Optic nerve course and its association with lateral lamella: relevance for endoscopic sinus surgery.

PURPOSE: To investigate the association between the anatomical course of the optic nerve within the sphenoid and posterior ethmoid sinuses (Delano classification) and anterior skull base morphology, particularly lateral lamella length and olfactory fossa depth. METHODS: Paranasal sinus CT scans from 192 patients (384 hemi-sinuses) were retrospectively reviewed. The course of the optic nerve was categorized using the Delano classification (types 1-4). Lateral lamella length and Keros classification were assessed for each case. Statistical comparisons were performed using ANOVA, post-hoc Tukey

Paranasal Sinus Visualization Capabilities of a Novel Articulating Rigid-Flexible Endoscope: A Cadaveric Study.

INTRODUCTION: Rigid endoscopes have allowed surgeons to safely perform endoscopic sinus surgery. However, their rigid nature creates inherent visualization limitations. The study herein looks to assess the visualization potential of a novel articulating rigid-flexible endoscope when compared with traditional rigid and flexible nasal endoscopes. METHODS: A new articulating endoscope capable of bending its distal tip and extending beyond the point of angulation was compared with 0- and 30-degree 4.0-mm rigid endoscopes, and a 3.7-mm flexible nasopharyngoscope in their ability to visualize prede

Gene expression profiles in human nasal polyp tissues studied by means of DNA microarray.

BACKGROUND: Nasal polyposis (NP) is a chronic inflammatory disease of the sinuses. Its pathogenesis is unknown. DNA microarray analysis allows simultaneous measurement of expression of thousands of genes in the same tissue sample and might help to identify gene alterations in various disorders. OBJECTIVE: We sought to screen for disease-related genes in NP by using DNA microarrays and to validate the altered expression of selected genes at the mRNA and protein level. METHODS: Expression microarrays containing approximately 10,500 genes were used to compare individual gene profiles of NP samp

Paradoxical evolution of spheno-orbital meningioma after cessation of progestin treatment.

BACKGROUND: Meningioma is the most frequent primary benign intracranial tumor, with a higher incidence in women. Treatment with progesterone acetates, including cyproterone, nomegestrol, chlormadinone, promegestone, medrogestone, and medroxyprogesterone acetate, has been identified as a risk factor of meningioma, particularly in the anterior and middle cranial fossae. Discontinuation of these treatments often leads to volume stabilization or regression of the tumor. CASE DESCRIPTION: A 42-year-old woman undergoing treatment with nomegestrol acetate (NA) presented with headaches and visual los

Sphenoid sinus inverted papilloma: a case report and literature review.

Introduction Inverted papilloma is a rare benign tumor of the nasal fossa, which usually originates from its lateral wall. Only 5% of cases demonstrate exclusive sinus involvement. Primary sphenoid sinus involvement is even rarer. Although considered a benign lesion, the tumor has a potentially invasive nature and has also been found to have an associated malignancy rate of 7 to 15%. Objectives To report a case of inverted nasal papilloma originating in a rare location: the sphenoid sinus. Resumed Report a 56-year-old woman, presented to our outpatient clinic complaining of frontal headache, o

The pneumatization patterns of the roof of the parapharyngeal space in CBCT.

INTRODUCTION: The rhinopharynx and the parapharyngeal space (PPS) are complex anatomical territories located beneath the skull base. Thorough knowledge of the complex anatomy of the PPS is essential in treatment of pathologies such as parapharyngeal abscesses. The roof of the PPS is overlooked in anatomy. It was hypothesized that the pneumatization pattern of the PPS roof is individually variable, as determined by the variable pneumatization patterns of the sphenoid and temporal bones. The study was aimed at assessing the anatomy of the PPS roof in CBCT. METHODS: The present study was perform

Radiological aspect of fungus ball within a mucocele of the sphenoid sinus.

Paranasal sinus fungus ball is within the non-invasive forms and is characterized by the presence of aggregated hyphae that do not invade the sinus mucosa. Mucoceles are benign, expansile, cyst-like lesions of the paranasal sinuses. The mucoid secretions of mucoceles are usually sterile. However, secondary infections, mostly bacterial, may lead to the development of pyocoeles. Although an association between a fungus ball and a mucocele is rare in the paranasal sinuses, this disease entity should be considered in the differential diagnosis of expansile, cystic sinus lesions. In this article, c

High-field magnetic resonance imaging of paranasal sinus inflammatory disease.

Magnetic resonance imaging using a 1.5 tesla magnet and a spin echo technique has revealed a remarkably intense signal from abnormal tissue in the human paranasal sinuses. Inflammatory disease in the maxillary, sphenoid, ethmoid, and frontal sinuses has been detected and demonstrated with greater clarity than any other available technique. The pathophysiologic basis for the intense signal has not been defined. These observations do, however, provide an opportunity to discover, clarify, and study paranasal sinus disease. Acute upper respiratory disease, allergic episodes, and the effect of drug

Side- and Sinus-Specific Relationships between Chronic Rhinosinusitis and Ischemic Stroke Using Imaging Analyses.

Recent studies have reported chronic rhinosinusitis (CRS) as an independent risk factor for stroke. However, the association with stroke depending on the affected sinuses has not been explored. This study aimed to elucidate the side- and sinus-specific relationship between CRS and ischemic stroke through imaging analyses. We retrospectively reviewed the medical records of patients who were diagnosed with ischemic stroke at a tertiary center. CRS was defined as having a total score of greater than or equal to 4, according to the Lund-Mackay scoring system, through brain magnetic resonance imagi

Successful treatment of mucormycosis after kidney transplantation.

Fungal infections are rare but represent serious complications following organ transplantation. We present a case of mucormycosis primarily affecting the paranasal sinuses in a 51-year-old man with a kidney allograft. The patient presented with headache, left facial and orbital pain, nasal discharge, and elevation of serum creatinine 18 months after kidney transplantation. Laboratory tests revealed cyclosporine nephrotoxicity, cytomegalovirus infection, and prediabetes. Imaging findings were compatible with left maxillary, ethmoidal, and sphenoidal sinusitis. Diagnosis was made based on pathol

Semi-Automated Segmentation on Computed Tomography Datasets for Three-Dimensional Reconstruction and Volumetric Analysis of the Paranasal Sinuses in Alpacas (Vicugna pacos) and Their Anatomical Variations.

Understanding the anatomy of alpaca (Vicugna pacos) paranasal sinuses is essential for accurate interpretation of diagnostic imaging and surgical planning, especially given the species' susceptibility to dental disease and associated sinusitis. However, detailed anatomical and volumetric data on alpaca sinuses remain limited. This study aimed to characterise the volumetric properties and anatomical composition of the paranasal sinuses in alpacas using semi-automated segmentation of computed tomography datasets, with the goal of supporting clinical practice in veterinary medicine. Seven adult a

Comparison of Blood and Tissue Eosinophil Count and Blood IgE in Patients with Chronic Sinusitis and Nasal Polyps.

BACKGROUND: The inflammatory mucosa of the sinus cavities is called sinusitis and is divided into various types based on its appearance and sign. Chronic rhinosinusitis is an inflammatory-infectious disease that involves the frontal, sphenoid, ethmoid, and maxillary sinuses. Chronic sinusitis is a multifactorial disease and the range of causes varies from environmental factors to genetic factors. The purpose of this study was to compare blood and tissue eosinophils and serum IgE levels in patients with chronic sinusitis with nasal polyp in Vali-e-Asr hospital in 1397. METHODS: In this descrip

CT and MRI features in Bipolaris fungal sinusitis.

Bipolaris is an increasingly recognized cause of fungal sinusitis. Reports of imaging features are sparse. Our purpose was to review the imaging features in patients with Bipolaris fungal sinusitis. A review of our data showed seven patients with culture-proven Bipolaris fungal sinusitis. Computed tomography of the paranasal sinuses in all the patients and MRI in five patients were analysed for the location, nature, extent of the disease and density/signal characteristics on CT/MRI. The sphenoid and posterior ethmoid sinuses were most often involved (six of seven), followed by the anterior eth

Bilateral Cavernous Sinus Thrombosis Secondary to Dental Infection-Induced Sinusitis: A Case Report.

Cavernous sinus thrombosis (CST) is a rare but severe condition that often arises as a complication of head and facial infections. Sinusitis, particularly involving the sphenoid or ethmoid sinuses, is a common precursor. A five-year-old boy presented to the pediatric intensive care unit with fever, ear pain, and eyelid swelling. Magnetic resonance imaging revealed rare bilateral CST with significant intracranial arterial narrowing. Cultures from the ear grew Fusobacterium necrophorum. Subsequent dental examination identified multiple untreated carious lesions, pulpal necrosis, and abscesses. C

Proximal occlusion for infectious carotid-cavernous fistula caused by invasive fungal sinusitis: illustrative case.

BACKGROUND: Thirteen cases of fungal cavernous-carotid aneurysms (CCAs) attributable to invasive fungal sinusitis have been reported in the literature. These reports included only 1 case of carotid-cavernous fistula (CCF). In this report, the authors present a case of CCF attributable to the rupture of an infected CCA caused by invasive fungal sinusitis that required proximal occlusion for treatment. OBSERVATIONS: A 56-year-old man with poorly controlled diabetes experienced a sudden loss of vision in his left eye. Cranial imaging revealed invasive sphenoid sinusitis and CCF attributable to a…
