Capsaicin and nicotine-sensitive afferent neurones and nasal secretion in healthy human volunteers and in patients with vasomotor rhinitis.

1. Applications of capsaicin, nicotine and methacholine were made locally onto the nasal mucosa in human controls and patients suffering from hyperreactive nasal disorders. Perception of sensation was registered as a sympton score and secretion quantified. The sensory reaction (irritation - pain) to capsaicin was similar in the three groups studied, i.e. controls, a group of patients with the diagnosis of vasomotor rhinitis and a group of patients with increased nasal secretion as the main symptom of the hyperreactive disorder. Nicotine induced only a mild itching sensation in the three groups

Effects on gastrointestinal functions and symptoms of serotonergic psychoactive agents used in functional gastrointestinal diseases.

The effects of antidepressants on the gastrointestinal tract may contribute to their potential efficacy in functional dyspepsia and irritable bowel syndrome; buspirone, a prototype 5-HT1A agonist, enhances gastric accommodation and reduces postprandial symptoms in response to a challenge meal. Paroxetine, a selective serotonin reuptake inhibitor, accelerates small bowel but not colonic transit, and this property may not be relevant to improve gut function in functional gastrointestinal disorders. Venlafaxine, a prototype serotonin norepinephrine reuptake inhibitor, enhances gastric accommodati

Functional disturbances within frontostriatal circuits across multiple childhood psychopathologies.

OBJECTIVE: Neuroimaging studies of healthy individuals inform us about the normative maturation of the frontostriatal circuits that subserve self-regulatory control processes. Findings from these studies can be used as a reference frame against which to compare the aberrant development of these processes in individuals across a wide range of childhood psychopathologies. METHOD: The authors reviewed extensive neuroimaging evidence for the presence of abnormalities in frontostriatal circuits in children and adults with Tourette's syndrome and obsessive-compulsive disorder (OCD) as well as a mor

Management of abdominal bloating and distension, from subjective to objective.

Abdominal distension is a clinical occurrence that involves a measurable, objective increase in abdominal circumference, which patients report as feeling like pregnant or like having a balloon inside the abdomen. This sign is often preceded by a subjective feeling of abdominal heaviness or bloating, reported as the sensation of having a huge amount of gas trapped inside. These manifestations are highly prevalent and may reflect on their own a gut-brain axis condition, such as functional abdominal distension, or be part of other disorders such as functional dyspepsia or irritable bowel syndrome

Restless leg syndrome in the differential diagnosis of entrapment and peripheral neuropathies.

Restless leg syndrome (RLS) is an abnormal sensation disorder. Defining the syndrome is difficult. It is transmitted autosomal dominant genetically, is especially prevalent in the lower limbs, and is seen in both genders. In the differential diagnosis of RLS, nocturnal leg cramps, akathisia, peripheral neuropathy, entrapment neuropathy, and vascular disease (for example, deep vein thrombosis) should be considered. A 52-year-old woman was admitted to our clinic with signs of paresthesia, she had abnormal sensation disorder in both legs and the right arm, which she had difficulty defining. She h

Anxiety and/or the depressive syndromes.

The relationship of anxiety to depression in a number of the depressive and neurotic syndromes (agitated depressive syndrome, affective hypochondriacal syndrome, depressed obsessive-compulsive syndrome, phobic and obsessive-compulsive syndrome, anxiety state, depressive neurotic syndrome) is far from clear. Are anxiety and depression entirely separate disorders which may co-exist in the same patient? Does anxiety give rise to the depression? Does depression give rise to the anxiety? Are anxiety and depression manifestations of a single underlying disorders, dysphoria? Therapeutic responsivity

Esophageal motor disorders: videofluoroscopic and manometric evaluation--prospective study in 88 symptomatic patients.

Esophageal motor disorders are best evaluated with manometry, which, however, is time-consuming and not generally available. The authors prospectively investigated the yield of videofluoroscopy in detection of esophageal motor disorders in comparison with that of manometry. Eighty-eight patients with dysphagia, globus sensation, noncardiac chest pain, or progressive systemic sclerosis underwent both manometry and videofluoroscopy at 0-32-day intervals. Videofluoroscopy was performed with one swallowing study in the upright position and up to three swallowing studies in the prone oblique positi

Sensory hypersensitivity predicts repetitive behaviours in autistic and typically-developing children.

The objective of this study was to examine the relationship between sensory hypersensitivity and restricted interests and repetitive behaviours associated with autism spectrum disorder and their typically-developing peers. Furthermore, the aims included the examination of the relationship across sensory modalities and various types of restricted interests and repetitive behaviours. Data were collected from the parents of 114 children: 49 of whom were diagnosed with autism spectrum disorder and 65 typically-developing children. Parents completed the Sensory Profile 2 - Child Version and the Rep

Mitochondrial DNA Heteroplasmy Associations With Neurosensory and Mobility Function in Elderly Adults.

BACKGROUND: Mitochondrial DNA (mtDNA) heteroplasmy is a mixture of normal and mutated mtDNA molecules in a cell. High levels of heteroplasmy at specific mtDNA sites lead to inherited mitochondrial diseases with neurological, sensory, and movement impairments. Here we test the hypothesis that heteroplasmy levels in elderly adults are associated with impaired function resembling mild forms of mitochondrial disease. METHODS: We examined platelet mtDNA heteroplasmy at 20 disease-causing sites for associations with neurosensory and mobility function among 137 participants from the community-based

Conversion disorder and its subtypes: a need for a reclassification.

Four subtypes of conversion disorder were described in DSM-IV. There are few publications concerning studies aimed at separating the subtypes of the conversion disorder. Usually, pseudoseizures are in focus and attempts are made to differentiate these seizures from other disorders. The aim of the present study has been to investigate differences between the four subtypes of the conversion disorder and to discuss the possibilities for a reclassification. Ninety-five patients were seen by two researchers and diagnosed as conversion disorders. The subtypes were determined according to DSM-IV crit

Constipated patients with functional defecatory disorder have secondary rectal hyposensitivity due to altered rectal biomechanics.

BACKGROUND AND AIM: Rectal hyposensitivity (RH) is common in constipation and often coexists with functional defecatory disorder (FDD). Rectal sensory thresholds are routinely evaluated with the anorectal manometry probe; however, the gold standard for the assessment of rectal sensitivity is with a barostat, use of which is limited by time constraints and availability. A novel rapid barostat bag (RBB) may facilitate measurements of rectal sensitivity. The aim is to evaluate the relationship between RH (measured by the RBB) and FDD (defined as any minor disorder of rectoanal coordination by the

The contribution of ADHD and attachment difficulties to online pornography use among students.

BACKGROUND: There is a high rate of ADHD among individuals with compulsive sexual behavior disorder (CSBD). There is also evidence for an association between compulsive sexual behavior disorder, sensation seeking and attachment difficulties. Problematic pornography use may be considered as a subtype of CSBD hence it merits investigation. The aim of the current study was to investigate the relationships between ADHD, attachment style, sensation seeking and problematic use of pornography online in the general population. METHODS: The sample was comprised of 85 participants [38 men (44.7%) and 4

Associations between dispositions to rash action and internalizing and externalizing symptoms in children.

Impulsivity is not a unitary construct; instead, dispositions to rash action can be divided into five moderately-correlated dimensions. However, the associations between these dimensions and symptoms of psychopathology among youth remain unclear. The goal of this study was to examine associations between different dispositions to rash action and psychopathology in a community sample of middle school youth. One hundred forty-four youth (M age = 11.9; 65% Hispanic, 30% African American; 50% male; 81% qualifying for free school lunches) participated in this study. Self-reported questionnaire meas

Interoceptive hypersensitivity as prognostic factor among patients with panic disorder who have received cognitive behavioral therapy.

The efficacy of cognitive behavioral therapy (CBT) in the acute-phase treatment of panic disorder is well established. However, there are data to show CBT may not always be able to prevent recurrence after treatment. The central cognitive component of panic disorder psychopathology is thought to be hypersensitivity to physical sensations. The present study reports that some aspects of interoceptive hypersensitivity, gastrointestinal fears in particular, were predictive of the course of panic disorder after end of CBT. Clinically it is suggested that new interoceptive tasks related to gastroint

Acute psychotic reactions and stress response syndromes following intramuscular aqueous procaine penicillin.

Acute non-anaphylactic reactions to procaine penicillin include fear of death, sensations of disintegration, perceived changes of body shape, and visual and auditory hallucinations. Fragments of the original reaction experience, or elaborations of it, feature prominently in a chronic syndrome which persists if the initial reaction is not supported emotionally or professionally. The cultural acceptance of the reality of 'mysterious' forces played a role in the syndrome. Compatible diagnostic labels were panic disorder, post-traumatic disorder, mixed phobic disorder, and depressive illness. The

Trait impulsivity moderates rate of alcohol consumption in daily life.

INTRODUCTION: Rate of alcohol consumption, the speed with which people drink, has been linked to a range of outcomes, including alcohol use disorder symptoms and increased positive affect. However, minimal work has identified who is most likely to drink at elevated rates. Impulsivity is associated with increased attention to positive reinforcers specifically (e.g., positive affect). We therefore examined whether people higher in trait impulsivity engage in faster consumption during drinking episodes. METHODS: Participants were current drinkers (N = 113; 54 people with borderline personality d

Psychological, medical, and pharmacological correlates of persistent genital arousal disorder.

INTRODUCTION: Little is known about the etiology or medical/psychological correlates of persistent genital arousal disorder (PGAD). AIM: The aims of this article were (i) to replicate the findings of earlier research identifying two subtypes of women with persistent arousal-those who meet all features of the condition and are at least moderately distressed, and those who meet only some features and are less distressed; and (ii) to identify the medical, psychological and/or pharmacological correlates of the condition. METHOD: A comprehensive web-based survey of persistent genital arousal (PGA

Anxiety sensitivity moderates behavioral avoidance in anxious youth.

Individuals who are high in anxiety sensitivity (AS) are motivated to avoid sensations of anxiety. Consequently, AS is hypothesized to contribute to overall avoidance of any feared stimuli. No studies have yet examined whether fear of a stimulus is a stronger predictor of behavioral avoidance in individuals who are high in AS compared to individuals who are low in AS. We examined whether AS moderates the association between fear of spiders and behavioral avoidance of spider stimuli in 50 clinically anxious youth. Fear of spiders significantly predicted avoidance of spider stimuli in youth high

[Hyperventilation. The cognitive-behavior-therapy and the technique of the exercises in the induction of panic disorder symptoms].

Panic disorder (PD) is characterized by frequent and recurrent acute panic attacks. The current cognitive conception for PD enhances the fear of corporal sensations, when the individual define his physical sensations as being related to a serious disease or imminent death, not interpreting them as natural physiologic mechanisms. We reported three severe cases of PD of the respiratory subtype, with agoraphobia, whose symptoms of hyperventilation predominated on clinical picture. The treatment consisted on tricycles antidepressant and cognitive-behavior therapy (CBT) sessions. The objective of r

Short-term focused cognitive therapy of panic disorder with aerophobia: a case report.

The efficacy of focused cognitive therapy was evaluated in the short-term treatment of a flight instructor suffering from panic disorder with aerophobic and agoraphobic symptoms. A modified panic induction technique (hyperventilation) was used in session to activate the specific bodily sensations he typically experienced during panic attacks. The patient was then asked to articulate his catastrophic interpretations of the physiological sensations he was experiencing and taught to re-interpret them in a non-catastrophic manner, which enabled him to reduce his symptoms to a more manageable level
