BACKGROUND: To reduce complications associated with conventional pterional craniotomy, a transsylvian keyhole approach for unruptured small anterior circulation aneurysms is proposed.
METHODS: A 7-cm linear scalp incision is made along the hairline, beginning at the zygoma, followed by minimal temporal muscle dissection. Two burr holes are drilled out at McCarty's point and the temporal bone, and a 3-cm equilateral triangle bone flap is made, whose apex is located above the sylvian point. After the sphenoid ridge is drilled off, aneurysms are exposed and clipped with conventional microsurgica

A 15-year-old female patient with progressive pulsatile exophthalmos caused by intraorbital encephalocele was evaluated with computed tomography (CT) and magnetic resonance imaging (MRI) in our clinic. She had no history of trauma or reconstructive surgery. When she was a little girl, she had undergone surgery for congenital glaucoma on the right eye. On the three-dimensional image of CT, a hypoplasic bone defect was observed in the greater wing of the right sphenoid bone. MRI and CT scan showed herniation through this defect of the arachnoid membrane and protruded cerebral tissue into the rig…

Two children are presented with bilateral choanal atresia: a girl with the typical features of the Treacher Collins syndrome, and a boy, a third cousin of the girl, without mandibulo-facial dysostosis. Simultaneous occurrence of choanal atresia and the Treacher Collins syndrome may be based on a defective ossification of the mesenchyme of the first visceral arch. Choanal atresia may be due to nonabsorption of the bucconasal membrane and/or extension of the sphenoid or palate bone.

Paediatric patients with acute myeloid leukaemia (AML) often present with symptoms associated with the disruption of normal haematopoiesis and subsequent cellular deficiencies. Periosteal reactions are common in paediatric leukaemia, but typically manifest as a thin, laminated pattern along long bones. Aggressive periosteal reactions are much less frequently seen. Here, we report a case of paediatric AML initially presenting with proptosis and periorbital swelling caused by aggressive, sunburst periosteal reactions surrounding the sphenoid and zygomatic bones. This unique presentation emphasis

Primary calvarial or intra-osseous meningiomas are uncommon lesions often confused preoperatively with other primary or secondary bone tumors of the skull. We present details of a patient with a primary calvarial meningioma involving the frontal, zygomatic, sphenoid and temporal bones that was radiographically diagnosed as fibrous dysplasia and initially treated conservatively for many years. Progressive vision loss and significant proptosis, with difficulty of eye closure, prompted surgical removal with cranioplasty and a 3D reconstructed prosthetic flap. Histological examination revealed int

The morphology of two East Indian dry skulls exhibiting anomalies which were suggested to represent incomplete forms of mandibulofacial dysostosis is described. Obvious although minor ossification anomalies were found localized to the temporal, sphenoid, the zygomatic, the maxillary and the mandibular bones. The observations substantiate the concept of the regional and bilateral nature of the malformation syndrome. Bilateral orbital deviations, hypoplasia of the malar bones, and incomplete zygomatic arches appear to be hard tissue aberrations which may be helpful in examination for subclinical

The falx cerebri (FC) corresponds to the invagination of the meningeal dural layer into the longitudinal fissure between the two cerebral hemispheres. Infrequently, the FC ossifies, forming an extra bone deposition, giving the impression of "supernumerary" bone. The present study aimed to investigate the presence of FC ossification in a large sample of the Greek population. Falx cerebri ossification was identified in 18 patients (18/669, 2.69%). Thirteen cases (13/669, 1.94%) presented with a single bone deposition (single focus) due to the FC ossification. Moreover, 5 cases (5/669, 0.75%) had

Sphenoorbital meningiomas (SOMs) are slow-growing tumors that originate from the sphenoidal wing and are associated with visual deterioration, extrinsic ocular movement disorders, and proptosis caused by hyperostosis of the lateral wall of the orbit. In some cases, the intracranial component is quite small or "en plaque," and the majority of the symptoms arise from adjacent hyperostosis. Craniotomy has traditionally been the standard of care, but new minimally invasive multiportal endoscopic approaches offer an alternative. In the current study, the authors to present their experience with the

BACKGROUND: The infraorbital foramen (IOF) is present on the maxilla under the infraorbital margin. Its identification is essential in various surgical procedures. The main aim of this study was the morphometric assessment of the position of the right and left infraorbital foramina in relation to specific structural elements of the facial skeleton, their width and direction, and also the determination of the location of these foramina above maxillary teeth in examined male skulls (belonging to European populations) dated to the beginning of the 20th century and the medieval and post-medieval p

BACKGROUND: Percutaneous trigeminal rhizotomy (PTR) is a widely used procedure for trigeminal neuralgia. However, comprehensive analyses that combine anatomic, radiological, and surgical considerations are rare.
OBJECTIVE: To present high-quality anatomic dissections and radiological studies that highlight the technical nuances of this procedure.
METHODS: Six silicon-injected postmortem heads underwent PTR. The surgical corridors were dissected, and the neurovascular relationships were studied. In addition, 20 dried human skulls and 50 computed tomography angiography and MRI scans were colle

BACKGROUND: The spina musculi recti lateralis (SMRL) is often visible along the lateral rim of the superior orbital fissure (SOF). Aim of this study is to characterize SMRL morphology and topography relative to known bony landmarks.
METHODS: Orbits from 291 adult dry skulls and from 60 CT scans were analyzed to measure the distance between the SMRL and the SOF or the inferior orbital fissures (IOF) as well as its height, width and orientation. Processes other than SMRLs were also recorded. Fetal skulls were observed for comparison with adult samples.
RESULTS: Forty-one per cent of orbits on

BACKGROUND: The base of the skull, particularly the pterygomaxillary region, has a sophisticated topography, the morphometry of which interests pathologists, maxillofacial and plastic surgeons. The aim of the study was to conduct pterygomaxillary morphometrics and test relevant hypotheses on sexual and laterality-based dimorphism, and causality relationships.
MATERIALS AND METHODS: We handled 60 dry skulls of adult Asian males (36.7%) and females (63.3%). We calculated the prime distance D [prime] for the imaginary line from the maxillary tuberosity to the midpoint of the pterygoid process be

Pterygomaxillary dysjunction with an osteotome is commonly used to mobilise the maxilla during Le Fort I osteotomy, despite the risk of serious complications. Different instruments and positions have been tried, including dysjunction through the tuberosity. Down fracture by digital pressure alone has also been advocated, but to our knowledge has not been widely adopted. We have therefore reviewed published papers to see if there is any clinical or anatomical evidence for the use of osteotomes to mobilise the maxilla vertically during a Le Fort I osteotomy for either pterygomaxillary dysjunctio

The correct radiographic identification of ossification of the pterygospinous and pterygoalar ligaments plays an important role in surgical procedures for the treatment of trigeminal neuralgia. Most of these procedures are performed through the foramen ovale, a site where these ligaments can be found to be partially or completely ossified. We studied the radiographic features of these ossified ligaments and their location in relation to the foramen ovale by the Hirtz axial technique. For this purpose, 93 dry skulls from the Discipline of Anatomy, São José dos Campos Dental School, UNESP, which

Percutaneous procedures for treating trigeminal neuralgia involve the penetration of foramen ovale using cutaneous landmarks and radiological guidance. The placement of radiopaque markers over the commonly used "zygomatic points" in front of the ears provides landmarks on intraoperative radiological studies that facilitate localization of the foramen ovale. Speed and safety of percutaneous trigeminal procedures should be enhanced. There is no increase in radiation exposure over that in other commonly used approaches.
