The cost effectiveness of tranexamic acid for preventing periprosthetic joint infection following total shoulder arthroplasty: a break-even analysis.

BACKGROUND: The use of tranexamic acid (TXA) has become widespread in orthopedics to promote hemostasis and has been successfully used to reduce blood loss and infection risk in joint arthroplasty. However, the cost effectiveness of routine TXA use for the prevention of periprosthetic infections in total shoulder arthroplasty remains unknown. METHODS: The acquisition cost of TXA ($5.22) for our institution, along with values from the literature for the average cost of infection-related care ($55,243) and the baseline infection rates for patients without TXA use (0.70%),were used to perform a

Shoulder elevation and arm extension influence elbow joint loading during door-opening in total elbow arthroplasty: a musculoskeletal modelling study.

Total elbow arthroplasty (TEA) is used to treat end -stage elbow pathologies, yet implant failure is common due to excessive joint loading. This study compared elbow-joint loading, kinematics, and kinetics in TEA patients and healthy controls, and assessed the influence of elbow flexion-extension (FE) and shoulder elevation (SE). Seventeen participants (6 TEA, 11 controls) performed a door-opening task. Musculoskeletal modeling was used to estimate elbow kinematics, net joint moments FE and varus-valgus (VV)), and humeroulnar joint reaction forces (HUJRF). Statistical parametric mapping (SPM)

Arthroscopic lavage speeds reduction in effusion in the glenohumeral joint after primary anterior shoulder dislocation: a controlled randomized ultrasound study.

Recent studies have shown that arthroscopic lavage of the glenohumeral joint within 10 days following a primary anterior dislocation significantly lowers the recurrence rate when compared with a nonoperative regime. We hypothesize that the lavage reduces distension in the joint and thereby facilitates adaptation and healing of the soft tissue lesion. Using ultrasound, we assessed the hemarthrosis in the glenohumeral joint weekly in 16 consecutive patients after traumatic primary anterior shoulder dislocation. The patients were randomized into two groups for treatment with either arthroscopic l

[Quality control of prosthetic replacements of knee, ankle, toe, shoulder, elbow and finger joints in Norway 1994. A report after the first year of registration of joint prostheses in the national registry].

Total hip replacements have been recorded in the Norwegian Arthroplasty Register since 1987, and recording of the other joint arthroplasties was started in January 1994. After 12 months 1,589 primary arthroplasties had been registered; these referred to 962 knees, 11 ankles, 76 toes, 113 shoulders, 69 elbows, 12 wrists, 335 fingers and 11 carpometacarpal 1 joints. Median age of the patients was 70 years. 80% were women. Rheumatoid arthritis was the predominant reason for joint replacement, except in the case of the knees where the dominant cause was osteoarthrosis (71%) and of shoulders, 28% o

Preoperative Malnutrition and Risk of Periprosthetic Joint Infection After Reverse Total Shoulder Arthroplasty: Evaluation Using the Geriatric Nutritional Risk Index.

BACKGROUND: Poor nutritional status has been reported to adversely affect postoperative outcomes in orthopedic surgery; however, its role in reverse total shoulder arthroplasty (rTSA) has not been fully established. This study investigated whether pre-operative nutritional status, evaluated using the Geriatric Nutritional Risk Index (GNRI), was associated with postoperative complications and clinical outcomes after rTSA. METHODS: A retrospective analysis was conducted on 157 patients who underwent rTSA at the institution between 2014 and 2023, with a minimum follow-up of 2 years. Patients wer

Anatomic versus reverse total shoulder replacement with an intact rotator cuff: a comparative musculoskeletal simulation study of glenohumeral joint biomechanics.

BACKGROUND: Reverse total shoulder replacement (rTSR) is increasingly being conducted, including in selected elderly patients with intact-cuff osteoarthritis, due to concerns about subsequent cuff failure or glenoid component loosening after anatomic total shoulder replacement (aTSR). However, the biomechanical differences between rTSR and aTSR remain insufficiently understood. This computational study aimed to compare the biomechanics of aTSR and rTSR with an intact rotator cuff under identical conditions, representing selected intact-cuff osteoarthritis cases in which either implant design m

Extended oral antibiotic prophylaxis does not reduce periprosthetic joint infection after primary outpatient total shoulder arthroplasty: A matched cohort analysis.

BACKGROUND: Over the last decade, total shoulder arthroplasty (TSA) has increasingly transitioned to the outpatient setting, where postoperative care pathways inherently limit the ability to deliver a full 24 hour course of intravenous antibiotic prophylaxis. Despite guideline recommendations, extended oral antibiotic prophylaxis is variably used after outpatient TSA, with limited evidence supporting this practice. METHODS: A retrospective cohort study using the TriNetX database identified patients undergoing primary outpatient TSA between 2015 and 2025. Patients were stratified by receipt of

Data-driven body mass index threshold associated with increased risk of 2-year periprosthetic joint infection following total shoulder arthroplasty.

BACKGROUND: Body mass index (BMI) is a modifiable risk factor for medical and infectious complications following total shoulder arthroplasty (TSA). Previous studies investigating BMI were limited to the conventional classification system, which may be outdated for modern day patients. Therefore, the purpose of this study was to identify BMI thresholds that are associated with varying risk of 90-day medical complications and 2-year prosthetic joint infection (PJI) following TSA. METHODS: A national database was utilized to identify 10,901 patients who underwent primary elective TSA from 2013 t
