresponsive wakefulness syndrome: shared or non-shared decision making? A qualitative study of the professional perspective in two moral case deliberations.

BACKGROUND: Patients in a vegetative state/ unresponsive wakefulness syndrome (VS/UWS) pose ethical dilemmas to those involved. Many conflicts occur between professionals and families of these patients. In the Netherlands physicians are supposed to withdraw life sustaining treatment once recovery is not to be expected. Yet these patients have shown to survive sometimes for decades. The role of the families is thought to be important. The aim of this study was to make an inventory of the professional perspective on conflicts in long-term care of patients in VS/UWS. METHODS: A qualitative study

[Mental conflict of urban Korean housewives--a qualitative analysis of 20 interviews].

This study analysed marital conflicts, related factors, and adjustments to those conflicts described by urban Korean women. The purpose was to discover directions for resolutions that would enhance the helpfulness of volunteers or professionals working with women in counselling settings. The investigator interviewed 20 married women, all mothers and housewives living in Seoul, attending marriage encounter programs and the Lifeline telephone counselling service. An interview schedule using six open-ended questions guided the data collection which took place between May 31 and December 29, 1988.

Hippocampal Synaptic Plasticity: Integrating Memory and Anxiety Impairments in the Early Stages of Alzheimer's Disease.

A decline in hippocampal function has long been associated with the progression of cognitive impairments in patients with Alzheimer's disease (AD). The disruption of hippocampal synaptic plasticity [primarily the reduction of long-term potentiation LTP] by excess production of soluble beta-amyloid (Aβ) has long been accepted as the mechanism by which AD pathology impairs memory, at least during the early stages of AD pathogenesis. However, the premise that hippocampal LTP underpins the formation of associative, long-term memories has been challenged. Here, we consider evidence that this canoni

Gender as moderator of the relations between interpersonal and noninterpersonal trauma exposure and mental health problems in conflict-affected children and adolescents.

OBJECTIVE: This study examined the moderating role of gender in the associations between cumulative exposure to interpersonal trauma (IPT) and noninterpersonal trauma (NIPT) and mental health outcomes in children and adolescents in conflict-affected areas. METHOD: Based on a sample of 980 children (rangeage = 6-12, Mage = 9.60, 471 girls) and 472 adolescents (rangeage = 13-18, Mage = 15.26, 218 girls) living in or near conflict-affected regions (Burundi, Northern Uganda, Iraq, Syria, and Sri Lanka), multigroup path analyses were conducted to examine whether cumulative exposure to IPT and NIPT

Parental incarceration and multiple risk experiences: effects on family dynamics and children's delinquency.

Children of incarcerated parents are exposed to factors that place them at risk for delinquency. Few studies have examined the effects of having an incarcerated parent after controlling for other experiences such as contextual risk factors and family processes. Past studies have also not examined effects of recent, but not current, parental incarceration on children. The present study examines an archival dataset, in which children aged 10-14 years and their parents/guardians reported children's risk experiences (e.g., exposure to poverty, parental substance use), family processes (e.g., level

Association of Maternal and Paternal Depression in the Postnatal Period With Offspring Depression at Age 18 Years.

IMPORTANCE: Paternal depression during the postnatal period has been associated with adverse child outcomes. Family environment has been reported as a pathway for risk transmission from fathers to children. The influence of paternal depression during the postnatal period on offspring depression remains to be clarified. OBJECTIVE: To investigate the association between paternal depression in the postnatal period and offspring depression and explore potential mediating and moderating factors that influence any association between paternal and offspring depression. What Should an Intern Do When She Disagrees With the Attending?

Disagreements, including those between residents and attending physicians, are common in medicine. In this Ethics Rounds article, we present a case in which an intern and attending disagree about discharging the patient; the attending recommends that the patient be hospitalized longer without providing evidence to support his recommendation. Commentators address different aspects of the case. The first group, including a resident, focus on the intern's potential moral distress and the importance of providing trainees with communication and conflict resolution skills to address inevitable confl

Dysfunction in attentional processing in patients with Parkinson's disease and visual hallucinations.

Mechanistic insights into visual hallucinations (VH) in Parkinson's disease (PD) have suggested a role for impaired attentional processes. The current study tested 25 PD patients with and 28 PD patients without VH on the attentional network test. Hallucinators had significantly lower accuracy rates compared to non-hallucinators, but no differences were found in reaction times. This suggests that hallucinators show deficits in attentional processes and conflict monitoring. Our findings provide novel behavioural insights that dovetail with current neurobiological frameworks of VH.

Anniversary reactions in a five-year-old boy. Unresolved conflict, guilt, and self-identifications.

This paper examines the important role of anniversary reactions in the psychological adjustment of seriously traumatized individuals. The clinical case study examines the impact of anniversary reactions on the posttraumatic symptoms and accommodation of a five-year-old boy. Unresolved conflict with the mother, an intense sense of guilt, and difficulty in self-identification were explored during subsequent sessions of psychoanalysis. The clinical significance of considering the possibility of anniversary reactions in children manifesting repetitive attempts of trauma mastering is discussed.

Neuromodulation of prefrontal cortex promotes deep processing during language comprehension: a tDCS/EEG study.

In this study, we used transcranial direct current stimulation (tDCS), a noninvasive neuromodulation technique, to test a set of hypotheses about the extent to which the prefrontal cortex (PFC) contributes to revision and updating processes during language comprehension. Following 20 min of PFC-targeted, Active Control, or Sham tDCS, EEG was recorded while participants performed a widely used paradigm in which they read sentences containing plausible and implausible thematic roles (e.g. The actress/film-maker was directed by the film-maker/actress on set). This linguistic manipulation allowed

Household Conflict in the Time of COVID-19: the Impact of Public Policy on Family Dynamics in Marginalized Urban Communities with High Infection Risk.

During public health emergencies, infection control policies play a critical role in curbing disease transmission. However, little is known about the ways in which these policies shape family dynamics. Challenges may be more pronounced among families living in low-income, high-risk urban communities. This study investigated conflicts related to COVID-19 within households, using data from a randomized control trial (n = 392) conducted between 08/2021 and 07/2022. The participants-primarily Latino or Black (95%), aged 18-54 (54%), and earning under $25,000 annually (63%)-were recruited from NYC

Gene × environment effects of serotonin transporter, dopamine receptor D4, and monoamine oxidase A genes with contextual and parenting risk factors on symptoms of oppositional defiant disorder, anxiety, and depression in a community sample of 4-year-old children.

Genetic factors can play a key role in the multiple level of analyses approach to understanding the development of child psychopathology. The present study examined gene-environment correlations and gene × environment interactions for polymorphisms of three target genes, the serotonin transporter gene, the D4 dopamine receptor gene, and the monoamine oxidase A gene in relation to symptoms of anxiety, depression, and oppositional behavior. Saliva samples were collected from 175 non-Hispanic White, 4-year-old children. Psychosocial risk factors included socioeconomic status, life stress, caretak

MRI volumetry for the preoperative diagnosis of trigeminal neuralgia.

To assess whether quantitative measuring methods can help improve the reliability of MRI-based evaluations of the pathological role of a neurovascular conflict between an artery and the trigeminal nerve. In a prospective study, magnetic resonance images were obtained from 62 patients with unilateral facial pain and 50 healthy test subjects. In coronal T1- and T2-weighted sequences volume measurements were performed by regions of interest and compared intraindividually (healthy versus affected side in the patient populations and right versus left side in the group of test subjects) and on the b

[Space motion sickness].

This review describes symptoms of space motion sickness (SMS) observed in many cosmonauts during the first day in orbit. It also discusses similar symptoms detected in them upon return to the Earth which constitute the readaptation syndrome. The paper gives a detailed discussion of different theories that explain the etiology and pathogenesis of SMS and the post-flight readaptation syndrome, viz, sensory conflict or sensory mismatch, disorders in the labyrinthine paired function, and blood and cerebrospinal fluid (CSF) circulation. It is emphasized that none of the theories used separately can

Homeostatic microglia initially seed and activated microglia later reshape amyloid plaques in Alzheimer's Disease.

The role of microglia in the amyloid cascade of Alzheimer's disease (AD) is debated due to conflicting findings. Using a genetic and a pharmacological approach we demonstrate that depletion of microglia before amyloid-β (Aβ) plaque deposition, leads to a reduction in plaque numbers and neuritic dystrophy, confirming their role in plaque initiation. Transplanting human microglia restores Aβ plaque formation. While microglia depletion reduces insoluble Aβ levels, soluble Aβ concentrations stay consistent, challenging the view that microglia clear Aβ. In later stages, microglial depletion decreas
