Trigemino-cervical reflex in spinal cord injury.

Abnormal enhancement of polysynaptic brainstem reflexes has been previously reported in patients with spinal cord injury (SCI). We aimed to investigate trigemino-cervical reflex (TCR) in SCI since it may reflect alterations in the connections of trigeminal proprioceptive system and cervical motoneurons. Consecutive 14 patients with SCI and 16 healthy subjects were included in this study. All patients were in the chronic phase. TCR was recorded over sternocleidomastoid (SCM) and splenius capitis (SC) muscles by stimulation of infraorbital nerve. We measured onset latency, amplitudes and duratio

Pharmacological approaches to chronic spinal cord injury.

Although research on neural tissue repair has made enormous progress in recent years, spinal cord injury remains a devastating condition for which there is still no cure. In fact, recent estimates of prevalence in the United States reveal that spinal cord injury has undergone a five-fold increase in the last decades. Though, it has become the second most common neurological problem in North America after Alzheimer's disease. Despite modern trauma units and intensive care treatments, spinal cord injury remains associated with several comorbid conditions and unbearable health care costs. Regular

Salmonella prostatitis in a man with spinal cord injury.

CONTEXT: Prostatitis is a very unusual manifestation of Salmonella urinary tract infection and has not been reported in men with spinal cord injury (SCI). FINDINGS: A 57-year-old man with paraplegia and a history of recurrent symptomatic urinary tract infections presented with Salmonella typhimurium prostatitis. Clinical and sonographic examination of the urinary tract, as well as urinalysis including microbiologic examination, revealed no relevant abnormalities. The microbiologic analysis of the ejaculate revealed growth of monophasic Salmonella enterica ssp. enterica serotype 4,12:i:-. A 6-

Baroreflex function after spinal cord injury.

Significant cardiovascular and autonomic dysfunction occurs after spinal cord injury (SCI). It is now recognized that cardiovascular disease is a leading cause of morbidity and mortality in SCI. Patients with SCI may also suffer severe orthostatic hypotension and autonomic dysreflexia. Baroreflex sensitivity (i.e., the capability of the autonomic nervous system to detect and respond effectively to acute changes in blood pressure) has been recognized as having predictive value for cardiovascular events, as well as playing a role in effective short-term regulation of blood pressure. The purpose

Nerve transfer strategies for spinal cord injury.

BACKGROUND: Spinal cord injury (SCI) is a devastating condition, which beleaguers its victims with long-term health issues. Nerve transfer is a feasible option for restoration of critical limb function in patients with SCI that potentially improves independence and quality of life. METHODS: This article delineates the general principles of nerve transfer and its specific application pertinent to SCI. The available nerve transfer strategies are described based on the targeted limb function, mostly involving critical upper extremity function. The role of nerve transfer for paraplegia, diaphragm

Penetrating injury of the spinal cord treated surgically.

A 44-year-old man reported acute neck pain, hyperalgesia, and mild paraplegia caused by a wire-penetrating injury of the neck and was hospitalized. The foreign body was located at the C6 level. Pre- and postoperative physical examination and imaging studies were performed to assess the degree of injury. Emergent surgery was performed 30 minutes after admission to prevent the patient from potential severe neurological impairment and infection. The patient's postoperative recovery was significant. Only hyperalgesia at the ulnar side of the forearm remained, with no other positive signs of neurol

Vascular dysfunctions following spinal cord injury.

The aim of this article is to analyze the vascular dysfunctions occurring after spinal cord injury (SCI). Vascular dysfunctions are common complications of SCI. Cardiovascular disturbances are the leading causes of morbidity and mortality in both acute and chronic stages of SCI. Neuroanatomy and physiology of autonomic nervous system, sympathetic and parasympathetic, is reviewed. SCI implies disruption of descendent pathways from central centers to spinal sympathetic neurons, originating in intermediolateral nuclei of T1-L2 cord segments. Loss of supraspinal control over sympathetic nervous sy

Supernumerary phantom limbs in spinal cord injury.

STUDY DESIGN AND OBJECTIVES: Case report and review of supernumerary phantom limbs in patients suffering from spinal cord injury (SCI). SETTING: SCI rehabilitation centre. CASE REPORT: After a ski accident, a 71-year-old man suffered an incomplete SCI (level C3; AIS C, central cord syndrome), with a C3/C4 dislocation fracture. From the first week after injury, he experienced a phantom duplication of both upper limbs that lasted for 7 months. The supernumerary limbs were only occasionally related to painful sensation, specifically when they were perceived as crossed on his trunk. Although the

Neuronal dysfunction in chronic spinal cord injury.

This review describes the changes of spinal neuronal function that occur after a motor complete spinal cord injury (cSCI) in humans. In healthy subjects, polysynaptic spinal reflex (SR) evoked by non-noxious tibial nerve stimulation consists of an early SR component and rarely a late SR component. Soon after a cSCI, SR and locomotor activity are absent. After spinal shock; however, an early SR component re-appears associated with the recovery of locomotor activity in response to appropriate peripheral afferent input. Clinical signs of spasticity take place in the following months, largely as a

Colonoscopic lesions in patients with spinal cord injury.

BACKGROUND/OBJECTIVE: In spinal cord injury (SCI), loss of central or peripheral neural control causes neurogenic bowel. Patients may not exhibit the typical signs and symptoms of gastrointestinal disease. Few studies have looked at the risk of gastrointestinal disease in this group and the indications for preventive screening. The objective of this study was to study colonoscopic lesions in patients with SCI and determine whether there are any differences in the prevalence of lesions between SCI and control patients. DESIGN: Case control study. METHODS: Twenty-five patients with SCI were co

Tracheostomy timing in traumatic spinal cord injury.

The study conducted is the retrospective study and the main objective is to evaluate the benefits and safety of early versus late tracheostomy in traumatic spinal cord injury (SCI) patients requiring mechanical ventilation. Tracheostomy offers many advantages in critical patients who require prolonged mechanical ventilation. Despite the large amount of patients treated, there is still an open debate about advantages of early versus late tracheostomy. Early tracheostomy following the short orotracheal intubation is probably beneficial in appropriately selected patients. It is a retrospective cl

Scapular stabilization in patients with spinal cord injury.

BACKGROUND: Severe scapular instability can be a considerable problem for people with high-level cervical spinal cord injury. Scapular instability reduces the effectiveness of the already weakened shoulder flexors and abductors, thereby limiting hand-to-mouth and hand-to-head activities. The winged scapula may cause inferior pole skin breakdown, as well as neck and shoulder pain. OBJECTIVE: To report the efficacy of a fusionless scapular stabilization procedure as a means to enhance function in a consecutive group of patients with high-level cervical spinal cord injury. METHODS: Four people
