[A retroperitoneal abscess after endoscopic sphincterotomy: a severe and infrequent complication].

We report the case of a 55-year-old female patient, previously cholecystectomized, who was admitted to the Hospital because of recurrent biliary abdominal pain, cholestasis and common bile duct enlargement by ultrasonography. 48 hours after an endoscopic sphincterotomy the patient developed abdominal pain and fever. CT scan demonstrated air and an infected liquid collection in the retroperitoneum. Although a percutaneous drainage was attempted, the persistence of the symptoms and the low drainage outflow of the infected material made surgical treatment necessary. Duodenal perforation in endosc…

Needle-knife sphincterotomy in a tertiary referral center: efficacy and complications.

BACKGROUND: The use of needle-knife sphincterotomy as a method of precut sphincterotomy has been criticized as potentially unsafe. Despite this, a number of tertiary referral centers have reported their successful use of this technique to increase the rate of common bile duct cannulation. METHODS: We assessed the safety and efficacy of needle-knife sphincterotomy in 72 consecutive patients in whom attempts at standard common bile duct cannulation were unsuccessful. Bile duct diameters were correlated to the complication rate. RESULTS: Cannulation of the common bile duct was successful immedi…

Removal of stones from the bile duct at ERCP without sphincterotomy.

The main risks associated with endoscopic stone removal arise from the sphincterotomy that is performed to facilitate stone extraction. The complication rate may be higher when the bile duct is not dilated. Between January 30, 1990, and March 30, 1993, we attempted to remove stones up to 8 mm in diameter through the intact papilla, without performing sphincterotomy, in 24 patients. Nine patients underwent balloon dilation of the sphincter or of a low duct stricture to facilitate stone removal. All patients were treated successfully and are well at follow-up. Two patients (one having had balloo…

[Current results of endoscopic sphincterotomy for lithiasis of the common bile duct].

Between January 1989 and June 1990, endoscopic sphincterotomy was performed in 308 consecutive patients with common bile duct stones (mean age: 74 years). Complete clearance of common bile duct was achieved at the first attempt in 65% of cases. This rate was significantly related to the size and the number of biliary stones. The success rate reached 97 percent after repeated endoscopic sessions (127 patients), mechanical lithotripsy (20 patients), extracorporeal or intracorporeal lithotripsy (18 and 11 patients, respectively). During the month following the endoscopic sphincterotomy, 39 patien…

Endoscopic sphincterotomy in the management of postoperative complications of hepatic hydatid disease.

The results obtained using endoscopic sphincterotomy (EST) in the management of postoperative biliary complications of hepatic hydatid disease are presented. The outcome was favourable in all eight patients treated with no associated morbidity or mortality. In three patients the clinical manifestations of cholangitis resolved, and in five of six patients the external bile fistula previously present for 26 to 68 days closed within 3 to 15 days. In one patient with a long-standing fistula (5 years) the bile outflow was reduced from 600cc to 50-100cc per day. No complications due to EST were enco…

Endoscopic sphincterotomy for bile duct stones in patients with intact gallbladders.

Endoscopic sphincterotomy was performed in 121 patients (age ranged 34-92 years; median 80 years) with intact gallbladders and bile duct stones. Clearance of the duct by basket or balloon extraction was attempted in 97 patients (80 per cent) and achieved in 93 of these patients (96 per cent). In 24 patients the duct was left to empty spontaneously and this occurred in 22 patients. Immediate complications due to endoscopic sphincterotomy occurred in five patients (two haemorrhage, two perforation, one haemorrhage and perforation). Of the 101 patients reviewed 12-72 months (median 24 months) aft…

Choledochoduodenostomy, transduodenal sphincteroplasty and sphincterotomy for calculi of the common bile duct.

The clinical features, indications and operative results in a consecutive series of 246 patients who underwent choledochoduodenostomy (CDD) and transduodenal sphincterotomy or sphincteroplasty (TDS) over a period of ten years (1972 to 1981) were reviewed. The over-all mortality for CDD was ten of 190 patients (5.3 per cent) and three of 56 patients for TDS (5.4 per cent). The over-all major morbidity rate for these procedures was 11.6 per cent for CDD and 21.4 per cent for TDS. This was not a statistically significant difference, but there were more elderly patients (more than 65 years old) in…

Combined supraduodenal and transduodenal exploration of the common bile duct with sphincterotomy.

From a consecutive series of 2,221 cholecystectomies, 177 patients underwent combined supraduodenal and transduodenal exploration of the common bile duct and 390 were treated by supraduodenal exploration alone. The mortality rate after both exploration procedures was the same (2.8 per cent). However, two patients died of septic complications associated with duodenal fistulas in the combined exploration group. Five patients had retained stones in the common bile duct which subsequently flushed out with a saline solution irrigation administered through the T tube in three patients and the stone

An evaluation of performance after informal training in endoscopic retrograde sphincterotomy.

Endoscopic retrograde sphincterotomy (ERS) has become the treatment of choice for certain disorders of the biliary tract and pancreas. Increased need for ERS has led experienced endoscopists to seek advanced training in this highly skilled procedure, and has resulted in the development of guidelines from the American Society for Gastrointestinal Endoscopy regarding appropriate experience, skill, and training before performing ERS. This report describes the process by which an endoscopist skilled in endoscopic retrograde cholangiopancreatography, but with no prior training in ERS, acquired appr…

[Analysis of 278 sphincterotomies of the urethral stria in 232 male patients].

The authors report 278 external sphincterotomies performed on 232 male neurological patients, presenting with specific etiologies requiring surgery: vesico-ureteral reflux (33.6%), ureterohydronephrosis (18.9%), and spontaneous hyperreflexia (7.4%). The level of the spinal cord injury was superior to T8 in 32.3% of cases (double the corresponding figure for paraplegic patients) and inferior to L1 in 31.2% (25% more than for paraplegics). Short-term results are good in 70% of cases, in terms of dysuria and infection. Iterative sphincterotomy is required in 19.8% of cases, and surgery of the nec…

Subcutaneous lateral internal sphincterotomy in the treatment of chronic anal fissure.

One hundred and seventeen patients with chronic anal fissure were submitted to subcutaneous internal lateral sphincterotomy. Immediate pain relief was observed in all patients except two, in whom sphincterotomy was incomplete. One hundred and ten patients had their first painless bowel movement 24 to 48 hours after the operation. Temporary incontinence to flatus was reported by 38 patients, whereas fecal incontinence was never observed. Healing time of the wound was six days, that of the fissure four weeks. In the 98 patients followed at six months and one year, no complications or recurrences…

British experience with duodenoscopic sphincterotomy for removal of bile duct stones.

Duodenoscopic sphincterotomy is rapidly becoming popular in Britain. Representatives of 14 British centres met in January 1980 to discuss progress and problems with the technique. This report summarizes current experience, with particular reference to hazards. Duodenoscopic sphincterotomy is mainly being used in patients who have previously undergone cholecystectomy and who no longer have a T tube drain in place. Sphincterotomy was achieved in 87 per cent of 679 patients attempted, and the common duct was cleared of stones in 87 per cent of these. Immediate complications followed in 8.5 per ce…

Sphincterotomy and postsurgical mortality after operations on the common bile duct.

In the light of an analysis of 692 operations on the common bile duct performed at the Department of General Surgery, Bielański Hospital in Warsaw, the author evaluated the postoperative mortality after these operations. Association of sphincterotomy with other procedures on the bile ducts does not increase the postoperative mortality, but the causes of death in cases with incision of the sphincter of the hepatopancreatic ampulla are connected mainly with the operation while in the remaining operations these causes are connected with more general factors. Acute postoperative pancreatitis takes…
