The floating shoulder—-clinical and radiological results after intramedullary stabilization of the clavicle in cases with minor displacement of the scapular neck fracture.

BACKGROUND: The aim of this study was to compare the clinical and radiologic results of titanium elastic nail (TEN) and plate osteosynthesis for treatment of clavicle fractures in patients with a floating shoulder injury. PATIENTS AND METHODS: From 2000 to 2008, 16 patients with a floating shoulder injury (ipsilateral clavicle and minor displaced scapular neck fracture) were treated by isolated stabilization of the clavicle. The patients were treated with open reduction and plate osteosynthesis (group 1[G1]) or TEN osteosynthesis (group 2 [G2]). Both procedures were compared with regard to fu

Management of peri-prosthetic fracture of the humerus with severe bone loss and loosening of the humeral component after total shoulder replacement.

There is little information about the management of peri-prosthetic fracture of the humerus after total shoulder replacement (TSR). This is a retrospective review of 22 patients who underwent a revision of their original shoulder replacement for peri-prosthetic fracture of the humerus with bone loss and/or loose components. There were 20 women and two men with a mean age of 75 years (61 to 90) and a mean follow-up 42 months (12 to 91): 16 of these had undergone a previous revision TSR. Of the 22 patients, 12 were treated with a long-stemmed humeral component that bypassed the fracture. All the

EVIDENCE-BASED REVIEWS & RECOMMENDATIONS FROM THE BRAZILIAN SOCIETY OF SHOULDER AND ELBOW SURGERY: REVERSE ARTHROPLASTY FOR COMPLEX PROXIMAL HUMERAL FRACTURES IN ELDERLY PATIENTS.

INTRODUCTION: Complex (Neer 3- or 4-part) proximal humeral fractures in elderly patients are associated with high complication and failure rates after open reduction and internal fixation with locking plates (ORIF-LP). Reverse shoulder arthroplasty (RSA) has emerged as an alternative, but its comparative effectiveness remains debated. METHODS: We conducted an evidence-based review of systematic reviews and randomized controlled trials including elderly patients with Neer 3- or 4-part fractures, identified through PubMed, Embase, Cochrane Library, and BVS (search to August 2025). High-quality

Concurrent Revision Reverse Total Shoulder Arthroplasty and Scapular Spine ORIF for Periprosthetic Fractures: A Report of 2 Cases Using a Dual-Strategy Solution.

CASE: Two septuagenarian women presented with chronic periprosthetic scapular spine fractures after reverse total shoulder arthroplasty (RTSA) placement. Preoperative radiographs revealed a displaced unstable scapular spine fracture with scapula notching and onlay humeral position. One case additionally demonstrated uptilted baseplate placement. Both patients underwent concurrent revision RTSA with fixation of the scapular spine to correct component malposition and reduce glenohumeral tension. At 27-month follow-up, both achieved radiographic union, pain reduction, and functional gains without

Comparison of Short Uncemented Metaphyseal Stem and Long-Stem Reverse Shoulder Arthroplasty in Proximal Humerus Fractures: Preliminary Study at 2-Year Follow-Up.

Introduction: In the last few years, short metaphyseal-socket prosthetic humeral stems have been introduced for reverse shoulder arthroplasty (RSA). A short stem may have advantages in humeral force distribution, reducing shear stress and preserving bone stock, keeping in mind the need for possible future revision surgery. The main objective of our study was to validate the use of a short stem prosthesis in the surgical treatment of humeral fractures by comparing clinical and radiological outcomes of our studied implant with those obtained with the use of traditional long-stem implants. Shoulder arthroscopy combined to hardware removal in proximal humeral fractures: a series of 58 cases with a mean follow-up of 2 years.

BACKGROUND: Proximal humerus fractures are common injuries. Locking plates and anterograde medullary nails are the two most common fixation devices used when open reduction and internal fixation (ORIF) is indicated. Complications related to fracture and to hardware are numerous, especially shoulder stiffness. The goal of this study is to report the clinical outcomes of gleno-humeral arthroscopic arthrolysis combined with hardware removal. METHODS: A total of 58 patients (25 men, 33 women) with a mean age of 58 years (24-79) were reviewed retrospectively. Forty of them were active workers (5 h

Effect of closed reduction and external fixation in elderly patients with anterior shoulder dislocation combined with humeral surgical neck fracture: a retrospective cohort study based on fracture-dislocation classification.

BACKGROUND: Anterior shoulder dislocation combined with humeral surgical neck fracture is a severe injury that often requires emergency surgery. However, for some patients, such as those of advanced age or with surgical contraindications, closed reduction can be considered. This retrospective cohort study aimed to confirm the feasibility and clinical outcomes of the closed reduction technique with effective external fixation for the treatment of anterior shoulder dislocation combined with humeral surgical neck fracture in elderly patients. METHOD: From January 2010 to March 2024, a total of 5

Effect of lateralization and distalization on tuberosity healing and functional outcomes after reverse total shoulder arthroplasty with the Univers Revers Total Shoulder System for proximal humerus fractures: a randomized controlled trial comparing 135° and 155° uncemented stems.

BACKGROUND: Controversy remains regarding whether humeral neck shaft angle influences tuberosity healing in reverse total shoulder arthroplasty for proximal humerus fracture (PHF). This study aimed to compare the tuberosity healing rates between reverse total shoulder arthroplasty using a lateralized and a distalized configuration of the Univers Revers Total Shoulder System for PHFs. The secondary objective was to evaluate functional outcomes based on stem inclination and tuberosity healing status. METHODS: This randomized controlled trial enrolled patients ≥65 years with a Neer 3- or 4-part

Early mobilization after reverse total shoulder arthroplasty is not associated with increased risk of acromial and scapular stress fractures: a retrospective cohort study.

BACKGROUND: Acromial and scapular spine stress fractures (ASFs) are recognized complications following reverse total shoulder arthroplasty (rTSA) and may negatively affect clinical outcomes. The influence of post-operative immobilization duration and timing of rehabilitation on ASF risk remains unclear. This study evaluated whether a shorter post-operative sling immobilization protocol was associated with an increased incidence of ASF following primary rTSA. METHODS: A retrospective single-center comparative cohort study was performed including 698 patients who underwent primary rTSA between

Reverse total shoulder arthroplasty versus open reduction internal fixation for proximal humerus fractures in elderly patients: an age-stratified propensity score-matched analysis.

BACKGROUND: The optimal surgical management of displaced proximal humerus fractures in elderly patients remains controversial. Reverse total shoulder arthroplasty (rTSA) has gained increasing acceptance for complex fracture patterns, yet comparative data against open reduction and internal fixation (ORIF) across age strata are limited. This study compared reoperation rates, emergency department (ED) utilization, and dislocation following rTSA versus ORIF in propensity score-matched cohorts stratified by age. METHODS: Using the TriNetX multi-institutional research network, patients aged 65-74

[Translated article] Gender and diabetes mellitus do not influence the outcomes of patients with proximal humeral fractures treated with a reverse shoulder arthroplasty.

BACKGROUND AND OBJECTIVE: Some studies have described that gender and the diabetes mellitus may impact the outcomes of reverse shoulder arthroplasty (RSA). The objective of this study was to compare the functional outcomes and survival of RSA based on the patient's gender and the presence or absence of diabetes mellitus as independent variables. MATERIALS AND METHODS: This retrospective study included all patients who underwent RSA for acute proximal humeral fracture with a minimum follow-up of 2 years. Two independent stratifications were conducted to assess the influence of female gender an

Cemented vs. uncemented reverse total shoulder arthroplasty for the primary treatment of proximal humerus fractures in the elderly-a retrospective case-control study.

BACKGROUND: Uncemented reverse total shoulder arthroplasty (RTSA) for the primary treatment of proximal humerus fractures (PHF) in elderly patients was introduced at our institution in 2017. Recent reports have raised concerns about increased rates of early bone resorption at the proximal humerus with uncemented fracture stems. The aim of this study was to find out whether there was any difference in functional or radiographic outcomes between cemented and uncemented RTSA for PHF. METHODS: Seventeen consecutive patients who underwent uncemented RTSA (group nC) in 2017 and 2018 were age and se

Lower Geriatric Nutritional Risk Index is associated with increased risk of acromial stress fracture following reverse total shoulder arthroplasty.

BACKGROUND: Acromial and scapular spine stress fractures are serious complications following reverse total shoulder arthroplasty (rTSA), and patient-level risk factors may contribute to their development. The Geriatric Nutritional Risk Index (GNRI) is a validated marker of physiologic reserve. The purpose of this study was to evaluate the association between GNRI and post-operative stress fracture after rTSA. We hypothesized that lower GNRI would be associated with increased fracture risk. METHODS: We performed a retrospective 1:2 age- and sex-matched case-control study of patients undergoing
