The Metabolic Pattern of Idiopathic REM Sleep Behavior Disorder Reflects Early-Stage Parkinson Disease.

Idiopathic REM sleep behavior disorder (iRBD) is considered a prodromal stage of Parkinson disease (PD) and other Lewy body disorders. Spatial covariance analysis of 18F-FDG PET data has disclosed a specific brain pattern of altered glucose metabolism in PD. In this study, we identify the metabolic pattern underlying iRBD and compare it with the known PD pattern. To understand the relevance of the iRBD pattern to disease progression, we studied the expression of the iRBD pattern in de novo PD patients. Methods: The iRBD-related pattern was identified in 18F-FDG PET scans of 21 patients with po

Presynaptic dopaminergic neuroimaging in REM sleep behavior disorder: A systematic review and meta-analysis.

The presence of polysomnography-confirmed REM sleep behavior disorder (RBD) is the stronger risk factor for having prodromal Parkinson disease (PD), followed by abnormal presynaptic dopaminergic radionuclide neuroimaging. Aim of the review is to conduct a meta-analysis of literature data regarding presynaptic dopaminergic neuroimaging in RBD. A literature search was conducted, resulting in 16 papers that met the inclusion criteria. Clinical and neuroimaging data were extracted. The studies are heterogeneous, especially for neuroimaging methodology. Two mathematical transformations were used to

REM sleep behavior disorder, autonomic dysfunction and synuclein-related neurodegeneration: where do we stand?

INTRODUCTION: From newfound parasomnia to a marker of future synucleinopathy, since its first description in 1986, REM sleep behavior disorder (RBD) has been systematically tackled from virtually many viewpoints in basic, translational, and clinical studies. The time delay between RBD and synucleinopathy onset offers an exceptional window for observation and design of neuroprotective trials. In the last few years, research has focused on characterizing possible differences within RBD patients in order to draw potential profiles more or less susceptible to further neurodegeneration. Nightmare sufferers show atypical emotional semantic associations and prolonged REM sleep-dependent emotional priming.

STUDY OBJECTIVES: The objective of this study was to investigate whether nightmare (NM) sufferers exhibit an abnormal network of emotional semantic associations as measured by a recently developed, rapid eye movement (REM) sleep-sensitive, associational breadth (AB) task. DESIGN: NM sufferers were compared to healthy controls (CTL) for their performance on an emotional AB task containing positive and negative cue words both before and after a nap with REM sleep. AB was assessed in both a priming condition, where cue words were explicitly memorized before sleep, and a non-priming condition, wh

REM sleep deprivation reverses neurochemical and other depressive-like alterations induced by olfactory bulbectomy.

There is compelling evidence that sleep deprivation (SD) is an effective strategy in promoting antidepressant effects in humans, whereas few studies were performed in relevant animal models of depression. Acute administration of antidepressants in humans and rats generates a quite similar effect, i.e., suppression of rapid eye movement (REM) sleep. Then, we decided to investigate the neurochemical alterations generated by a protocol of rapid eye movement sleep deprivation (REMSD) in the notably known animal model of depression induced by the bilateral olfactory bulbectomy (OBX). REMSD triggere

Abnormal occipital event-related potentials in Parkinson's disease with concomitant REM sleep behavior disorder.

BACKGROUND: Rapid eye movement sleep behavior disorder is found in 33-46% of patients with Parkinson's disease and was shown to be associated with cognitive deficits. Our goal was to improve our understanding of the role of this sleep disorder in cerebral dysfunction occurring in Parkinson's disease using a visual cognitive task and event-related potentials. METHODS: Sixteen patients with Parkinson's disease and rapid eye movement sleep behavior disorder, 15 patients with Parkinson's disease without rapid eye movement sleep behavior disorder and 16 healthy control subjects were included. Post-learning REM sleep deprivation impairs long-term memory: reversal by acute nicotine treatment.

Rapid eye movement sleep deprivation (REM-SD) is associated with spatial learning and memory impairment. During REM-SD, an increase in nicotine consumption among habitual smokers and initiation of tobacco use by non-smokers have been reported. We have shown recently that nicotine treatment prevented learning and memory impairments associated with REM-SD. We now report the interactive effects of post-learning REM-SD and/or nicotine. The animals were first trained on the radial arm water maze (RAWM) task, then they were REM-sleep deprived using the modified multiple platform paradigm for 24h. Du

Relationship between (123)I-MIBG scintigrams and REM sleep behavior disorder in Parkinson's disease.

BACKGROUND: Uptake of (123)I-labeled meta-iodobenzylguanidine (MIBG) in myocardial scintigrams has been shown to be as low in patients with idiopathic RBD as in Parkinson's disease (PD) patients. AIM FOR STUDY: To clarify whether the existence of RBD accelerates autonomic dysfunction in PD, we investigated the association between MIBG scintigraphic findings and RBD measures among non-dementia PD patients. SUBJECTS & METHODS: We conducted clinical interviews to assess REM sleep behavior disorder (RBD) symptoms, and performed polysomnograms (PSG) recordings and MIBG scintigrams on 49 PD patient

Relationship between visual hallucinations and REM sleep behavior disorder in patients with Parkinson's disease.

OBJECTIVES: REM sleep behavior disorder (RBD) has been documented to precede or to co-occur with Parkinson's disease (PD). Parkinson's disease is one of the most common neurological conditions associated with visual hallucinations. Cognitive dysfunction is present in PD, even at the early stages of these diseases. In this study we aimed to investigate the relationship between visual hallucinations and RBD in patients with idiopathic Parkinson's disease (IPD). Additionally, we evaluated the association of the cognition and the pattern of cognitive impairment with VHs and RBD, effects of factors

Instrumental REM sleep deprivation in neonates leads to adult depression-like behaviors in rats.

STUDY OBJECTIVES: Previous studies have demonstrated that neonatal suppression of rapid eye movement (REM) sleep by pharmacologic agents, particularly clomipramine, produces adult depressive behavior. These findings suggest the hypothesis that REM sleep deprivation (RSD) mediates the depressogenic behaviors of neonatally administered antidepressant drugs. Drug suppression of RSD, however, was thought to be confounded by the other effects of the drugs. The current study was aimed to show the adult effect of neonatal RSD in rats by instrumental means, ie, a computer-controlled shaking method. D

Association of REM sleep behavior disorder and neurodegenerative disease may reflect an underlying synucleinopathy.

Our objective was to examine whether rapid eye movement (REM) sleep behavior disorder occurs in disproportionally greater frequency in multiple system atrophy (MSA), Parkinson's disease (PD), and dementia with Lewy bodies (DLB), collectively known as the synucleinopathies, compared to other nonsynucleinopathy neurodegenerative disorders. In study 1, we reviewed the clinical records of 398 consecutive patients evaluated at Mayo Clinic Rochester for parkinsonism and/or cognitive impairment. The frequency of suspected and polysomnogram (PSG)-confirmed REM sleep behavior disorder (RBD) among subje

Anesthesia with abdominal surgery leads to intense REM sleep during the first postoperative week.

Characteristics of nocturnal sleep were investigated in six patients after anesthesia and cholecystectomy and in another six after anesthesia and gastroplasty. All night polysomnographic recordings were obtained while each patient slept in a private surgical ward room through two nights before and five or six nights after operation. Anesthesia included thiopental, N2O, isoflurane, and fentanyl. Postoperative analgesia was provided with parenteral morphine. Other aspects of care were routine. Nocturnal sleep was markedly disturbed after both surgical procedures. Throughout the operative night a

Detecting isolated REM sleep behavior disorder at home using a lower-back wearable sensor.

Isolated REM Sleep Behavior Disorder (iRBD) is a strong predictor of neurodegenerative diseases, particularly synucleinopathies. Current diagnosis requires overnight video-polysomnography (vPSG) in sleep laboratories. Limited access to vPSG and differences in sleep habits result in diagnostic challenges. Here we aimed to evaluate the feasibility of identifying iRBD from a lumbar-mounted wearable sensor in the home setting and explored night-to-night variability. Seventy-three participants (15 iRBD, 58 controls) underwent vPSG, followed by six nights of wearing a lower-back inertial measurement

Dynamic thalamocortical functional connectivity disruptions in Parkinson's disease with probable REM sleep behavior disorder.

As the symptom of rapid eye movement sleep behavior disorder (RBD) is paroxysmal during sleeping, the disruptions of dynamic functional connectivity (FC) between thalamic subnuclei and the cortex may play a critical role in Parkinson's disease (PD). A total of 35 PD patients with probable RBD (PD-pRBD) and 40 PD patients without probable RBD (PD-npRBD) and 41 healthy controls were enrolled. All participants underwent functional magnetic resonance imaging scan and clinical assessment. Altered dynamic FC between bilateral 14 thalamic nuclei and cortex was calculated. Although the RBDSQ demonstra

Physiological Signatures Derived from Wrist Accelerometry Improve Detection of Isolated REM Sleep Behavior Disorder.

Isolated REM sleep behavior disorder (iRBD) is the strongest prodromal marker of α-synucleinopathies, but polysomnography is impractical for population-scale screening and questionnaires lack sufficient specificity. Beyond nocturnal movement, recently published models can derive sleep staging and other physiological signatures from raw high-resolution wrist accelerometry alone. Across four cohorts (366 subjects: 95 iRBD, 271 controls; 5,804 nights), we extracted 876 features spanning sleep macrostructure, stage-probability dynamics, motor activity, and cardiorespiratory variation. Acetazolamide to prevent ventilatory drive withdrawal in REM sleep apnoea: a randomised controlled trial.

BACKGROUND: Obstructive sleep apnoea (OSA) pathogenesis during rapid-eye movement (REM) sleep has been linked to dips in ventilatory drive and downstream genioglossus hypotonia. The carbonic anhydrase inhibitor acetazolamide is known to increase ventilatory drive and improve OSA severity. Therefore, we tested the effect of acetazolamide on REM-predominant OSA severity (apnoea hypopnoea index (AHI) and hypoxic burden, co-primary outcomes) and underlying physiological mechanisms (ventilatory drive, ventilation and pharyngeal muscle activity). METHODS: 11 participants with REM-predominant OSA pe

Probable REM sleep behavior disorder marks a less levodopa-responsive phenotype in Parkinson's disease.

BACKGROUND: Probable REM sleep behavior disorder (pRBD) is a clinically relevant phenotype in Parkinson's disease (PD), commonly associated with greater non-motor burden and a more diffuse disease profile. However, its independent clinical correlates remain incompletely defined, particularly regarding acute dopaminergic responsiveness. OBJECTIVE: To identify independent clinical correlates of pRBD in a comprehensively phenotyped PD cohort, with particular attention to acute levodopa responsiveness. METHODS: In this retrospective cross-sectional study, 468 patients with PD underwent multidime
