Purpose: To examine the spectrum of scleritis in four tertiary institutions across the Asia-Pacific. Methods: Clinical records from 354 patients were reviewed from centers in Australia, Singapore, and India, excluding those with insufficient data (n = 24). Results: Indian patients presented younger (41.5 ± 13.4 years) than Australians (50.8 ± 17.5) and Singaporeans (48.6 ± 15.9), with fewer women (49% vs 62%/57%). Diffuse disease was universally most common. Autoimmune and infectious disease proportions were similar in Australia (31%/10.3%) and Singapore (27.5%/8.3%) but reversed in India (8.3…

Scleritis is an inflammatory disease of the sclera; outer tunic of the eye on which the oculomotor muscles are inserted. It can be associated with a systemic disease up to one time out of 3. These associated diseases are mainly rheumatoid arthritis, vasculitis, including granulomatosis with polyangiitis in the first line and spondyloarthropathies. Before mentioning such an etiology, it is necessary to eliminate an infectious cause, mainly herpetic, which is regularly underestimated. The classification of scleritis is clinical. We distinguish between anterior scleritis and posterior scleritis. …

Scleritis is an umbrella term for a heterogeneous group of ocular diseases that can be associated with autoimmune or systemic disorders. The purpose of this article was to review the literature regarding the use of biologic drugs to treat refractory scleritis. A search of the MEDLINE, Embase, and LILACS electronic databases was conducted, and the reference lists of published articles were hand-searched. No language filters were used. This search strategy yielded no randomized trials of the use of biologic therapies to treat scleritis; only case reports and retrospective studies were retrieved.…

The aim of this study is to report clinical and imaging findings, and treatment outcomes of a patient with nodular posterior scleritis. A 41-year-old woman was diagnosed as nodular posterior scleritis in the light of clinical and imaging findings. At first admission best corrected visual acuity was 20/50 in her right eye. Fundus examination revealed an amelanotic subretinal mass under the superior temporal arcade associated with subretinal fluid surrounding it. B-scan ultrasonography, optical coherence tomography, fluorescein angiography, and indocyanine green angiography findings confirmed th…

PURPOSE: To design a slit-lamp mountable spectrometer for the assessment of ophthalmic patients and to illustrate a potential clinical application by measuring the spectral characteristics of inflamed eyes of differing aetiologies.
METHODS: A slit lamp mountable instrument was designed and built, and methods for data analysis developed. Reflectance spectra were recorded from two patients with scleritis, three with non-scleritic red eyes and from two controls with non-inflamed eyes.
RESULTS: Measurements were reproducible and demonstrated statistically significant differences in the spectral …

Intr … elopment of scarring and thinning along with complete resolution of choroidal detachment. Infective scleritis should be considered as one of the provisional diagnosis in patients presenting with features of unilateral ocular inflammation, especially in cases with history of trauma or in predisposed individuals.

Surgically induced necrotizing scleritis, which can lead to potentially devastating consequences, is a rare sequela to many types of ocular surgeries. We describe a 58-year-old woman who developed necrotizing scleritis following strabismus surgery for thyroid ophthalmopathy. Tectonic corneal patch grafting resulted in a favorable outcome.

In this retrospective case series, we studied the predisposing factors, causative organisms, clinical spectrum, and outcomes of 12 cases of culture-proven infectious scleritis. Nine of 12 patients had a history of preceding trauma (surgical or accidental). Past surgical history included small-incision cataract surgery (4), pterygium surgery (1), and trabeculectomy (1). Six patients had multifocal scleral abscesses due to Pseudomonas, Klebsiella, or Nocardia. Only 2 patients retained useful vision (>6/18). A poor visual acuity at presentation usually resulted in a worse visual outcome (P = 0.00…

A 46-year-old man developed surgically induced necrotizing scleritis (SINS) 7 months following uneventful primary bare sclera pterygium excision. The patient underwent two scleral patch grafts for SINS but despite the use of cyclophosphamide and azathioprine, the scleral patch grafts failed within 1 month of surgery on both occasions. Tacrolimus was used for his third scleral patch graft. No recurrence of necrotizing scleritis was observed for 2 years. Tacrolimus may be considered as a valuable adjunctive immunosuppressant in the management of resistant necrotizing scleritis.

PURPOSE: To report the case of a patient with undiagnosed Hodgkin's lymphoma who presented with coexistent unilateral nodular episcleritis and scleritis.
DESIGN: Interventional case report and literature review
METHODS: Review of clinical history, laboratory findings, histology of episcleral and cervical lymph node biopsies, and follow-up.
RESULTS: A 20-year-old female presented with a 5-month history of redness and pain in her left eye, with associated symptoms of dyspnea, malaise, and fever. The patient was found to have multifocal nodular episcleritis and scleritis that was not responsiv…

PURPOSE: This study aimed to describe the clinical characteristics of scleritis presented de novo in patients older than 60 years of age.
DESIGN: Type of scleritis, ocular manifestations, previous ocular surgery, and disease association were evaluated in patients older than 60 years with scleritis. Comparisons were made between patients with scleritis older and younger than 60 years.
RESULTS: Patients with scleritis older than 60 years (61 of 195 patients, 31.3%) had more necrotizing scleritis (47.5%, p=0.0001), decrease in vision (57.4%, p=0.0001), peripheral ulcerative keratitis (29.5%, p=…

BACKGROUND: Presentation and surgical treatment of a sequestrating senile scleral plaque, a rare clinical disorder.
PATIENT: A 76 year-old-female patient presented with foreign body sensation and epiphora. Half a year before, she had first noticed a "black spot" in her eye which had now progressed to a painless yellow-greyish plaque.
CASE HISTORY: The patient had previously undergone a steroid-treatment elsewhere with a diagnosis "necrotizing scleritis" without any improvement. We found a yellow-greyish degenerative plaque sequestrating from the surrounding scleral tissue. In order to preven…

PURPOSE: The purpose of the study is to describe an unusual case of infectious posterior scleritis after excision of a pterygium.
METHODS: The case history of a patient in whom anterior and posterior scleritis developed after excision of a pterygium is reviewed. Histopathologic findings are presented.
RESULTS: Pathologic examination results of a scleral biopsy site and the eye showed extensive involvement of the posterior sclera with fungi that on culture grew Pseudallescheria boydii. The development of posterior scleral thickening and intrascleral abscesses gave the clinical impression of a…

Exudative retinal detachment and oedema of the posterior retina are well-recognized clinical entities for which a cause is rarely found. Scleritis may cause both these clinical syndromes by a spread of inflammation from the sclera into the uveal tract. Although treatment may result in a rapid resolution of scleritis, retinal detachment and macular oedema persist and prove extremely resistant, even to high doses of systemic steroids. Recovery may be incomplete and may occur only after weeks or months of treatment.
