Actinomyces turicensis Necrotizing Soft-Tissue Infection of the Thigh in a Diabetic Male.

Background: Necrotizing soft-tissue infections are a devastating infection that is rarely caused by Actinomyces spp. Case Report: A 45-year-old obese previously healthy male presented to the emergency department with diabetic ketoacidosis. The patient developed systemic signs of infections and right medial thigh pain subsequently diagnosed as a necrotizing soft-tissue infection. Successful treatment included prompt surgical intervention and initiation of broad-spectrum antimicrobial drugs. Conclusion: Actinomyces turicensis may be the pathogen causing certain necrotizing soft-tissue infections

Necrotizing Myositis: A Rare Necrotizing Soft Tissue Infection Involving Muscle.

INTRODUCTION: Necrotizing myositis (NM) is an extremely rare necrotizing soft tissue infection involving muscle. Unlike similar infections (eg, necrotizing fasciitis, clostridial myonecrosis) that can be more readily diagnosed, NM can have a benign presentation then rapidly progress into a life-threatening condition with a mortality rate of 100% without surgical intervention. CASE REPORT: A 74-year-old man with a history of prostate cancer with radiation therapy, seed implants, and 2 transurethral resection procedures presented to the emergency department after a fall. He was initially diagno

Antibiotics for skin and soft tissues infections in type 2 diabetes mellitus.

BACKGROUND: Type 2 diabetes mellitus is a chronic pathology characterized by high prevalence, high morbidity and mortality. According to the data of the Ministry of Health of Volgograd region the number of patients with type 2 diabetes was 68,227 people on 01.01.2014. Medical and social significance of type 2 diabetes mellitus is determined by its complications. Skin and soft tissue infections (SSTIs) in patients with type 2 diabetes are among the main factors of hospitalization and mortality [1]. Diabetic foot syndrome is found in 30-80% of patients [2]. OBJECTIVE: Pharmacoepidemiological an

Flaps for closure of soft tissue defects in infected revision knee arthroplasty.

PURPOSE: Total knee arthroplasty revision has wound healing deficits of up to 20 %. Defects in the knee region of multimorbid patients are hard to treat as complete explantation and revision arthroplasty is often too burdensome for them. In this study, we present our results with flaps for the treatment of defects after knee replacement, arthrodesis or osteosynthesis. METHODS: Twenty-five patients (26 knees) with defects in the knee region were treated with flaps. Mean follow-up was 37 months (13-61) and the patients had a mean age of 72 years (49-85). A total of 39 flaps were performed (27 m

Emergency ultrasonography in patients with clinically suspected soft tissue infection of the legs.

INTRODUCTION: We aimed to retrospectively review the ultrasonography (US) findings of patients with clinically suspected soft tissue infection of the legs and to determine whether there is a correlation between US diagnosis and further clinical management. METHODS: We reviewed the US findings of consecutive patients with clinically suspected soft tissue infection of the legs who were referred for emergency US during a consecutive two-year period. The indications for US were recorded and the findings evaluated. The effect of the US findings on further clinical management (medication alone vers

Necrotizing soft tissue infection of the breast: case report and literature review.

BACKGROUND: Necrotizing soft tissue infection (NSTI) is characterized by progressive infectious gangrene of the skin and subcutaneous tissue. Its treatment involves intensive care, broad-spectrum antibiotic therapy, and full debridement. METHODS: We present two cases of NSTI of the breast, adding these cases to the 14 described in the literature, reviewing the characteristics and evolution of all cases. CASE REPORT: On the fourth day after mastectomy, a 59-year-old woman with ulcerated breast cancer developed Type I NSTI caused by Pseudomonas aeruginosa, which had a favorable evolution after

[Rapid infection of bone and soft tissues of the upper extremity--case report].

Long duration of intravenous drug abuse brings the risk of infectious and surgical complications. The case report of the forearm soft tissue and bone infection followed by rapid progress of compartment syndrome is described. Primary bone infection is unusual. It is necessary to think about it if the treatment of soft tissues comes short of the inflammation suppression. Especially if an intravenous drug abuser is concerned. The case stresses the necessity of thinking of all the diagnostic possibilities and bone affliction with its risks and of the well-timed treatment urgency.

[Case of Scedosporium apiospermum cutaneous soft tissue infection treated with voriconazole].

A 69-year-old man treated with corticosteroids and immunosuppressive agents for acutely exacerbated interstitial pneumonia was found to have an ingrown nail in the left big toe and that suppurated despite treatment by dermatologists. Culture of the pus expressed from the toe yielded Scedosporium apiospermum. The patient suffered liver dysfunction a few days later when treated with intravenous voriconazole (VRCZ), which was discontinued due to the high plasma VRCZ concentration. Discrete erythema and subcutaneous nodules developed in left leg 2 or 3 weeks later. Ultrasonography showed tubular s

Infection following soft tissue injury: its role in wound healing.

PURPOSE OF REVIEW: Wound infection has always been an unconquered problem temporarily improved with the discovery of antibiotics but now struggling with an epidemic of resistant organisms. Wound healing has become a popular sub-speciality for the doctor and allied health professional working in the laboratory or at the bedside. It is a field with many new journals and frequent congresses that publish elaborate proceedings. These have been examined for this review. RECENT FINDINGS: Measurement of infection remains elusive. A clear difference between contamination and infection is difficult to

Coccidioidal pneumonia, arthritis, and soft-tissue infection after kidney transplantation.

A 60-year-old male kidney transplant recipient presented with pneumonia 4 months after transplantation. Coccidioidomycosis was suspected and empirically treated with fluconazole. He was maintained on suppressive fluconazole without problems. Three weeks after discontinuing secondary prophylaxis, the patient experienced coccidioidal arthritis and an infection of the soft tissue of the hand that required debridement. Transplant recipients may have quiescent disseminated coccidioidomycosis that is reactivated by immunosuppression after withdrawal of suppressive antifungal therapy.

Soft-tissue infection due to Mycobacterium smegmatis: report of two cases.

Mycobacterium smegmatis is an uncommon pathogen in humans. Fourteen cases of skin or soft-tissue infection due to M. smegmatis have been previously reported. We report two cases of posttraumatic M. smegmatis infection of the lower extremity. M. smegmatis infection produces chronic cellulitis with fistula formation that is most commonly a result of direct traumatic inoculation of contaminated material. Extensive surgical debridement followed by skin grafting has been necessary for cure in the majority of cases.

[Clinical studies on cefprozil granules in pediatric skin soft tissues infections].

Cefprozil (CFPZ), a newly developed cephalosporin in fine granular form, was administered to pediatric patients with skin and soft tissue infections. MICs were determined for 6 drugs including CFPZ, cephalexin (CEX), cefaclor (CCL), ampicillin (ABPC), methicillin (DMPPC), cloxacillin (MCIPC) against 53 clinical isolates of Staphylococcus aureus from these patients. An inoculum size of 10(6) CFU/ml was used in the MIC-determinations. CFPZ was given to 73 patients with ages ranging from 6 months to 10 years and 8 months and 71 cases were evaluable for clinical effects as follows; impetigo (65),

Improving Empiric Antibiotic Selection for Patients Hospitalized With Skin and Soft Tissue Infection: The INSPIRE 3 Skin and Soft Tissue Randomized Clinical Trial.

IMPORTANCE: Empiric extended-spectrum antibiotics are routinely prescribed for patients hospitalized with skin and soft tissue infections (SSTIs) despite low likelihoods of infection with multidrug-resistant organisms (MDROs). OBJECTIVE: To evaluate whether computerized provider order entry (CPOE) prompts presenting patient-specific and pathogen-specific MDRO infection risk estimates could reduce empiric extended-spectrum antibiotics for noncritically ill patients admitted with SSTI. DESIGN, SETTING, AND PARTICIPANTS: This cluster randomized clinical trial included 92 hospitals and assessed

Fire needle therapy for recurrent skin and soft tissue infections: a case report and literature review.

BACKGROUND: Recurrent skin and soft tissue infections (RSSTIs) are challenging infectious diseases with a clinical incidence ranging from 7% to 45%, imposing sustained physical and psychological burdens and significant economic pressure on healthcare systems. For purulent infections, current Western medicine primarily relies on antibiotics and incision and drainage. Still, it faces limitations including increasing antibiotic resistance and surgical trauma, which further elevate the risk of RSSTIs. Fire needle therapy, as an external treatment in Traditional Chinese Medicine, may improve local

Impact of Statin Therapy on Clinical Outcomes of Patients Hospitalised with Skin and Soft Tissue Infections.

Background: Statins have anti-inflammatory properties beyond their lipid-lowering effects, but their impact on skin and soft tissue infections (SSTIs) remains unclear. This study evaluated whether statins improve clinical outcomes in patients hospitalised with SSTIs. Methods: Adults aged ≥18 years hospitalised with SSTIs at a tertiary hospital in Australia between 1 June 2021 and 31 December 2021 were identified using the International Classification of Diseases 10th Revision Australian Modification (ICD-10-AM) codes. Patients were categorised into two groups based on statin use at admission.
