A meme's-eye view of nonspeech oral-motor exercises.

The ideas motivating the use of nonspeech oral motor exercises (NSOMEs) cluster into three memeplexes that reflect the rich history of oral motor and nonspeech activities in speech-language pathology; a bottom-up, discrete skill theory of learning; and common treatment practices. The lack of clinical guidance provided by research also plays a role in the use of NSOMEs. The essence of the oral motor memeplex is the history of oral motor activities in speech-language pathology and the often detailed coverage these activities receive in the most widely read textbooks and publications in our profe…

Contributions to Gain in Speech Sound Production Accuracy for Children With Speech Sound Disorders: Exploring Child and Therapy Factors.

Purpose The purpose of this study was to explore the extent to which child- and therapy-level factors contribute to gains in speech sound production accuracy for children with speech sound disorders in receipt of school-based services. Method Data were obtained from 126 kindergarten and first- and second-grade children currently in receipt of speech therapy services in their public school setting. Pretest and posttest measures of spontaneous speech production and language ability were collected at the beginning and end of one academic year. Using a spontaneous speech sample, percentage of cons

Gelastic dysarthria: Speech-triggered pathological laughter with evidence for a selective pontine gating mechanism.

We report a highly selective manifestation of pathological laughter in which speech initiation consistently triggers laughter, resulting in disrupted articulation. A 55-year-old male with bilateral pontine infarction exhibited laughter that was time-locked to speech, most prominent during speech production and absent during non-speech tasks. Functional MRI during speech demonstrated activation of language regions, medial prefrontal cortex, amygdala, and ventral pons, with concurrent subjective laughter. We propose that ventral pontine lesions disrupt inhibitory pathways, leading to disinhibiti

Person-centered care and speech and language therapy.

Person-centered care (PCC) has become the foundation for practice in many areas of health care provision. Research has suggested that providing PCC may improve therapy outcomes, client satisfaction, and perceived quality of care, as well as address aspects of evidence-based practice. As members of the health care provider community, speech-language pathologists (SLPs) will be expected to provide PCC as part of their therapeutic interactions. Some research, however, has indicated that SLPs have a tendency to provide more structured, task-oriented therapies. In this article, a working definition

Waiting for child developmental and rehabilitation services: an overview of issues and needs.

Concern about the length of time that children, young people, and families may have to wait to access assessment, diagnostic, interventional, therapeutic, and supportive child developmental and rehabilitation (CDR) services is widespread, but adequate data collection and research on this issue remain limited. We review key concepts and issues relevant to waiting for CDR services from the published literature, a national workshop devoted to this topic, and international experience. We conclude that gaps in data, evidence, and consensus challenge our ability to address the issue of waiting for C

Development and validation of a screening procedure to identify speech-language delay in toddlers with cleft palate.

The purpose of this study was to develop and validate a clinically useful speech-language screening procedure for young children with cleft palate ± cleft lip (CP) to identify those in need of speech-language intervention. Twenty-two children with CP were assigned to a +/- need for intervention conditions based on assessment of consonant inventory using a real-time listening procedure in combination with parent-reported expressive vocabulary. These measures allowed evaluation of early speech-language skills found to correlate significantly with later speech-language performance in longitudinal

Addressing Stuttering Variability in Assessment and Treatment: Perspectives of Speech-Language Pathologists.

PURPOSE: This study investigated how speech-language pathologists (SLPs) observe and address variability in stuttering assessment and treatment. Variability was examined in observable overt stuttering behaviors and beyond, encompassing the emotions, beliefs, and behaviors associated with stuttering, as well as speakers' ability to engage in daily activities and participate fully in their lives. The overarching goal of the study was to identify strategies for ensuring that variability is appropriately accounted for in clinical work related to stuttering. METHOD: The study employed a mixed-meth

A Framework for Measuring Treatment Fidelity in Motor-Based Speech Intervention for Children With Childhood Apraxia of Speech.

PURPOSE: Examining adherence to a treatment protocol is a critical component of intervention research. This is particularly challenging in treatment involving young children with childhood apraxia of speech (CAS) where treatment is provided in a dynamic manner. The purpose of this work was to first present a framework for measuring fidelity of Dynamic Temporal and Tactile Cueing (DTTC), a motor-based treatment designed for children with CAS. This framework includes the DTTC Fidelity Checklist and a collaborative review process between investigators and treating speech-language pathologists (SL

Electropalatography and the Linguagraph system.

This paper describes the technique of electropalatography and the development of Linguagraph, which is a user-friendly, clinical instrument, for measurement of tongue/palate contact, during speech. Linguagraph allows objective assessment of tongue function; appropriate targeting of therapy is therefore possible. Visual feedback is also provided, for therapy, and an objective measurement of outcome is easily obtained. Linguagraph was used, for both therapy and assessment, in a clinical trial. Technical aspects of Linguagraph and of the trial results are presented here. These suggest that the in

Breaking down the ear-brain dichotomy: the effects of age-related hearing loss on the cortical language system.

Older individuals frequently experience hearing difficulties, often due to sensorineural hearing loss (HL) primarily originating in the inner ear. However, it is not uncommon for older adults with HL to also exhibit impairments in speech intelligibility and comprehension, and seemingly paradoxically, similar circumstances are often observed in older adults without observable hearing pathology. In this review article, we address the fallacy of a strict ear-brain dichotomy in the context of HL and provide empirical evidence that a one-sided perspective that posits that speech processing deficits
