Clinicopathological and molecular features of myeloid sarcoma as initial presentation of therapy-related myeloid neoplasms: a single institution experience.

Therapy-related myeloid neoplasms (t-MN) have a common origin in prior cytotoxic therapy and/or radiation. These neoplasms include therapy-related acute myeloid leukemia, myelodysplastic syndrome (t-MDS), and myelodysplastic/myeloproliferative neoplasms (t-MDS/MPN). Myeloid sarcoma (MS), on the other hand, is a rare disease manifesting as an extramedullary collection of immature cells of myeloid lineage. Rarer still is therapy-related MS (t-MS), which has not been adequately studied due to its rarity and its lack of recognition as a unique entity among other t-MN. Here, we report what is to ou

Intracranial extramedullary relapse of acute myeloid leukemia presenting as myeloid sarcoma mimicking meningioma: a case report and literature review.

BACKGROUND: Myeloid sarcoma (MS) is a rare extramedullary tumor of immature myeloid cells. Intracranial MS occurring as an extramedullary relapse without concurrent systemic acute myeloid leukemia (AML), is exceptionally rare and poses a considerable diagnostic challenge due to its non-specific clinical and radiological features, often mimicking more common intracranial neoplasms like meningioma. CASE PRESENTATION: We report a case of isolated intracranial MS in a 61-year-old female with a prior history of FLT3-ITD mutated AML (M2), who had been in sustained complete hematologic remission for

Testicular myeloid sarcoma: a relapse of acute myeloid leukaemia after allogeneic peripheral blood stem cell transplantation- a rare presentation.

Myeloid sarcoma is an uncommon pathological diagnosis of proliferation of blasts of one or more of the myeloid lineages in regions other than the blood and bone marrow. Myeloid sarcoma of the testis after allogeneic bone marrow stem cell transplantation is very rare and only few cases are reported in the literature. It is usually misdiagnosed as malignant lymphoma, particularly with large cell lymphoma, due to similar histological morphology. Due to difficulty in diagnosis, it is suggested that an appropriate panel of immunohistochemical marker studies be performed in conjunction with clinical

Radiotherapy for Orbital Myeloid Sarcoma as Extramedullary Relapse in a Pediatric Acute Myeloid Leukemia Patient Post-haploidentical Bone Marrow Transplantation.

The proliferation of immature granulocytic cells characterizes myeloid sarcoma (MS), which can be a rare extramedullary manifestation of acute myeloid leukemia (AML). MS can occur as an isolated lesion, concurrently with AML, or as a post-treatment relapse. In this case report, we describe a 12-year-old male with high-risk AML who underwent chemotherapy followed by haploidentical hematopoietic stem cell transplantation (HSCT). Two years post-HSCT, he experienced a relapse with testicular MS, requiring orchiectomy and radiation therapy. One year later, he experienced a second relapse, with the

First Case of FLT3-Tyrosine Kinase Domain Mutant Acute Myeloid Leukemia With Unusual Onset as Isolated Bilateral Testicular Myeloid Sarcoma.

Testicular myeloid sarcoma (TMS) is a challenging pathology often posing diagnostic difficulties due to the poorly differentiated nature of tumor cells at the initial presentation. The delay in diagnosis significantly impacts patient life expectancy, emphasizing the need for prompt identification and treatment initiation. In certain cases, the presence of the Fms-like tyrosine kinase (FLT3) mutation adds complexity to the disease, requiring tailored therapeutic approaches. In this report, we present a unique case of bilateral TMS with FLT3 tyrosine kinase domain (TKD) mutation. The patient exh

Myeloid sarcoma as the only manifestation in a rare mixed lineage leukemia-fusion-driven acute myeloid leukemia: A case report.

BACKGROUND: The mixed lineage leukemia (MLL)-eleven-nineteen lysine-rich leukemia (ELL) fusion gene is a rare occurrence among the various MLL fusion genes. We present the first case in which myeloid sarcoma (MS) was the only manifestation of adult MLL-ELL-positive acute myeloid leukemia (AML). CASE SUMMARY: We report a case of a 33-year-old male patient who was admitted in June 2022 with a right occipital area mass measuring approximately 7 cm × 8 cm. Blood work was normal. The patient underwent right occipital giant subscalp mass excision and incisional flap grafting. Immunohistochemistry w

Successful Treatment of Relapsed Pediatric Acute Myeloid Leukemia Presenting as Central Nervous System Myeloid Sarcoma: A Single-Institution Case Series.

Relapsed acute myeloid leukemia presenting as an isolated central nervous system myeloid sarcoma (CNS MS) is very rare and generally entails poor outcomes. CNS MS treatment is not well defined and can include systemic chemotherapy, intrathecal chemotherapy, radiation therapy, or hematopoietic stem cell transplant. Thiotepa, vinorelbine, topotecan, and clofarabine (TVTC) has been successful for reinduction therapy in relapsed/refractory leukemia to induce remission before hematopoietic stem cell transplant. There is no published evidence of TVTC being utilized for CNS MS. In this series, we rep

Prognostic Role of Postinduction Minimal Residual Disease and Myeloid Sarcoma Type Extramedullary Involvement in Pediatric RUNX1-RUNX1T1 (+) Acute Myeloid Leukemia.

BACKGROUND: Acute myeloid leukemia with the t(8;21)(q22;q22) rearrangement (RUNX1-RUNX1T1 (+) AML) is known to have a favorable prognosis. Our study aimed to determine the most important prognostic variables among an aggregate of clinical, genetic, and treatment response-based factors in pediatric RUNX1-RUNX1T1 (+) AML. MATERIALS AND METHODS: We analyzed the characteristics and outcome of 40 patients who were diagnosed with and treated for RUNX1-RUNX1T1 (+) AML from April 2008 to December 2016 at our institution. RESULTS: A<-2.2 log fusion transcript decrement after remission induction, myel

Acute myeloid leukemia with concurrent myeloid sarcoma treated with autologous bone marrow transplantation: two illustrative cases and a literature review.

Myeloid sarcoma (MS) is an invasive extramedullary solid tumor composed of immature cells of the myeloid series. It complicates the clinical course of a minority of patients with acute myeloid leukemia (AML). Traditionally its presence has been regarded as an indicator of aggressive disease. Currently, the optimal treatment of AML with concurrent MS remains to be determined. We report two cases of autologous bone marrow transplantation (auto-BMT) for AML with concurrent MS followed by a review of the literature.

Two Cases of Myeloid Sarcoma at Rare Sites: Cytological Manifestations and Diagnostic Experience.

Myeloid sarcoma is a rare extramedullary manifestation of acute myeloid leukemia, and primary involvement of the gastrointestinal tract and uterine cervix is particularly uncommon. Due to its low incidence and nonspecific clinical manifestations, the rates of clinical misdiagnosis and missed diagnosis remain high. Herein, we report two cases of myeloid sarcoma arising at rare sites. Case 1 was a 52-year-old male who presented with acute gastrointestinal obstruction. Intraoperative frozen section favored lymphoma, whereas touch imprint cytology of the tumor cut surface revealed numerous round t

Myeloid sarcoma transformed from myeloproliferative neoplasm: A case report and review of literature.

BACKGROUND: Myeloid sarcoma (MS) is a rare hematological malignancy that may be associated with myelodysplastic or myeloproliferative neoplasms. Herein, we report a case of transformation from myelofibrosis to MS. CASE SUMMARY: A 56-year-old male patient was found to have multiple bone lesions by the pelvic magnetic resonance imaging. Pathological analysis of the lesions indicated a myeloid tumor, and immunohistochemistry revealed a TP53 mutation. Bone marrow aspiration was consistent with myelofibrosis. Based on the patient's history of polycythemia and the immunohistochemical findings of th

A Rare Case of Partial Small Bowel Obstruction Secondary to Intestinal Myeloid Sarcoma.

This case report details a rare case of small bowel myeloid sarcoma (MS) in an otherwise fit and well 49-year-old male presenting initially with vague obstructive symptoms and weight loss. The patient ultimately required an operation for a small bowel obstruction where a laparotomy and small bowel resection were performed due to three cicatrising completely obstructing lesions in the mid-jejunum. Fewer than 1% of patients with acute myeloid leukaemia (AML) present with MS as an initial diagnosis, and only 6.5% of these are intestinal in origin. This report adds to the current body of literatur

Multimodal imaging study of pancreatic myeloid sarcoma: a case report and literature review.

Myeloid sarcoma (MS) is a rare extramedullary parenchymal tumor composed of immature myeloid cells, occurring mainly in the lymph nodes, skin, soft tissue, testicles, bones, peritoneum, and gastrointestinal tract, and rarely in the pancreas. Herein, we report the case of a 68-year-old female patient who visited our hospital for medical assistance due to acute abdominal pain. Abdominal computed tomography (CT) and magnetic resonance imaging showed a mass approximately 8 cm in diameter in the pancreatic tail, which was suspected to be a malignant tumor. To further assess the presence of distant

18F-FDG PET/CT Imaging of Testicular Myeloid Sarcoma in a Pediatric Patient.

A 5-year-old boy presented with right testicular swelling for 1 week. Ultrasonography revealed a right testicular hypoechoic mass with increased blood flow. A malignant tumor was suspected, and further investigation with F-FDG PET/CT showed a soft tissue mass with increased FDG uptake in the right testis. Biopsy of the mass confirmed a diagnosis of myeloid sarcoma.

De novo isolated myeloid sarcoma: comparative analysis of survival in 19 consecutive cases.

Institutional database search (1999-2020) for acute myeloid leukaemia (AML) identified 109 cases of myeloid sarcoma (MS), of which 19 were isolated and presented de novo. The latter displayed longer survival (median 78 months), compared to MS with synchronous intramedullary AML (n = 32; median 16 months) and de novo AML without MS (n = 729; median 22 months; P = 0·13). However, the difference in survival was no longer apparent after accounting for bone marrow cytogenetic risk status (P = 0·67). Treatment-induced MS tumour resolution was not affected by the presence of intramedullary disease (P
