Peer pressures: social instability stress in adolescence and social deficits in adulthood in a rodent model.

Studies in animal models generate and test hypotheses regarding developmental stage-specific vulnerability that might inform research questions about human development. In both rats and humans, peer relationships are qualitatively different in adolescence than at other stages of development, and social experiences in adolescence are considered important determinants of adult social function. This review describes our adolescent rat social instability stress model and the long-lasting effects social instability has on social behaviour in adulthood as well as the possible neural underpinnings. Visceral leishmaniasis and vulnerability conditions in an endemic urban area of Northeastern Brazil.

BACKGROUND: Visceral leishmaniasis (VL) is considered an urban endemic that may be related to sociodemographic factors and urban infrastructure. The objective of this study was to analyze the conditions of vulnerability and the incidence of VL, and to identify the spatiotemporal pattern of the disease. METHODS: Data on reported cases of VL were collected from 2007 to 2016. Thematic maps on the spatiotemporal distribution were produced using values normalized by the area of the neighborhoods. The primary indicators of vulnerability were normalized for later construction of the synthetic indica

Reduced medial prefrontal responses to social interaction images in remitted depression.

CONTEXT: Major depressive disorder is associated with impairments in processing emotional stimuli, and residual impairments are observed during remission, possibly indicating trait vulnerability. Stimuli with social context represent a distinct class of emotional stimuli, which in healthy volunteers are associated with specific neural substrates but have not previously been studied relative to vulnerability to depression. OBJECTIVE: To explore whether individuals with remitted major depressive disorder had altered neuronal processing of social emotional stimuli. DESIGN: Cross-sectional desig

Community-Level Disparities in the Inhalation Injury Patient Population.

Low-income neighborhoods face higher fire risk due to overcrowding, substandard housing, and limited access to safety devices, contributing to greater burn severity among affected residents. Inhalation injury, a frequent consequence, often necessitates early intubation and transfer to burn centers. Given the high degree of coordination required, this study examines how management strategies and outcomes vary across levels of social vulnerability and urbanicity, 2 community-level proxies for access to care. We retrospectively reviewed inhalation injuries diagnosed by bronchoscopy at a single AB…

Ecological and Syndemic Predictors of Drug Use During Sex and Transactional Sex among U.S. Black Men Who Have Sex with Men: A Secondary Data Analysis from the HPTN 061 Study.

Threats to sexual health among U.S. Black men who have sex with men (MSM) may manifest in a context of social adverse experiences. Situational sex is one such context, which we characterize as sexual behaviors driven either by a desire to cultivate a specific sexual experience or attributable to social vulnerability. Two characterizations of situational sex explored in this study were drug use during sex and transactional sex. Guided by ecological and syndemic frameworks, we conducted a secondary data analysis of social conditions and sexual behaviors among a prospective cohort of Black MSM fr…

Physical vulnerability of buildings to flooding in Lilongwe City, Malawi.

Research on flood vulnerability has mainly focussed on social, economic and human vulnerability and the few studies that have attempted to analyse the physical vulnerability of buildings to natural hazards (seismicity and floods) have been done at the subnational spatial scale resulting in generalised vulnerability outcomes. Additionally, most of the studies used models to analyse vulnerability which are known for uncertainties in the results. This study investigated the physical vulnerability of buildings to flooding in low-income settlements of Biwi and Kawale 1 in Malawi's capital city, Lil

Risk-Taking, Social Support, and Belongingness Contribute to the Risk for Cannabis Use Frequency in University Students.

Background: Cannabis use and misuse is known to be associated with a variety of negative health, academic, and work-related outcomes; therefore, it is important to study the factors that contribute to or moderate its use. Objectives: The aim of this study was to determine whether risky behavior, belongingness and social support as clustering variables play a role in cannabis use frequency. Method: In a university student sample, participant data on risky behavior, belongingness and social support were used to generate vulnerability profiles through cluster analysis (low vulnerability with low

Reach and engagement in the ECAIL trial targeting socially disadvantaged families across the COVID-19 pandemic and cost-of-living crisis: A mixed-methods implementation study.

BACKGROUND: The ECAIL trial, launched in 2017, was designed to evaluate a multicomponent intervention for childhood obesity prevention among hard-to-reach families. At a maternity hospital in Lille, France, healthcare providers screened pregnant women experiencing social vulnerability, and dietitians delivered a home-based intervention until age 2. The COVID-19 pandemic led to a six-month suspension in 2020. This study compared eligibility and participation before the pandemic and after resumption, and examined how implementation and reach evolved across the pandemic and subsequent cost-of-liv…

Testicular torsion disparities in the pediatric population.

BACKGROUND: There are disparate findings in the literature on the impact of race and insurance status on gonadal loss in testicular torsion. We sought to determine if race or levels of social vulnerability influence the rate of torsion or gonadal loss. METHODS: Retrospective cross-sectional review between December 2017 and September 2019. Social vulnerability index was dichotomized using the 75th percentile. Primary outcome was the diagnosis of testicular torsion. RESULTS: 515 patients were included. There was no difference in median age, torsion diagnosis, and orchiectomy rate between the t…

Multidomain Vulnerability and Fear of Falling in Mexican American Older Adults: A Repeated Measures Analysis.

OBJECTIVES: To examine how multidomain vulnerability relates to fear of falling among Mexican American older adults across repeated survey waves. METHODS: This was a secondary analysis of three waves (2012-2021) of the H-EPESE study with 893 person-wave observations. Fear of falling was the ordinal outcome. A vulnerability index combined four indicators: mobility impairment, depressive symptoms, loneliness, and urinary incontinence. Ordinal GEE models adjusted for age, sex, cognitive status, and survey wave. RESULTS: Multidomain vulnerability was strongly associated with fear of falling. Com

Decline in Cancer Screening in Vulnerable Populations? Results of the EDIFICE Surveys.

BACKGROUND: We studied cancer screening over time and social vulnerability via surveys of representative populations. METHODS: Individuals aged 50-75 years with no personal history of cancer were questioned about lifetime participation in screening tests, compliance (adherence to recommended intervals [colorectal, breast and cervical cancer]) and opportunistic screening (prostate and lung cancer). RESULTS: The proportion of vulnerable/non-vulnerable individuals remained stable between 2011 and 2016. In 2011, social vulnerability had no impact on screening participation, nor on compliance. In…

Communities' perceptions of factors contributing to child sexual abuse vulnerability in Kenya: a qualitative study.

Child sexual abuse (CSA) is a major global health concern. Although it is prevalent in Kenya, scant literature on factors contributing to CSA vulnerability exists. Using qualitative data from 28 focus groups and ethnographic field notes, we explored and assessed community perceptions of factors contributing to CSA vulnerability in Homa Bay County, Western Kenya. Findings suggest that people living in these communities perceived CSA as being influenced by multiple factors: developmental stage, peer pressure, huge gender disparities exacerbated by negative social norms and cultural practices, th

Social determinants of mortality due to visceral leishmaniasis in Brazil (2001-2015): an ecological study.

INTRODUCTION: We aimed to analyze the relationship between visceral leishmaniasis mortality and social determinants of health (SDH). METHODS: This was an ecological study of all leishmaniasis-related deaths in Brazil, from 2001 to 2015. We analyzed 49 indicators of human development and social vulnerability. The association was tested using the classical and spatial regression model. RESULTS: Mortality was associated with indicators that expressed low human development and high social vulnerability: lack of garbage collection, low schooling, unemployment rate, low per capita income, and inco…

Understanding Regional Patterns of Overdose Deaths Related to Opioids and Psychostimulants.

BACKGROUND: As overdose rates increase for multiple substances, policymakers need to identify geographic patterns of substance-specific deaths. In this study, we describe county-level opioid and psychostimulant overdose patterns and how they correlate with county-level social vulnerability measures. METHODS: A cross-sectional observational study, we used nationwide 2016-2018 restricted access Centers for Disease Prevention and Control county-level mortality files for 1,024 counties. We estimated quartiles of opioid and psychostimulant overdose mortality and provided estimates of their associa…

Rural and urban environmental exposures and the risk of progressive pulmonary fibrosis.

BACKGROUND: Non-occupational exposures are increasingly recognized as a driver of poor outcomes in patients with interstitial lung disease (ILD), including urban and rural populations. However, both populations also experience barriers to care and higher social vulnerability that may be associated with higher risk for progression. The impact of agricultural antigen exposure and concomitant social vulnerability has not been well characterized. OBJECTIVES: To evaluate the risk of progression in both urban and agricultural antigen-exposed communities in patients with ILD. DESIGN: Retrospective …

Ebola, informal settlements, and the role of place in infectious disease vulnerability: evidence from the 2014-16 outbreak in urban Sierra Leone.

Studies of vulnerability often focus on the differential susceptibility of marginalised groups to the effects of disaster. This paper considers how vulnerability is also associated with the characteristics of place, especially the social setting of the informal settlement. In this light, it assesses specifically how cultural, historical, and political economic forces resulted in increased vulnerability to Ebola virus disease (EVD) within informal settlements in Sierra Leone during the epidemic of 2014-16. Key informant and community member interviews and focus-group discussions in two communit

Do socioeconomic health gradients persist over time and beyond income? A distributional analysis using UK biomarker data.

This paper analyses the relationship between health and socioeconomic disadvantage by adopting a dynamic approach accounting for spatial and temporal changes across ten domains including social isolation, environment, financial hardship and security. As a first step we develop a measure of overall multidimensional deprivation and undertake a decomposition analysis to explore the role of breadth and duration of deprivation on shaping the deprivation gradient in health. Subsequently, we employ unconditional quantile regression to conduct a distributional analysis of the gradient to understand ho

Beyond 'vulnerable groups': contexts and dynamics of vulnerability.

This paper reviews approaches to vulnerability in public health, introducing a series of 10 papers addressing vulnerability in health in Africa. We understand vulnerability as simultaneously a condition and a process. Social inequalities are manifest in and exacerbate three key dimensions of vulnerability: the initial level of wellbeing, the degree of exposure to risk, and the capacity to manage risk effectively. We stress the dynamic interactions linking material and social deprivation, poverty, powerlessness and ill health: risks or shocks and their health impacts are intimately interconnect
